Dr Sian Roberts & Deidre Watkins Over the last 3 years Dr. Roberts developed the John Hampden surgery to make it dementia friendly and got funding to support a personalised dementia specialist nurse - Deirdre Watkins. We have learnt as a surgery the importance of caring and supporting vulnerable patients in the community so they are able to live an independent and fulfilling life. Dear Patient,

We are delighted to have been shortlisted for a Dignity in Care award by Bucks County Council. We would like to share with you our team project on dementia and hope you can support us, by voting via this link; www.buckscc.gov.uk/votedignity.

Our John Hampden Surgery team shares the vision in that patients should enjoy life and live well with dementia. We have recognised the importance of person centred care for our patients with dementia.

With this in mind, we have worked hard on making our surgery as Dementia Friendly as possible.

Each member of our staff has become a “Dementia Friend” and as a team, we have had dementia awareness training to help us all understand how to help and communicate with patients with dementia. We provide double appointments and appointment prompts, support for carers and annual reviews for both the patient with dementia and for their carer. We work closely with partners Carers Bucks, Age UK and Alzheimer’s Society as well as the local social service team and memory clinic team, to provide a streamlined package of care.

We have looked objectively at the surgery itself and improved the signage, changed carpets, provided a memory box and a memory clock. We are part of the Dementia Friendly community for Great Missenden, sharing and learning from other local businesses.

We employ a Community Support Nurse, Deirdre, who visits patients in their homes, to support them and their carers and to offer advice regarding relevant local services. Deirdre recognises that it can be stressful for patients to have dementia and for carers too and she provides fantastic support at times of difficulty.

Deirdre also helps with the planning of what a patient with dementia may like from family or health professionals should they deteriorate in the future.

Deirdre encourages patients to complete a” This is me " document, which is a simple and practical tool that people with dementia can use to tell people about their needs, preferences, likes, dislikes and interests. It enables health and social care professionals to see the person as an individual and deliver person-centred care that is tailored specifically to the person's needs. It can therefore help to reduce distress for the person with dementia and their carer. It can also help to prevent issues with communication. We share the "This is me" document with all our staff so we can get a real understanding for the patient to help tailor our approach with each and every patient. We can also send the document with any written referrals to hospital, so that the hospital staff too can get to understand the patients, and helps reduce anxiety and improve communication.
