Graboyes and Eric Topol, “Anatomy and Atrophy of Medical Paternalism” (Mercatus Research, Mercatus Center at George Mason University, Arlington, VA, 2017). ABSTRACT Since Ancient Greece, a guiding principle of Western medicine has been pater- nalism—the idea that doctors have intrinsically superior insights, patients should defer to their edicts, and this asymmetry is a desirable state of affairs. In the 20th century, new medical knowledge and technologies accumulated at an unprecedented rate, and medical paternalism arguably reached its zenith. Now, however, new technologies are eroding the doctor’s privileged role by deconstructing, digitizing, and democratizing medical knowledge. Digital technologies and other breakthroughs offer unprecedented opportunities to save lives and cut costs. Along with self-interest, however, selective strains of risk aversion, technophobia, and egalitarianism among physicians are gener- ating resistance to the new reality. This article catalogs the motives for and impact of medical paternalism, the reasons for its decline, and potential policy responses that would ease the transition. Graboyes is Senior Research Fellow at the Mercatus Center at George Mason University in Arlington, VA. Author of “Fortress and Frontier in American Health Care,” his work asks, “How can we make health care as innovative in the next 25 years as information technology was in the past 25 years?” In 2014, Dr. Graboyes received the Reason Foundation’s Bastiat Prize for Journalism, an international competition “which honors the writing that best demonstrates the importance of individual liberty and free markets with originality, wit, and eloquence.” Previously, he was senior health care advisor for the National Federation of Independent Business; Sub- Saharan Africa economist for Chase Manhattan Bank; and regional economist/director of education at the Federal Reserve Bank of Richmond. He traveled extensively in Africa for Chase. Twice, he was visiting health care scholar in the Republic of Kazakhstan. Over a 20-year period, he taught full-time at the University of Richmond and part-time at Virginia Commonwealth University, the University of Virginia, The George Washington University, and George Mason University. He earned his PhD in Economics from Columbia University; master’s degrees from Columbia, Virginia Commonwealth University, and the College of William and Mary; and a bachelor’s from the University of Virginia. He was President of the National Economists Club and the Richmond Association for Business Economics and chaired the Health Economics Roundtable for the National Association for Business Economics. Graboyes MERCATUS WORKING PAPER In response to the quickly escalating disruptions to the US healthcare system from the COVID-19 pandemic, the Mercatus Center has decided to release a working-paper version of its forthcoming 2020 edition of the Healthcare Openness and Access Project (HOAP). This prerelease version has not been peer reviewed. The opinions expressed in Mercatus Working Papers are the authors’ and do not represent official positions of the Mercatus Center or George Mason University. Jared M. Rhoads, Darcy N. Bryan, and Robert F. Graboyes. “Healthcare Openness and Access Project 2020: Prerelease.” Project Overview, Mercatus Center at George Mason University, Arlington, VA, March 2020. Abstract The Healthcare Openness and Access Project (HOAP) is a collection of state-by-state comparative data on the flexibility and discretion that US patients and providers have in seeking and delivering healthcare. HOAP combines these data to produce 41 indicators of openness and accessibility. In turn, these indicators are aggregated into 5 broad categories (Professional Regulation, Institutional Regulation, Patient Regulation, Payment Regulation, and Delivery Regulation), which in combination form the overall HOAP index. In addition, there are 7 indicators grouped under the title “Watchlist”—variables worth tracking, but not incorporated at this time into the categories or overall index. The indicators, categories, and overall index are all scored on a 1-to-5 Likert scale. Using the data provided on HOAP’s website, readers may adjust the weight given to each indicator to custom-build subjective measures and rankings that differ from the ones presented in this paper.
