LACK OF FORM CASES
DOCUMENTS REQUIRED: 1. The baptismal record of at least one Catholic party. The record must be of recent issue, within the last six months, and contain all recent additions and notations 2. Marriage record. (For marriages in California: a copy of the full Certification of Vital Record – Certificate of Registry of Marriage is needed as best evidence.) 3. The Final civil decree of divorce, dissolution, or annulment. 4. The Oath form with sworn data. (This Yellow Form)
Please note: At least one party must have been bound by the Canonical form of Marriage at the time of the marriage: that is, baptized and not having left the Church by a formal act. In the case of Orthodox Christians, please consult the Vicar for Canonical Affairs. No Priest, Deacon, or other Minister should send anyone to the Canonical Affairs Office without first consulting the Vicar. In general, no Priest nor petitioner may fix a definite time for a marriage celebration until canonical freedom has been completely established.
