6 The Magazine of Cottage Health We never stop caring

During the Thomas Fire and Debris Flow, Cottage Health opened an Incident Command Center to anticipate and meet the needs of the community and ensure Cottage employees could get to the hospitals to provide care. Cottage Health’s Level 1 Trauma Center and emergency departments responded as 28 patients arrived at our hospitals in Santa Barbara and Goleta Valley.

During the closure of Highway 101, 700 Cottage Health employees were unable to drive to work. For employees essential to immediate patient care, the Incident Command Center arranged 4,000 transports and 900 nights of overnight housing, and also arranged the transport of hundreds of lab specimens to Pacific Diagnostic Laboratories for processing in Goleta.

Reactions so far have been favorable. “Doctors want to know.”

The task force’s efforts are also expanding within Cottage Health hospitals. “We’re looking at prescribing of pain medicines at discharge from the hospital and beginning to get data about what the practice patterns are,” explains Dr. Paul Erickson, Medical Director for Psychiatry and Addiction Medicine. Being watchful for overprescribing trends, he says, their analysis takes into account needs to individualize treatment.

Another key piece of solving the epidemic is getting treatment to those who need it. Dr. Erickson has been working with a team at Santa Barbara Cottage Hospital in developing an Addiction Consult Service to identify and engage any hospitalized patient who may need medical care for addiction. The consults will offer brief screening, and when appropriate, more comprehensive evaluation and treatment.

SBCH is also collaborating with Santa Barbara Neighborhood Clinics for a new walk-in service for those with addiction disorders. The Bridge Clinic will welcome patients referred from the hospital or Neighborhood Clinics for same-day treatment followed by stabilization over a one- to two-month period. Patients being discharged from the hospital could get services at the Bridge Clinic and would later be referred to community providers.

“People with addiction disorders may have crisis in other areas of their lives as well, and getting immediately accessible treatment will offer the best opportunity to address their drug use,” Dr. Erickson says.

Spring 2018 13 Population Health: From planning to action

On average, California and Santa Barbara are healthy, but that’s not the whole story. Data reveals significant disparity in health indicators based on race, education and income.

In the past two years since Cottage Health established a dedicated Population Health department, the team has been working with community organizations to listen and learn, to assess needs and identify what’s already working, to support successes and build on them by facilitating new partnerships and more sharing of data and learning across organizations.

The Population Health team provides partner organizations with program evaluation support to ensure measurable outcomes, and created an online platform for accessing shared data: CottageData2Go.org.

14 The Magazine of Cottage Health behavioral chronic access to food housing health conditions care insecurity insecurity

Based on community interviews, surveys and focus group input, five priority areas were identified as shared goals for our community

Cottage’s population health efforts are focused on supporting partnerships and initiatives that will address these needs, and creating new initiatives where gaps exist.

To support other nonprofits already working successfully in these areas, Cottage Health restructured its community partnership grants with a focus on behavioral health initiatives (those addressing mental health and addiction).

One of the newest pilot programs is a medical respite program that will assist hospital patients who are homeless with accessing continued health care, preparing them to be housing-ready and securing food.

The Population Health team provides partner organizations with program evaluation support to ensure measurable outcomes, and created an online platform for accessing shared data: CottageData2Go.org.

Cottage also funds research studies that will further the shared goals and collective work being done to meet community needs in these priority areas.

To promote inquiry, discovery and collaboration on promising medical innovations, the Cottage Health Research Institute (CHRI) recently hosted the Collaborative Research Symposium in Santa Barbara. More than 125 of the best minds in the region participated, including Cottage physicians and caregivers, UCSB faculty and researchers and business leaders working in biomedical, imaging and related fields. Speakers shared data or emerging technologies on topics ranging from vision loss to diabetes management to cancer care. The 2018 research grants program was announced, outlining seed funding available for the development of important new research initiatives that have the potential to improve health and patient care. Special thanks to Janet Larson Dunbar and Alex Pananides for their vision and support of this event to gather scientists and innovators.

A tradition of support Kirk and Jimmy Jo Edwards have deep roots in the Santa Ynez Valley. Both of their parents were early supporters and volunteers for what would become Santa Ynez Valley Cottage Hospital (SYVCH), even before construction began.

After living out of the area for more than 30 years, Kirk and Jimmy Jo had the opportunity to move back. A deciding factor was living near a hospital they knew provided excellent care.

They found out just how important that was when Jimmy Jo was rushed to Santa Ynez Valley Cottage Hospital last year. The Emergency Department treated her for a serious reaction to blood thinners. Kirk explained, “This was a critical situation after Jimmy Jo fell unconscious at home and had internal bleeding.” She needed help immediately. SYVCH serves the Valley with 24/7 emergency services, inpatient and outpatient care, advanced radiology services Physicians and staff made a quick diagnosis and stabilized her, and an onsite specialty Physicians’ Clinic. The hospital then transported her to Santa Barbara Cottage Hospital where provides exceptional personalized care for more than 36,000 she spent three days in the Medical Intensive Care Unit. patient visits each year. An avid golfer since he was 18, James Lee of Santa Barbara spends a lot of time on the course perfecting his game. He suffered a back injury a few years ago that kept him from playing but rehabilitation allowed him to return to the sport he loves.

James became a volunteer with Cottage Rehabilitation Hospital and the Adapted Golf program. Watching others in the program learn to golf for the first time or to improve their game despite limitations inspired Jim to make a donation to Cottage Larry Buker (Adapted Golf Instructor), René Van Hoorn, Alexis Sonier Rehabilitation Hospital Foundation. (golfer seated on cart), and James Lee.

“Being out on the golf course with others in the Adapted Golf program really helped me with my attitude. All of the participants out here are so amazing,” James said. “This program shows the human spirit in a nutshell.”
