FIFTY YEARS OF PSYCHIATRY IN ITS CONTEXT

The context in which psychiatry is developing exerted a significant impact on its paradigms and the perspectives of its future. Urbanisation, globalisation, emphasis on economic criteria of success and massive population movements with consequent social changes made most of the paradigms of psychiatry defined in the late 20th Century obsolete. This concerned the organization of mental health care, education of health workers as well as the main directions of research in the field of mental health. The lecture will discuss these developements and suggest the outlines of paradigms of psychiatry in the early decades of the 21st Century. Keywords: Psychiatry, Context, Paradigm.

Kontekst u kojemu se psihijatrija razvija imao je značajan utjecaj na paradigme i perspektive njezine budućnosti. Urbanizacija, globalizacija, naglasak na ekonomskim kriterijima uspjeha i masovno kretanje stanovništva s društvenim promjenama koje su slijedile, učinili su zastarjelima većinu paradigmi psihijatrije, koje su definirane u kasnim godinama dvadesetog stoljeća. To se odnosi na načine i modele organiziranja mentalno-zdravstvene skrbi, obrazovanje zdravstvenih radnika, kao i na glavne vodilje u istraživanjima iz oblasti mentalnog zdravlja. Predavanje će razmotriti ta zbivanja i predložiti osnovne crte paradigmi psihijatrije u ranom dvadestprvom stoljeću. Ključne riječi: psihijatrija, kontekst, paradigma.

FUTURE OF PSYCHIATRY: CHALLENGES AND OPPORTUNITIES

Mental disorders are highly prevalent and cause considerable suffering and disease burden all over the world. The public health impact of mental disorders is profound as the estimated disability-adjusted life-years attributable to mental disorders have been shown to be very high. Despite the growing evidence about the impact of mental illnesses, mental health services continue showing big gaps. With less number of mental health professionals, scarcity of mental health resources and now often facing additional problems of migration of trained psychiatrists and mental health professionals to the already resource rich countries, the situation gets even worse. There are also concerns among the professionals that profession is in crisis and that it faces a number of external and internal challenges. Issues regarding diagnosis, treatment, prognosis and outcome of mental health disorders along with de-medicalization of healthcare within mental health services and marginalisation of psychiatrists in service development and organisation are posing questions whether psychiatrists are endangered species. This paper presents an overview about such challenges and their impact on current understanding of some conceptual issues in psychiatry. It will be argued that there is no health without mental health and psychiatry should require innovation, networking and better understanding and orientations about nature and different dimensions of psychiatric disorders. It is hoped that such efforts would aim at promoting psychiatry as a preferred discipline among the medical profession as well as wider respect and acknowledgement from the general public.

NEGATIVE SYMPTOMS IN SCHIZOPHRENIA: ASSESSMENT AND TREATMENT PERSPECTIVES

Negative Symptoms represent core aspect of schizophrenia and predict poor outcome. They do not form an homogeneous symptom cluster and can be intrinsic to schizophrenia, or secondary to other psychopathological dimensions, such as positive symptoms or depression, to antipsychotic side-effects or to social isolation. While effective treatment for primary negative symptoms are not yet available, secondary negative symptoms can be prevented and treated to improve the outcome and quality of life of people with schizophrenia. In the last decade, a large consensus has been achieved concerning the construct and assessment of negative symptoms. New instruments were developed to better evaluate these symptoms, reducing the overlap between them and other dimensions of impairment in people with schizophrenia. However, in clinical settings, psychiatrists are less skilled in the evaluation of negative symptoms than of other symptom dimensions.
