Glaucoma has for ages been an enemy and thief of sight, and our understanding of Glaucoma has come a long way from the earliest notions of the condition. The word glaucoma originally meant clouded in Greek; as such, it may have referred both to a mature cataract and to corneal oedema that might result from chronic elevated pressure. Now, glaucoma is a word that does not refer to a single disease entity, but rather, a group of diseases that differ in their clinical presentation, pathophysiology and treatment. The definitive characteristics of glaucoma are based on visual field loss or the appearance of the disc or retinal nerve fibre layer (RNFL). Thus, visual Dr. V. S. GUPTA (M.S. Ophthal.) (2) Glaucoma (both open-angle and angle- closure) is the second leading cause of blindness worldwide, with approximately 8.4 million people blind from glaucoma. There are approximately 11.2 million persons aged 40 years and older with glaucoma in India. (3) Primary open angle glaucoma is estimated to affect 6.48 million persons. (3) Glaucomatous visual changes and defects are irreversible because of retinal ganglion nerve cell damage. Due to the irreversible nature of glaucomatous defects, current strategy aims at early detection through screening, and further detailed examination of 'at risk' suspects. Although not all patients with elevated intraocular pressure(IOP) develop glaucoma, reduction of elevated IOP is the only as yet proven approach to protect against visual field loss in patients with primary open-angle glaucoma (POAG).(4),(5) When you may develop glaucoma depends on the level of IOP your optic nerve may tolerate without getting damaged. The changes of primary open angle glaucoma are noticed by the patient after significant disease progression due to a relative lack of attendant alerting symptoms. These factors make glaucoma the 'sneak thief' of vision. No symptoms. No pain. Normal vision. Without treat ment, people with glaucoma will slowly lose their peripheral vision. As glaucoma remains untreated, people may miss objects to the side and out of the corner of the eye and may seem to be looking through a tunnel. Over time central vision may decrease until no vision remains. Hence the need for early diagnosis, prompt treatment and lifelong compliance. Glaucoma is detected by means of a detailed eye examination including your visual acuity, visual field testing for peripheral vision, dilated eye examination for inspecting the optic nerve for any signs of damage, tonometry for measuring your IOP and pachymetry for measuring your central corneal thickness. A gonioscopy may rule out any occludable angles as the treatment for angle-closure glaucoma is different from that of open-angle glaucoma. Lastly,an optical coherence tomohraphy (OCT) for your retinal nerve fibre layer (RNFL) is included in your baseline investigations so that progress or control of glaucoma may be gauged at subsequent follow-up visits. Among the identified suspects, the question arises of appropriate timing to initiate therapy. Therapy once initiated, is likely to continue for the rest of the patients' natural life, constituting a substantial burden on the patient, physically, mentally and financially. In general, the overall goal to manage glaucoma patients is to preserve visual function while maintaining the best quality of life. This can be achieved by preventing or slowing the progression of glaucomatous damage by lowering IOP to a level at which further damage is minimal. For this, there are three management modalities: 1) Medical therapy 2) LASER therapy 3) Surgery Initial medical therapy remains the standard for most of the newly diagnosed patients of glaucoma. This includes use of topical and occasionally orally administered agents that lower IOP by enhancing aqueous flow or reducing production or both. Currently 6 different classes of glaucoma medications are available: i. Prostaglandin – related agents ii. β –adrenergic receptor antagonist ii. Adrenergic receptor agonist iv. Carbonic anhydrase inhibitors iii. Cholinergic or miotic agents vi. Hyperosmotic agents The introduction of topical ß-blockers in the 1970s was a significant advance, and ß-blockers quickly became the most commonly prescribed class of agents to lower intraocular pressure (IOP). They generally were the most effective medication to do so and were well tolerated by most patients. The type of laser therapy and surgery will depend on the form of glaucoma and how severe it is. Selective laser trabeculoplasty (SLT), Argon laser trabeculoplasty (ALT), Micropulse laser trabeculoplasty (MLT) and Laser cyclophotocogulation are the various laser techniques used for POAG. Lasers produce a focussed collimated beam of light intended to produce drainage channel for aqueous. Being done in the outpatient setting, there is hardly any pain during or after the procedure so that patients can resume dsaily activities immediatelt after the procedure. Patient compliance to medication has always occupied a central position in the course of ocular disease. Compliance becomes all the more important in the management of any of the conditions in the glaucoma 'spectrum', owing to the lifelong nature of treatment of an irreversible condition. In light of inherent hurdles to therapy, the aim becomes to try, as far as possible, to keep the patient on as few medications as possible. The drugs mentioned above are all candidates for monotherapy, the exact choice depending on patients' tolerability, drug efficacy, side effect profile, and the clinician's experience with the drug. 4 MAHAVIR VISION - AUGUST 2015 Medical management is usually the first treatment option chosen for most types of open angle glaucoma. The decision to start treatment is one that must be made with complete involvement of the patient, as it is likely to continue for the patient's lifetime. Here arises the role for an agent with sufficient efficacy, minimal side effects, affordable cost, and ease of administration. These factors are important in a country like India where many patients are from low socio-economic strata, and low levels of patient literacy and understanding add significant problems to ensuring patient compliance and follow up.Again, the decision to start treatment is many times not straightforward, and the need for an ideal agent is felt all the more, in light of the adding the burden of side effects and cost to the patient. It is important to recognize that no single medication can be used in all patients in all circumstances. Each of the available drugs has its own unique advantages and disadvantages, and currently six separate classes of anti-glaucoma drugs are available. It is necessary to individualize each patient's treatment regimen to maximize the benefits and limit the undesirable effects. It is not realistic to anticipate using all the available types of drugs for any single patient. Rather, it is vital to select the best agent for each particular patient's needs.Wisdom lies in anticipating the possible course of glaucoma so that the best possible treatment can be offered to the patient.
