Jo Anne Nugent, Ed D, author of A Handbook on Dual Diagnosis says it best: In the past, there has been an unfortunate ten- dency to assume that the most important element of treatment is the expertise of the medical pro- fessional, such as the psychiatrist. In fact, in many situations, support people felt paralyzed until they could access this clinical expertise. Certainly, the clinical professional plays a key role in diagnosis and treatment of a person with a dual diagnosis. However, people who support the individual on a day to day basis have just as crucial a role to play. Some of the key functions they fulfill are: 3 What is “Dual Diagnosis”? The term dual diagnosis is often used to refer to people who have a substance abuse disorder and a mental illness. However, it is the same term we use in the field of intellectual and developmental disabilities (ID/DD) when we refer to someone who has an Intellectual or Developmental Disability (ID/DD) and also has a Mental Illness (MI). You may hear other terms such as co- morbidity, co-occurring illnesses, concurrent disorders, comorbid disorders, co-occurring disorder, dual disorder. All of these terms are interchangeable with the term “dual diagnosis.” What is an Intellectual or Developmental Disability (ID/DD)? Intellectual disability used to be, and often still is, referred to as Mental Retardation. It is a term used to describe someone who is limited or slower in their ability to learn new information and develop new skills such as communication, self-care and social skills. People with ID/DD learn and develop more slowly than typical children. The may take longer to learn to speak, walk, and take care of their personal needs such as dressing or eating. As many as three out of every 100 people have an intellectual or developmental disability (ID/ DD). The most common causes ID/DD are: Genetic conditions -- Abnormalities of chromosomes and genes. Examples of genetic conditions are Down syndrome (trisomy 21), fragile X syndrome, and phenylketonuria (PKU). Problems during pregnancy -- When the baby does not develop normally inside the mother. For example, a woman who drinks alcohol or gets an infection like rubella during pregnancy may have a baby with mental retardation. Perinatal problems -- Problems during labor and birth, such as not getting enough oxygen. Health problems -- Diseases like whooping cough, the measles, or meningitis. Different from the normal ups and downs that everyone goes through, the symptoms of bipolar disorder are severe. They can result in damaged relationships, poor job or school performance, and even suicide. But there is good news: bipolar disorder can be treated, and people with this illness can lead full and productive lives. More than 2 million American adults,1 or about 1 percent of the population age 18 and older in any given year,2 have bipolar disorder. Bipolar disorder typically develops in late adolescence or early adulthood. However, some people have their first symptoms during childhood, and some develop them late in life. It is often not recognized as an illness, and people may suffer for years before it is properly diagnosed and treated. Like diabetes or heart disease, bipolar disorder is a long-term illness that must be carefully managed throughout a person�s life.

Her work combines cutting-edge research with deeply humanistic and often personal narratives—an approach that has cut through stigma to become a cornerstone of the field and a beacon for sufferers of mood disorders. Dr. Jamison is the co-author of Manic-Depressive Illness, the standard medical textbook in the field, and has written more than 125 scientific and clinical articles about mood disorders, suicide, creativity, and lithium. In her autobiography An Unquiet Mind: A Memoir of Moods and Madness, Jamison chronicles her battle with the cyclical highs and lows of bipolar disorder, a disease that for her is simultaneously lethal and life-affirming. Her manic highs are characterized by hypersexuality, fluidity of ideas, exuberance, extreme confidence, and hyperactivity (128). In contrast, when she cycles into depression after a manic episode, a desolate fog encapsulates her mind, bringing thoughts of suicide (39). Even as a professor of psychiatry who understands firsthand the ramifications of depression, she battles against taking the very same medications she urges her own patients to take. She fears that these mood stabilizers would eradicate the sweeping majesty of her mania, as depicted above (92). After years of resisting consistent treatment, she finally decides to stay on her medication, realizing that a life with less dramatic fluctuations in moods is preferable over no life at all.

    Serious Emotional Disturbance (SED) Expert Panel Meetings Substance Abuse and Mental Health Services Administration (SAMHSA) Center for Behavioral Health Statistics and Quality (CBHSQ) September 8 and November 12, 2014 Summary of Panel Discussions and Recommendations In September and November of 2014, SAMHSA/CBHSQ convened two expert panels to discuss several issues that are relevant to generating national and State estimates of childhood serious emotional disturbance (SED). Childhood SED is defined as the presence of a diagnosable mental, behavioral, or emotional disorder that resulted in functional impairment which substantially interferes with or limits the child's role or functioning in family, school, or community activities (SAMHSA, 1993). The September and November 2014 panels brought together experts with critical knowledge around the history of this federal SED definition as well as clinical and measurement expertise in childhood mental disorders and their associated functional impairments. The goals for the two expert panel meetings were to operationalize the definition of SED for the production of national and state prevalence estimates (Expert Panel 1, September 8, 2014) and discuss instrumentation and measurement issues for estimating national and state prevalence of SED (Expert Panel 2, November 12, 2014). This document provides an overarching summary of these two expert panel discussions and conclusions. More comprehensive summaries of both individual meetings’ discussions and recommendations are found in the appendices to this summary. Appendix A includes a summary of the September meeting and Appendix B includes a summary of the November meeting). The appendices of this document also contain additional information about child, adolescent, and young adult psychiatric diagnostic interviews, functional impairment measures, and shorter mental health measurement tools that may be necessary to predict SED in statistical models.

His understanding is reflective of the nineteenth-century German clinician, Emil Kraepelin. I propose that Kraepelin and Aretaeus possess more similarities than differences. They were homologous in research techniques and nosology, with one significant difference in aetiology. Presently, Aretaeus’ classification remains recognized in psychiatry, though with a slight deviation in understanding and under different psychiatric labels.

We talk about as a person who writes on his or her own time, by encouraging writing to heal, about dealing with his or her own choice. I chose those who consider disturbing writing, about celebrating the creative themselves writers over students on the belief that impulses that come with some mental illnesses , and such participants would likely be more aware of we talk, however rarely, about educating ourselves as how their writing practices and processes were teachers as to the unique processes such students bring affected by their illnesses.

    Pica Written by Urszula Kelley, MD Pica is an eating disorder that involves eating items that are not typically thought of as food and that do not contain significant nutritional value. Symptoms and Behavior The persistent eating, over a period of at least one month, of substances that are not food and do not provide nutritional value. The ingestion of the substance(s) is not a part of culturally supported or socially normative practice (e.g., some cultures promote eating clay as part of a medicinal practice). Typical substances ingested tend to vary with age and availability. They may include paper, soap, cloth, hair, string, wool, soil, chalk, talcum powder, paint, gum, metal, pebbles, charcoal, ash, clay, starch or ice. The eating of these substances must be developmentally inappropriate. In children under two years of age, mouthing objects—or putting small objects in their mouth—is a normal part of development, allowing the child to explore their senses. Mouthing may sometimes result in ingestion. In order to exclude developmentally normal mouthing, children under two years of age should not be diagnosed with pica. Generally, those with pica are not averse to ingesting food. The eating of nonnutritive substances can be associated with another mental disorder, (e.g., intellectual disability, autism spectrum disorder, schizophrenia) or medical condition (e.g., pregnancy or iron deficiency anemia), and, when considered in the context of the co-occurring disorder or condition, is sufficiently severe to warrant independent clinical attention (e.g., some women may eat nonfood items during pregnancy, but their doctor may determine that their actions do not indicate the need for separate clinical care).
