REGISTRATION PROCEDURES

Information within this manual will provide you with a base of all regulations and procedures to follow when registering your members and teams. Please refer to the Hockey Manitoba Constitution, Regulations & Rules of Competition handbook for all detailed regulations. Each Minor Hockey Association will receive approximately 5-10 handbooks (teams will separately receive their handbooks). Should your Association require additional copies, please contact your Zone Registrar or the Hockey Manitoba office and we will send you the appropriate numbers.

Hockey Canada Registry (HCR) Database

Hockey Manitoba is once again offering use of the Registry (HCR) database for the 2010-11 season to all Minor Hockey Associations. Read Only access is available for your association. This will allow you to view your teams, players, coaches and officials. If you would like Read Only access it can be set up through the Hockey Manitoba office. Because you will not be utilizing the on-line database for data entry, hard copies of all forms including rosters, addition/deletions, playoff application forms etc. are required to be submitted to your zone registrar as in previous seasons.

Team Roster Form

Each Minor Hockey Association will complete a Hockey Manitoba Roster Form for each team. All information must be completed. If a member is “NEW”, please print “NEW” in column titled “Last team registered”. PLEASE NOTE: Rosters can also be filled out on-line from our website at www.hockeymanitoba.mb.ca and submitted to the appropriate Zone Registrar as indicated below.

Completion of Roster Form

1. OBTAIN INFORMATION FOR ALL PLAYERS & TEAM OFFICIALS A. Full Name B. Gender C. Date of Birth D. Mailing Address including Legal Location (township/section/range/quarter), where applicable E. Postal Code F. Phone number G. Name of last team registered with, (if a new player, please write “NEW” in this column) H. Please check and list team officials’ certification(s) attained to date. I. Make sure that the player resides nearer to your centre than to other centres or has been approved through the Hockey Manitoba transfer procedures. J. Parent/Guardian’s signature

Privacy Policy

The information on the Roster and Additions/Deletions form is collected for all registered participants and is required by Hockey Manitoba (its employees, team officials, volunteers, leagues and Associations) to facilitate its hockey programs and to administer the rules that govern sanctioned events. Parent/Guardian’s signature is required and indicates understanding and agreement with respect to the aforementioned use of personal information.

Hockey Manitoba does not share the information we collect outside our Branch and Associations, however we may from time to time use this information for the purposes of offering additional services, including promotions offered by third parties. This type of usage of your personal information by Hockey Manitoba and/or its Associations is entirely at your discretion. Please indicate your preference by circling Yes or No next to your signature.

Copy of birth certificate (PLAYER ONLY) B. Make sure player resides nearer to your centre than to other centres or has been approved through the Hockey Manitoba transfer procedures.

3. PRINT CLEARLY.

4. Keep Roster form as neat as possible. If you make an error, cross out that line and re-write the information in the next line.

5. The MINIMUM number of players per team is 10 (minimum 15 for Western Bantam Championships, minimum 13 for Female National Championships) and the MAXIMUM number of players per team is 19. This includes goaltenders. Bantam and higher categories - Circle goaltenders.

6. List up to five Team Officials per Roster. Please place the team official in the correct space provided. (i.e. Head Coach, Asst. Coach, Manager) If more space is required, list additional officials on an Additions/Deletions form. Be sure to circle to team officials certification.

7. Team Safety person shall be placed in the area marked “SAFETY”. Please complete this area even if listed as coach. If a team has more than one Safety, please list on an Additions/Deletions Form.

8. Please circle the team’s primary contact. Also be sure to include a team e-mail contact in the space provided.

9. IMPORTANT: The Zone Registrar must approve all rosters prior to the team playing any league games.

10. Initiation and Novice players may register at any time, providing that there was a team registered prior to January 1st and that there is room on such roster for the player to register.

11. DO NOT list any affiliate players on roster forms.

12. Appropriate Membership/Registration fees MUST be remitted with each roster.

13. Appropriate Insurance fees MUST be remitted with each roster.

NOTE: Incomplete or improperly completed rosters or those with missing documents will be returned, resulting in delays for the approval of players’ certificates and rosters.

Player & Team Approval

1. Once completed, the Minor Hockey Association will send the rosters to the Zone Registrar for approval along with all appropriate fees. Any errors or omissions on your forms or fees will delay the Zone Registrar approving players and or teams from participating in sanctioned activities. Please ensure that all paperwork and fees are in order prior to forwarding to your respective Registrar.

2. All Hockey Manitoba and Hockey Canada insurance and membership fees are listed in this manual.

3. Each Association must use the Hockey Manitoba registration reconciliation form when sending in rosters and fees for every batch sent to your local Registrar. This will assist the Zone Registrar and the Hockey Manitoba office to ensure that all applicable fees have been paid. Please refer to the Registration Fee form included in this manual.

4. The Zone Registrar will review each roster as per Hockey Manitoba registration regulations. Once approved, the Registrar will endorse the roster with their signature and return a copy to the Minor Hockey Association.

5. The Zone Registrar will keep the regional copy of each roster for his/her files and send the top copy to the Hockey Manitoba office.

6. Hockey Manitoba will enter all information into the registration database. Copies of your Association’s computer-generated rosters will be available through your Zone Registrar once they have been entered into the database. The computer-generated roster or original rosters are both available as official proof of membership.

7. Any errors within the computer-generated printout must be reported immediately to your Zone Registrar.

Additions & Deletions to a Roster

Any player or team official wishing to register to a team that has already submitted a roster to your Zone Registrar must be placed on an Additions/Deletions form as included in this manual. As with the regular roster, the Zone Registrar must approve the addition of any member before they can participate in any sanctioned activity. All appropriate fees must accompany the Additions/Deletions form as well as a Hockey Manitoba reconciliation form to assist with ensuring all members have paid the appropriate fees.

NOTE: Privacy information on the Additions/Deletions form is applicable as previously indicated.

REGISTRATION DATES TO REMEMBER

October 1 a) Deadline for submissions of new and existing minor hockey amalgamations. b) Final date for submission of Minor Provincial Host Applications

December 1 a) Final date for coaches to obtain certification for Initiation, Coach, Development 1, Respect in Sport and Safety programs. b) Final date for the application of inter-regional transfers to the Regional Registration Committee. c) Final date for submission of transfers to the Hockey Manitoba office. d) Final date for submission of rosters in the HCR database to be eligible for Provincial Play-offs (does not include final date of registering players).

December 15 Final date for submission of Provincial Play-off application form to enter provincial competition.

January 10 Final date for releasing players to be eligible to register up until February 10th (all categories).

February 10 Final date for registering new players or players released from teams on or before January 10.

Note: Minor Hockey Associations wishing to host a 2010-2011 Provincial Championship must complete a host application form by October 1, 2010. A bulletin outlining the process is distributed every year at the end of August. Please see the Hockey Manitoba handbook for a complete list of regulations pertaining to the provincial championships including categories of competition.
