All of the students that I have mentored for this research elective have presented their work at least once at a local, regional, or national conference, including live presentations at the national conferences of the Society of Hospital Medicine and American College of Chest Physicians.

I have served as the supervising clinical faculty for approximately 20 medical students during their participation in a new elective in point-of-care ultrasound. In addition to providing the students with didactics, they have received individual mentored scanning practice on actual patients and practiced invasive ultrasound-guided procedures with simulation models.

07/2014 - Present CIRC5005 Clinical Skills Graduate Discipline Coordinator

In addition to leading the construction of the Center for Clinical Ultrasound Education in the School of Medicine, I have served as a Discipline Coordinator and one of the primary teaching faculty for the Longitudinal Medical Student Ultrasound Curriculum which is part of the medical student Clinical Skills Course. As one of only two discipline coordinators and faculty for this curriculum, I teach basic bedside ultrasound skills to approximately 450 medical students from the first and second years of each year.

I served as a group facilitator and clinical preceptor for the 1st year medical student clinical skills course which is part of the core medical school curriculum. Additionally, I have served as a clinical preceptor for 1st year medical students during the Longitudinal Preceptorship Program. I met with each student individually approximately once per month, took them to the bedside to practice physical exam skills, and assisted with their final writing assignment.

Since 2012, I have served as Clinical Faculty for approximately 65 third-year medical students during their inpatient ward rotation at the Audie L. Murphy VA Hospital. offered by the Society. My roles included designing the train-the-trainer program, developing and delivering lectures and hands-on instruction, and evaluating each faculty's teaching skills.

10/2016 - Present Course Director, VA VA SimLEARN National Simulation 210 Point-of-care Ultrasound Center, Orlando, Florida

Training Workshops Since 2016, I have led the Department of Veterans Affairs national initiative to train providers primarily from emergency, critical care, and hospital medicine in point-of-care ultrasound applications. I direct the training workshops and serve as faculty to teach didactics and hands-on sessions. All training courses take place at the VA‘s SimLEARN National Simulation Center in Orlando, Florida. Since the launch of the program in 2016,

training workshops have been held every 1-2 months and approximately 210 VA providers have been trained through fiscal year 2018. Due to the demand, the course enrollment is being expanded, and educational offerings for advanced users and "champions" are being developed for launch in 2019.

across the country and internationally. The workshop has sold out on every occasion, and course evaluations have been excellent with >95% of attendees rating the course as "good‘ to "excellent" in effectiveness for enhancing knowledge and skills.

04/2013 - 04/2017 Faculty, Ultrasound- American College of Physicians 550 guided Lumbar Puncture Annual Meetings

Workshop From 2013 until 2017, I served as faculty yearly to teach ultrasound-guided lumbar puncture at the American College of Physicians Annual Conference. Each year approximately 110 participants attend a focused 90-minute hands-on session on the performance of lumbar puncture with ultrasound guidance. My roles included lecturing on

the use of ultrasound to guide lumbar puncture, mentoring chief residents to teach the workshop, providing hands-on instruction, and procuring supplies and ultrasound equipment. The course evaluations have been excellent with scores >4.5 on a 5-point scale.

on Emergency Trauma Care As faculty of the international conference, I taught approximately 30 physicians the hands- on skills to perform trauma assessments using bedside ultrasound. The workshop concentrated on teaching the E-FAST exam (Extended Focused Assessment with Sonography for Trauma) and other basic emergency medicine ultrasound applications.

Hospitalist Pre-course In 2010, I developed a new pre-course for the Society of Hospital Medicine titled, "Point-of- care Ultrasound for the Hospitalist." The pre-course has been offered every year at the Society‘s annual meetings and is the Society‘s highest-rated and longest-standing pre- course. Despite doubling the capacity, the pre-course has been sold-out since its inception. As the pre-course director and faculty, I lead development of the educational curriculum; recruitment and organization of faculty; conduct training sessions for faculty; organize an annual faculty retreat; coordinate the meeting space and procurement of equipment; and work with the Society staff to coordinate all on-site logistics. Each year I also give several of the lectures during the pre-course. My overall evaluations for this pre-course have been >4.5 on a 5-point scale.

Training Course I developed an ultrasound-guided procedures training program for the Section of Hospital Medicine at the University of Chicago. The hospitalists cared primarily for solid organ transplant patients, and these high-risk patients often needed bedside procedures. I obtained institutional funding for a portable ultrasound machine and simulation models, developed simulation-based training modules for different procedures, and taught the modules to approximately 20 hospitalist physicians and mid-level providers.

RESEARCH

EXPERTISE: Diagnostic Point-of-care Ultrasound - Since my final years in medical school, I have had an interested in bedside diagnostics which evolved into a passion for diagnostic and procedural applications of point-of-care ultrasound. During my residency and years as junior faculty, I learned how to perform focused diagnostic ultrasound examinations using portable ultrasound machines, and since 2006, I have been teaching other healthcare providers clinical applications of point-of-care ultrasound. Currently, I teach point-of-care ultrasound applications to medical students, residents, fellows, and faculty locally, as well as nationally and internationally. My research interests have centered on evaluating the impact of point-of-care ultrasound on health services. My current work focuses on the implementation of point-of-care ultrasonography in the VA healthcare system and measuring the long-term impact of integrating clinical use of bedside ultrasound in a large integrated healthcare system.

Ultrasound-guided Procedures - Use of ultrasound guidance to perform bedside procedures has evolved to become the standard of care for many bedside procedures. By collaborating with critical care, emergency medicine, and hospitalist physicians, I have developed training curricula to teach common bedside procedures with the use of ultrasound guidance. I have trained a variety of healthcare providers, ranging from students to physicians in practice, and I have led educational programs in ultrasound-guided procedures locally, nationally, and internationally. Currently, I continue teaching diverse groups of learners and pursuing health services research related to ultrasound-guided bedside procedures.

Clinical Research 10/2015- Pleural Fluid Echogenicity by Ultrasound Imaging and Computer Based Pixels, 07/2017 The purpose of this study was to determine the relationship between the sonographic appearance of both transudative and exudative pleural effusions and assess whether pleural fluid echogenicity measured by pixel density can be used to differentiate the two types of effusions.

07/2015- Pleural Ultrasonography to Rule out Post-bronchoscopy Pneumothorax, The goal 11/2017 of this study was to determine the sensitivity, specificity, and likelihood ratios of pleural ultrasonography to detect post-bronchoscopy pneumothorax compared to portable chest x-ray in lung transplant recipients.

01/2015- Pleural Fluid pH: Comparing analytical methods and impact on clinical data, 06/2017 Pleural fluid pH is used to guide immediate decision-making in the management of pleural effusions, but the most common method used today, pH meter, has not been validated against the gold standard method. The purpose of this study was to determine the diagnostic accuracy of a pH meter vs. arterial blood gas analysis and determine whether the pleural fluid pH is accurate enough to guide clinical decision-making.

10/2014- Cardiac Microlesion Formation During Invasive Pneumococcal Disease, This goal 09/2015 of this project was to better understand the invasion of cardiac myocytes by S. pneumoniae during severe pneumococcal sepsis that leads to cardiac injury, followed by cardiac scarring after antibiotic therapy in a non-human primate model.

Co-Investigator 07/2015- Myocardial fibrosis following pneumococcal pneumonia, 05/2017 The purpose of this study was to elucidate the pathophysiology of myocardial microlesion formation and cardiac scarring that occur during severe pneumococcal sepsis in a non-human primate model.

09/2009- Future Research Needs for Prevention and Treatment of Clostridium difficile, 09/2012 The purpose of this comparative effectiveness review was to provide an overarching assessment of the evidence for comparing the accuracy of diagnostic tests and the effectiveness of prevention and treatment interventions on initial and recurrent C. difficile infection-related patient outcomes in adults.

09/2009- Procalcitonin for Diagnosis and Management of Sepsis, The purpose of this 09/2012 systematic review was to synthesize comparative studies examining the use of procalcitonin to guide antibiotic therapy in patients with suspected local or systemic infections in different patient populations.

07/2005- Influence of Hypoalbuminemia or Hyperalbuminemia on the Serum Anion Gap, 06/2006 The purpose of this study was to examine the alteration in anion gap based on changes in serum albumin and total protein using a linear regression model. Our regression data obtained from a large database supported an adjustment factor of 2.3 per g/dL decrease or increase in serum albumin.

Investigator Initiated Projects 01/2013- Use of Ultrasound for Central Venous Catheterization (US-CVC), The purpose of 01/2016 this study was to assess the frequency of use of ultrasound guidance to insert central venous catheters by intensivists and hospitalists at different insertion sites and explore the potential provider, educational, and system barriers to use of ultrasound.

This presentation caught the attention of the society's leadership because of the controversy around this topic. Afterwards, my colleagues and I were invited to collaborate with the society's technology and standards committee to develop a roadmap to outline an approach to develop simulation-based privileging programs.

Grant Detail: This Department of Veterans Affairs QUERI partnered evaluation project is evaluating the impact of a Point-of-care Ultrasound (POCUS) Training Course on provider skill acquisition and retention; the frequency of POCUS use by trained providers; and the barriers/facilitators to POCUS in the VHA. Data sources include the pre- and post-course assessment tools knowledge and skills tests, medical coding data, and course evaluations. Providers that participate in the POCUS Training Course are being compared to control providers from wait-listed facilities. Findings from this project will guide ongoing efforts of our operating partners, Specialty Care Centers of Innovation (SCCI) and the Simulation Learning and Research Network (SimLEARN), to build a national POCUS training program and facilitate implementation of POCUS use system-wide in the VA healthcare system.

Grant Detail: The Agency for Healthcare Research and Quality (AHRQ) conducts the Effective Health Care Program as part of its mission to organize knowledge and make it available to inform decisions about health care. AHRQ has an established network of Evidence-based Practice Centers (EPCs) that produce Evidence Reports/Technology Assessments to assist public- and private-sector organizations in their efforts to improve the quality of health care.
