Health IT Vendors in CPC+
Comprehensive Primary Care Plus (CPC+) is a national advanced primary care medical home model that aims to strengthen primary care through regionally based multipayer payment reform and care delivery transformation. CPC+ practices are required to meet specific health IT requirements that differ by track.
In order to support Track 2 practices, health IT vendors must provide the Centers for Medicare & Medicaid Services (CMS) with Letter(s) of Support [LOS(s)] and sign a Memorandum of Understanding (MOU). Vendors may submit an individual LOS for each Track 2 practice they are supporting, or a Global LOS if they are supporting multiple practices.
Three tables below provide information on health IT vendors who have signed MOUs to support Track 2 CPC+ practices; on other health IT vendors participating in CPC+; and on those interested in participating.
• Table 1 lists health IT vendors that have provided CMS with a Global LOS and signed an MOU as part of supporting Track 2 practices. The “Vendors with Signed MOU” column contains links to vendor websites, while the “Global LOS” column includes links to their letters. Practices seeking a vendor to support their participation in CPC+, especially Track 2, may be interested in reviewing this list for potential partners.
• Table 2 names other health IT vendors that currently support Track 1 CPC+ practices. Vendors in this table may offer one or more of the health IT functions required for CPC+, however they have not signed an MOU to support Track 2 practices.
• Table 3 includes health IT vendors that have expressed interest in supporting CPC+ practices, but are not doing so currently.
Please note that this document includes vendors who voluntarily submitted contact information to CMS. CMS does not endorse any vendor included in these tables, nor does inclusion in this document confirm that a specific vendor’s products will meet the CPC+ health IT requirements. Practices should reach out to vendors directly to learn more about their product.
