The panelists will provide an overview Infant Mortality CoIIN of the process for developing an action planning process/framework with a Representatives focus on what the end product is supposed to be. Title V will touch on the following: Moderator: Margie . Brief overview of the six MCH population domains and the Hudson National Performance Measure (NPM) his/her state selected to prioritize; . Brief overview of the State Performance Measures (SPMs) his/her state identified based on the findings from your assessment; . Description of the process used for identifying/selecting the State-Initiated Evidence-Based or Evidence-Informed Strategy Measures (ESMs) and the activities that will be implemented to support its progress; and . 10:30 March of Dimes Collaboration and Sharing Kenn Harris, New Haven This session will feature March of Dimes (MOD) representatives to share the Healthy Start and Incoming history of Healthy Start and the MOD, current strategies to address, discuss President, National goals, and potential partnership opportunities with Healthy Start. Time Subject Speaker/Facilitator 9:00AM Welcome, Day II Recap, Questions and Comments Angela Hayes-Toliver, DHSPS 9:00 Evaluators Roundtable and Brainstorming Session Facilitator Jeffrey Guidry, NOTE: Evaluators will meet at New Haven Healthy Start (Community Evaluator, Staten Island Foundation for Greater New Haven, 70 Audubon Street, New Haven, CT Healthy Start 06510) This session will include an opportunity to brainstorm and discuss collaborative opportunities among evaluators. The benefits of GIS mapping will also be covered.

Evaluators will conclude their discussion around 11:40 AM, return to the full group meeting at the Q Club and report out to the full group at 12 noon. 9:15 Trauma Informed Care (TIC) Training Opportunities to Improve Quality Gloria Castro, Consultant, Service Delivery Infant-Parent Program, Many Healthy Start grantees have identified that the impact of trauma on University of , San Healthy Start participants is significant. This session will include a TIC expert Francisco to facilitate discussion and sharing among grantees on their TIC efforts. The session will also explain the community training opportunities available to Margie Hudson, Hartford grantees on TIC and how one grantee has used this training to elevate the Healthy Start issue among their community. 10:45 Break
