Engaging individuals to reach wellness goals and healthier outcomes Just over two years ago, Steve transitioned from a shared living space with 24-hour onsite support to his own 2 apartment. The transition had been mostly successful with the exception of his continued weight gain due to lack of adherence to the wellness programs that were put in place. With the added safety of the built-in PERs with GPS in the Reemo Smartwatch, Steve feels more comfortable getting outdoors for walks around his neighborhood. He is now excited about the ability to track his progress to ensure he reaches his goals.

frequent questions FROM CASE MANAGERS

We are here to help you. If you have any questions not answered below, please contact us. Email [email protected] or call 1-877-697-3366.

Aren't some individuals fearful of technology and worried it will be difficult to use? Many Reemo Smartwatch users share that they are surprised at how easy it is to use. The smartwatch comes ready to use out of the box with a Quick Start Guide that contains images and easy to understand instructions. They only need to turn it on; no set up required. The screens are limited and intuitive. In fact, 80% of users begin using the device within the first 10 days.

Reemo also makes a welcome call to the individual's phone and sends messages to the watch to answer any questions, encourage regular watch use, and promote physical activity.

What are the benefits the smartwatch provides to individuals and their care circle? The Reemo smartwatch helps individuals: • Feel safe and secure doing what they love – inside and outside the home – with the built-in PERS call-for-help and GPS location • Get help quickly when they need it • Be more confident staying physically active • Stay connected with friends and family • Provide the individual and care circle peace of mind with independently living

What if an individual feels they don’t need a PERS device or don’t need it yet? Because the Reemo Smartwatch is more than just a traditional PERS device, it is not only for those who are frail or fear they may fall. It is an attractive watch that provides activity tracking, heart rate monitoring, access to emergency 24/7 service and can connect them to family or friends for non- emergencies. Encouraging individuals to begin using the watch now gives them the opportunity to get into the habit of wearing the watch daily and tracking their activity and heart rate.

How are the individuals and members of their Care Circle connected? Reemo connects the individual with their personal care circle via several ways. The Reemo user is able to share their data with their adult children, neighbors, and/or care team. When granted permission by the user, the care circle will have access to a web-based dashboard to see trends on their health and activity.

When using the call-for-help feature from their watch, the user can request the agent call their listed contacts for non-emergencies in support of their daily life and for any reason. In the event there is a confirmed emergency dispatch, the agent will inquire with the end-user if they would like their listed emergency contacts called. If the end-user consents, they will make an outbound call to the emergency contact(s). In addition, in an emergency call where the end-user is non-responsive, the agent will also call the listed emergency contact(s).

How are individuals engaged and supported while using the Smartwatch? • 5-7 business days after the Smartwatch is shipped, Reemo places a welcome call to remove any concerns, answer questions and encourage regular usage. • Messages are sent via the watch to introduce features, encourage, educate and inspire. • The Reemo support line is available Monday-Friday, 9 am – 5 pm CST and staffed with individuals trained to work with this population. Reemo can be reached at 1-877-697-3366.

This watch is ideal. It has gotten me to move more and challenges me.”

- Reemo smartwatch user THE PERSONAL INDEPENDENCE SMARTWATCH

How are emergency calls handled? Reemo provides a direct, one-touch, call feature. The user simply touches and holds their “call for help” watch screen and a direct two-way call to the 24/7 concierge call center through the watch is made. The agent’s portal will know who the caller is, what program they are part of, who their emergency contact(s) is and their pertinent information (location, etc.). This enables the agent to have an informative and interactive conversation with the caller and provide the appropriate level of help as required by the user. In the event it is an emergency call, local EMT/resource with priority- level access will be dispatched to the location of the user. The agent will stay on the line until help arrives, then they will notify the emergency contact(s) for that user.

What if the individual uses the call-for-help button but does not have an emergency? Reemo’s call experience is designed to be a “friend on the other end” experience, meaning the agent is trained and happy to support the end-user in whatever they need, for as long as they want to talk. This removes the fear that pushing the button means an ambulance will be sent, and instead provides the actual, real-world help and support most consumers typically seek out.

If it is confirmed that it is not an emergency event, the agent will help the caller receive support as they need it. The majority of calls placed via the Reemo smartwatch are not emergency calls. Instead they are requests to have someone in their care circle (adult child, friend, neighbor) contacted on their behalf, a referral to a specific resource, or simply to talk because they are lonely. Reemo is a firm believer that by removing the stigma of ‘emergency push buttons’ and replacing them with a friendly, helpful voice creates the best consumer experience and best supports a consumer as they maintain their independence and health.

One day there was a storm and all the phone lines were out. My heartrate dipped very low so I made an emergency call. I am very impressed that I can get help even though the phones were out.”

In those rare cases, the Reemo Smartwatch may not be the right option for them.

Does the member need an email address to receive the Reemo smartwatch? No. The Reemo smartwatch will have full functionality and work out-of-the-box if the member has no email. However, with an email address and internet access, the member will have a more robust user experience including:

• Receiving tracking information when the watch ships. • Access to the Reemo online dashboard to view their historical health data and trends. • Ability to add additional emergency contacts via their online profile so when they call the 24/7 support the agent can connect the member with these additional people. • Ability to give viewing rights of their personal dashboard to their chosen friends and family so they can remotely ensure the member is active and safe.

Does the Reemo smartwatch fit different sized wrists? Yes. There are two watch bands that come with the watch – small and large.
