Whether your program is educating future physicians, nurses, physician-assistants, dentists, naturopathic doctors or podiatrists – this Faculty Guide can be used to enhance the statewide vision of redefining pain and addiction. The essence of the curriculum rests with its ten Core Components – this Faculty Guide provides a further level of detail under each Component and Objective. As colleagues in health care, we understand that this material represents a significant change from the way we were taught about pain and addiction – or the way we were taught about medicine at all. This curriculum moves from a micro- to a macro- perspective, stressing whole-person care and the societal impact of pain and addiction. It reflects the interrelated nature of pain and addiction – two areas that have been traditionally siloed. It specifically calls for clinicians to be able to critically analyze material, their own practices, and their own biases. The approach to pain and addiction is advancing rapidly. The material in this Faculty Guide is aggressively up-to-date and evidence-based, yet remains in line with the National Pain Strategy, the Institute of Medicine and modern educational theories. Financial Implications of Promoting Excellence in End-of-Life Care PROMOTING EXCELLENCE IN END–OF–LIFE CARE A NATIONAL PROGRAM OFFICE OF THE ROBERT WOOD JOHNSON FOUNDATION Completing the Picture of Excellence In 1997, The Robert Wood Johnson Foundation launched a national program Promoting Excellence in End-of-Life Care with a mission of improving care and quality of life for dying Americans and their families. We soon realized that the metaphor of a jigsaw puzzle seemed apt in describing our efforts to expand access to services and improve quality of care in a wide range of settings and with diverse populations. No single approach would suffice—a variety of strategies, models of care, and stakeholders are necessary to successfully complete the picture. This monograph represents one aspect of our work and one piece of the puzzle of ensuring that the highest quality of care, including palliative care, is available to all seriously ill patients and their families. Breaking the Fever: A New Construct for Regulating Overtreatment Isaac D. Buck* The Department of Justice’s (“DOJ”) current theory of overtreatment regulation — and, in fact, all of the prominent amount of medical necessity-based health care fraud enforcement — adopts the argument that providers are violating the False Claims Act when they submit bills to the federal government for care they administered that is not medically necessary. Besides stoking the ire of the provider community, this regulatory strategy is susceptible to inefficiency, imprecision, and — as I have argued before — overuse. Whether a procedure was medically necessary can be a highly difficult question to answer, one easily swayed by clinically-complex details, and one made murkier by clinical and geographic variation, Medicare’s often outdated and/or inaccurate coverage determinations, and overtreatment’s settlement-based regulatory regime that features few trials. This strategy can stifle innovation and arrest the natural evolution of the standard of care. Given these difficulties, and aware of the imminent cost challenges facing the Medicare program, this Article presents a proposal that seeks to improve the efficiency and intellectual honesty of overtreatment regulation by advocating for an explicit shift in the focus of enforcement from medical necessity to excess utilization. Page 1/36 Abstract Background: Medical tests provide important information to guide clinical management. Overtesting, however, may cause harm to patients and the healthcare system, including through misdiagnosis, false positives, false negatives and overdiagnosis. Clinicians are ultimately responsible for test requests, and are therefore ideally positioned to prevent overtesting and its unintended consequences. Through this narrative literature review and workshop discussion with experts at the Preventing Overdiagnosis Conference (Sydney, 2019), we aimed to identify and establish a thematic framework of factors that inuence clinicians to request non-recommended and unnecessary tests. Methods: Articles exploring factors affecting clinician test ordering behaviour were identied through a systematic search of MedLine in April 2019, forward and backward citation searches and content experts. Two authors screened abstract titles and abstracts, and two authors screened full text for inclusion. Identied factors were categorised into a preliminary framework which was subsequently presented at the PODC for iterative development.
