20102010 HolidayHoliday CampCamp The Holiday Camp is a teaching and instructional camp being offered at Jesuit College Preparatory School in Dallas, TX. NCAA Coaches, NLL players, MLL players and NCAA Division I, II and III players make up a staff dedicated to helping each camper reach their full potential. The intent of the camp is to expose Texas players to a very high level of coaching without having to spend lots of money travelling back to the east coast. Every year we have campers at the high school and youth level from NY, CT and MD, that are visting relatives for the holidays remark that our staff is phenominal. We already have 20 NCAA coaches confirmed for our 2010 holiday camp. As the camp approaches, we will have updates on our current staff in the staff section of the Deuces website. In the past our staff has consisted of coaches and players from Syracuse University, Penn State, Army, Yale, Salisbury State, Limestone College, Washington and Lee, Butler, Brown, Amherst College, Dartmouth, Johns Hopkins, University of Maryland, Bates College, Virginia Military Institute, Skidmore College, Providence College, Dickinson College, Stony Brook University, The Chicago Machine, The Rochester Rattlers, The New Jersey Pride and more. We are committed to providing you the most experienced camp staff possible. We have designed a program that emphasizes individual and positional skills training taught in a fun environment. The program also exposes each Camper to fundamental offensive and defensive team concepts which are integral to a players complete development. Players are broken down by age and, at the youth camp, separate instruction is provided to beginner players with no lacrosse experience. Separate goalie and faceoff instruction will also be provided at this year’s camps. We look forward to seeing you at this year’s Holiday Camp so that we can continue to bring the east coast lacrosse camp experience to you.

1. Permission & Release: I, the undersigned parent/legal guardian of ______(”Camper”) authorize Camper!s participation in the Dallas Deuces Lacrosse Organization, Inc. Holiday Camp and all related games and activities (”Camp”). In and for consideration of Player!s participation in the Camp, I hereby release and forever discharge and agree to indemnify and hold harmless Dallas Deuces Lacrosse Organization, Inc. Deuces Camps, LLC, Jesuit College Prepararoy School, their employees,officers, directors, shareholders, representatives, subsidiaries, affiliates, partners, operators and any or all other individuals and entities associated there with from any claims, suits, liability for damamges, economic or otherwise, sustained by reason of any of any personal injury or death, arising out of, or directly or indirectly occasioned by (i) Player!s participation in the Camp and/or (ii) Camper!s activities upon the premises where the Camp is conducted. In addition, I fully understand the inherent dangers of participating in the sport of lacrosse, including, without limitation, the risks of catastrophic injury, paralysis and even death.

2. Medical Permission: I represent and warrant that Camper has been examined by a physician and found to be in good health and able to compete in all Camp activities without restrictions. Furthermore, I give my consent and permission to the employees of Dallas Deuces Lacrosse Organization, Inc., Deuces Camps, LLC and/or the employees of the Camp to provide, through a medical staff of their choice, any and all medical, transporation and emergency services as warranted to Camper as a result of Camper!s participation in the Camp and I agree to be responsible for any and all costs related thereto.

3. Photo Release: As the parent or legal guardian of Camper, I hereby verify by my signature below, that the Camp has permission to use my son!s photograph in Boys’materials Camp and brochures promoting the Camp. Grades4. Refund 3Policy: - 12A $35 administrative fee (”Administrative Fee”) will be retained if you cancel, regardless of the reason. After October 31, 2010 refunds less the Administrative Fee will only be made for medical reasons and such refund requests must be accompanied by a letter from aOctober physician 31, 2010detailing refunds the less medical the reason(s) why the Camper cannot participate in the Camp. There will be no refunds for injuries that occur during Camp. If a Camper is dismissed or voluntarily withdrawals from Camp, there will be no refunds.
