District Nurses And Health Visitors Shropshire County Primary Care Trust employs community nurses and health visitors who work alongside us with our patients. They are based at the Oswestry Health Centre. Contacting the Health Visitors To speak to a health visitor or to make an appointment for the baby clinic, please ring 01691 663608. NB: health visitors are no longer situated in our surgery or hold clinics here. Health visitors also facilitate groups in the local area, including parentcraft, postnatal group, postnatal depression group, weaning sessions, baby massage and child behaviour courses. Contacting the District Nurses If you are housebound you can arrange to be seen by one of our district nurses by contacting them directly between 9.00am - 5.00pm seven days a week on 01691 663609. You do not need to be referred by a doctor. The district nurses assess for care required and can provide medical-based nursing skills and advice to people in their own homes, which can include: • Wound care • Leg ulcer care • Bladder and bowel care • Continence assessment • Chronic disease management - eg diabetes, coronary heart disease • Terminal disease management - care for people near the end of their life, including support for their family • Wheelchair/equipment assessment • Advice and support for carers

Surgery Hours The surgery is open between 8.15am - 6.00pm, Monday to Friday, except Thursdays when it is open between 8.15am - 5.00pm. The phones are switched over at 8.00am each morning, Monday to Friday. Saturday morning surgery is for pre-booked appointments only and for patients who would find it difficult to attend Monday to Friday because of work commitments etc.

Appointments Nurse Assessment System When phoning for a GP appointment, call on the day the appointment is required. The receptionist will ask for a brief description of your current illness or reason for the call, this ensures they can direct you to the most appropriate service. Nursing and phlebotomy appointments can be pre booked. There are a variety of appointment types and the receptionist will offer you the most appropriate type.

How To Register As A Patient Firstly, please check you are within our practice boundary (see map of practice area). The receptionist can assist you to complete the necessary forms. It would be helpful if you have your NHS number (which is on your medical card) with you. If you can't find your medical card don't worry as we can still register you. We will try to help you see the doctor of your choice but this is not always possible. When you do register, you will be asked to complete a simple health questionnaire which gives us routine information about your past medical history. You will also be invited to make an appointment with one of our nursing team for a New Patient Healthcheck. Temporary Residents You can see a doctor anywhere in the UK if you are away from home and in need of medical help. You can do this simply by asking to see the nearest doctor as a temporary resident. We will be happy to see any relatives or friends staying with you if they happen to need a doctor. Please bring with you some form of photographic identification if you wish to register temporarily.

How To Make An Appointment Please remember to make a separate appointment for each person to be seen. Please tell reception if you need a longer than usual appointment for additional discussion or a medical (standard consultations are 10 minutes). It helps to be as specific as possible when asking for an appointment. This allows reception staff to find the most appropriate care for you - a phone call or a nurse consultation may be faster and more appropriate than waiting to see a doctor face to face. If you cannot keep an appointment please telephone us on 01691 655844 and let us know. The doctors will sometimes run late. Please be patient if you are asked to wait: you may need the longer consultation next time.

How To Obtain Test Results Routine blood, urine and swab tests take about four days and cervical smears and x-ray reports take about two weeks. The telephone number for results is 01691 655844. Please ring between 2.00 and 5.00pm. For reasons of confidentiality, results will only be given to patients themselves, or parents of young children, so please expect to be asked for confirmatory details.

Patient Confidentiality And Data Protection We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. If you wish to view your own patient records, please put your request in writing to the practice manager.

Out-Of-Hours Service Shropshire Clinical Commissioning Group is responsible for commissioning GP services outside normal surgery hours. Out-of-hours services are provided between 6.00pm and 8.00am the following day during the week, and at weekends from 6.00pm on Fridays through to 8.00am on the following Monday. Bank Holidays are also treated as out of hours.

A call handler will ask for your details and your reason for calling (telephone calls may be recorded). A doctor or nurse will then contact you:

• to give advice - it is often possible to sort out problems over the phone.

• to arrange an appointment with the doctor at the Primary Care Centre. If you attend without an appointment you may not be seen.

• to arrange a home visit if you need to be seen and are not fit enough to travel eg very elderly or bedbound patients, terminal care cases. Please note: if your problem is not urgent, please wait until the surgery next opens. If you are not sure how urgent your problem is, ring NHS 111 on 111 for advice. Please remember: in very urgent cases or if there is a problem reaching the doctor and you feel you cannot wait, call an ambulance on 999.

Plas Ffynnon Doctor Visiting Guidelines

The following system has been devised along the lines of a traffic light system to try to improve access to GP services for all our patients. To ensure that the best use is made of your GP services please attend the surgery wherever possible. If you really require a home visit please call as soon as possible in the day - the doctors do their visits over lunchtime and late visit requests can be very disrupting, although we do appreciate it is not always easy to predict.

GREEN LIGHT (visit recommended) For terminally ill patients, and the truly housebound.

AMBER LIGHT (visit may be useful) On occasions where, after consultation with a health professional, it is agreed that a seriously ill patient may be helped by a visit.

RED LIGHT (visit is not usual or useful) In most of the following cases a visit would not be an appropriate use of your doctor’s time: Common illnesses such as most childhood illnesses, abdominal pain, headaches, coughs and diarrhoea/vomiting. In these cases patients are usually well enough to travel. More accurate diagnosis is possible in the surgery. Please remember it is not harmful to wrap up a child with a fever and take them to the surgery to be seen. If you do not have a car there are usually friends, family, neighbours or taxi services able to help.

If you are off between four and seven days (inclusive) you should complete a self-certification form (available from the DSS, your employer or ask a receptionist). You may ask your doctor to provide you with a private certificate but a charge will be made for this service. If you are off for eight or more days, your doctor will complete the appropriate medical certificate for which there is no charge.

Dispensary If you live outside Oswestry, Gobowen and Morda, you may obtain all your medicines from our dispensary. The dispensary is open from 8.30am - 6.00pm Monday to Friday (except on Thursday when it closes at 5.00pm).

How To Obtain Repeat Prescriptions If you are taking regular medication then you will receive a repeat prescription slip. Non-NHS Medical Services Certain services provided by the practice are not covered by the NHS and you will be asked to pay a fee. These services include: • Insurance claim forms • Private sick notes • BUPA/PPP forms • Various special letters and forms • Some travel vaccinations • Pre-employment medicals • Taxi, HGV and PSV medicals • Solicitors' reports • Holiday cancellation letters If you would like to make an appointment for a medical examination, ask the receptionist to make an appointment. We carry out private medical examinations on Wednesday mornings. A price list is available at the reception desk. Access For The Disabled We have a large car park with dedicated spaces for disabled drivers. The building is designed to be easily accessible to people in wheelchairs. We have a dedicated WC (including baby-changing facilities). For the benefit of those people who are hard of hearing, the public address system is fitted with an induction loop. Most of our ground floor door signs have a Braille identification panel. We are happy to allow guide dogs (but no other pets please) into the building. The Car Park We ask you to use our car park responsibly. Park here when you visit the surgery or dispensary. The car park is now monitored by Parking Eye, there is no charge for patients who are attending the surgery, you will need to input your car registration at check-in or reception, and you have 1 ½ hours free parking. If you wish to make use of the car park at weekends and evenings then you need to pay and display. Fines are issued (which do not go to the practice) if you park without paying or inputting your car registration. Breast Feeding You are welcome to breast feed your baby when visiting the surgery, please feel free to do so. If you need privacy, please speak to a receptionist. You can expect an acceptable standard of cleanliness and warmth in a comfortable environment.
