Peer Recovery Program 2020 Quarter 2 Report January 1 – June 30, 2020

In This Report In 2020 Quarters 1 and 2, Recovery Specialists received referrals during 8,455 patient visits and provided both in-person and virtual services. After a 43% decrease from February to April, referrals rebounded to pre-COVID-19 levels in May and June. During 11,008 follow-up calls conducted 3, 6, 9, and 12 months post-consultation, 646 patients reported being in recovery, representing 6% of eligible patients and 69% of patients who were successfully contacted.

The 2020 Quarter 2 Report presents information on referrals, the population served, and program outcomes, as well as program updates and a summary of the program’s response to COVID- 19 (see pages 3 and 4). The Appendix contains data stratified by location. The information included in this report is preliminary and subject to change.

Program Background The RWJBarnabas Health (RWJBH) Institute for Prevention and Recovery (IFPR) Peer Recovery Program (PRP) provides recovery support services for individuals with substance use disorder. PRP employs Recovery Specialists who are in long-term recovery from substance use disorder and provide consultation and peer recovery support as well as clinically trained Patient Navigators who link individuals to appropriate levels of care.

PRP, formerly known as the Opioid Overdose Recovery Program (OORP), serves patients 24 hours a day, 7 days a week, in 20 hospitals throughout Essex, Hudson, Middlesex, Monmouth, Ocean, Somerset, and Union counties. In addition to serving emergency department patients who received naloxone to reverse an opioid overdose, PRP also serves patients with substance use disorder in emergency departments of all participating hospitals, inpatient units of RWJBH hospitals, and outpatient settings. Participating organizations are listed in the Appendix.

Referral Summary Recovery Specialists received referrals during 8,455 patient visits in 2020 Quarters 1 and 2. There were 18,585 referrals in 2019, 9,060 referrals in 2018, 1,898 referrals in 2017, and 720 referrals in 2016. Location-specific data is available in the Appendix.

COVID-19 Response Throughout the COVID-19 pandemic, IFPR has continued to operate the Peer Recovery Program with Recovery Specialists staffing RWJBH hospitals 24 hours a day, seven days a week, and providing in-person services with appropriate precautions. PRP staff implemented and, in certain cases, sunsetted policies and procedures to best serve patients and support hospital staff while prioritizing safety and wellbeing.

Services remain phone-based for patients who have tested positive or are awaiting test results for COVID-19 and for patients at non-RWJBH hospitals. PRP implemented weekly remote shifts for Recovery Specialists in April 2020, ensuring one Recovery Specialist remained on-site each shift while reducing potential contacts. When working remotely, Recovery Specialists provide phone-based services for patients who cannot be served in person. Based on a data-driven approach that incorporates PRP referral volume and hospital policy, PRP began phasing out most remote shifts in June 2020. Additionally, personal protective equipment availability is reported at the end of every in-person shift, and a cleaning product inventory is updated weekly.

PRP continues to prioritize self-care and peer-led supervision for staff. In addition to administrative and educative supervision, Recovery Specialists receive supportive supervision through monthly group and individual sessions focused on reflection, self-care, and patient and staff wellbeing. Clinical Health Educators Lauren Olsen and Ruth Palacio also hold three weekly Self-Care Live virtual sessions for IFPR staff. Internally developed self-care newsletters are distributed to IFPR staff via email each week and were made available to all RWJBH staff.

IFPR Recovery Specialists and community-based Case Managers continue to hold twice-daily virtual All Recovery Meetings, which provide social support for those seeking or maintaining recovery, as well as family members and friends.1 All Recovery Meetings now reach approximately 800 attendees per week and 60 attendees per meeting, nearly five times the pre-COVID-19 average.

Program Updates In response to COVID-19 and to ensure that individuals are served without needing to visit a hospital, PRP began piloting phone-based services for individuals in outpatient settings. Services are available for patients identified by RWJBH Medical Group providers and individuals referred from other outpatient or community settings. We now refer to the initial services provided by Recovery Specialists as a consultation rather than a deployment.

To prevent the spread of COVID-19, PRP limited in-person follow-up with patients. With nearly 75% fewer in-person follow-up contacts in Quarter 2 than in Quarter 1, Recovery Specialists may now supplement phone-based follow-up with text messages. Patients are first contacted by phone call and may elect to continue communication via text message. Updated data collection forms reflect this change and future reports will include text message follow-up data.

Please join us in congratulating Angela Cicchino and Nicholas Beam, who were promoted from PRP Supervisors to Managers. In their new roles, Angela and Nicholas will oversee recovery support services and Recovery Specialists at additional hospitals, manage after-hours operations at all hospitals, and take a greater role in executing RWJBH Tackling Addiction Task Force initiatives.

Population Served Of the 8,455 referred patients, over two-thirds were male. Most patients were between the ages of 26-44 (40.6%) and 45-64 (43.1%). The majority (54.1%) of patients were white, 23.2% were Black, and 14.7% were Hispanic/Latino. Nearly two-thirds of all patients were covered by Medicaid or were uninsured, 14.2% were homeless, and 39.8% did not have access to transportation.

Program Outcomes To measure success, the Institute for Prevention and Recovery utilizes the Substance Abuse and Mental Health Services Administration’s (SAMHSA’s) working definition of recovery, which emphasizes the process of change through which individuals improve their health and wellness, live self-directed lives, and strive to reach their full potential.2 Four major dimensions—health, home, purpose, and community—support recovery. PRP assists in the process of recovery by providing recovery support services and clinical referrals.

In 2020 Quarters 1 and 2, 51 patients were not available for consultation (i.e., left against medical advice, was unresponsive, was deceased, etc.). 94.9% of patients available for consultation accepted consultation by a Recovery Specialist. 92.3% of patients who accepted consultation also accepted some form of support services.

Recovery Specialists conduct follow-up with patients for a minimum of eight weeks post- consultation. In 2020 Quarters 1 and 2, PRP staff conducted 81,049 instances of follow-up, including 5,790 face-to-face contacts. Patient Navigators were responsible for approximately one-seventh of all instances of follow-up.

In 2020 Quarters 1 and 2, PRP staff attempted 11,008 of these follow-up calls and 941 patients were successfully contacted. Of these patients, 646 reported currently being in recovery, representing 5.9% of eligible patients and 68.7% of patients who were successfully contacted.
