Mumps Virus Pathogenesis Clinical Features
Mumps Mumps Mumps is an acute viral illness. Parotitis and orchitis were described by Hippocrates in the 5th century BCE. In 1934, Johnson and Goodpasture showed that mumps could be transmitted from infected patients to rhesus monkeys and demonstrated that mumps was caused by a filterable agent present in saliva. This agent was later shown to be a virus. Mumps was a frequent cause of outbreaks among military personnel in the prevaccine era, and was one of the most common causes of aseptic meningitis and sensorineural deafness in childhood. During World War I, only influenza and gonorrhea were more common causes of hospitalization among soldiers. Outbreaks of mumps have been reported among military personnel as recently as 1986. Mumps Virus Mumps virus is a paramyxovirus in the same group as parainfluenza and Newcastle disease virus. Parainfluenza and Newcastle disease viruses produce antibodies that cross- 11 react with mumps virus. The virus has a single-stranded RNA genome. The virus can be isolated or propagated in cultures of various human and monkey tissues and in embryonated eggs. It has been recovered from the saliva, cerebrospinal fluid, urine, blood, milk, and infected tissues of patients with mumps. Mumps virus is rapidly inactivated by formalin, ether, chloroform, heat, and ultraviolet light. Pathogenesis The virus is acquired by respiratory droplets. It replicates in the nasopharynx and regional lymph nodes. After 12–25 days a viremia occurs, which lasts from 3 to 5 days. During the viremia, the virus spreads to multiple tissues, including the meninges, and glands such as the salivary, pancreas, testes, and ovaries. -

It accounts for only 3% of all cases of sialolithiasis. Parotid ductal calculus is rare as compared to submandibular ductal calculus. Case presentation: A 3-year-old boy presented with acute painful right parotid swelling with pus discharge from the Stensen duct. Computed tomography revealed calculus obstructing the parotid duct causing proximal ductal dilatation and parotid gland and masseter muscle oedema. The child was treated with conservative measures, and subsequently the swelling and calculus resolved. Conclusions: Small parotid duct calculus in children may be successfully treated with conservative measures which obviate the need for surgery. We discuss the management of parotid sialolithiasis in children and conduct literature search on the similar topic. Korotych UKRAINIAN MEDICAL STOMATOLOGICAL ACADEMY, POLTAVA, UKRAINE ABSTRACT The aim: The study of the presence of disorders in the ante- and postnatal periods of development of children from 2 months to 15 years with chronic parenchimatic parotitis, which may affect its course. Materials and methods: It has been examined and treated 88 children, aged from 2 months to 15 years with chronic parenchimatic parotitis, and their mothers were interviewed, who indicated the pathological course of pregnancy, childbirth and indicated the type of breastfeeding babbies. The scope of the survey included general, additional methods, consultations by related specialists and statistical processing of results. Results: 88 children with the exacerbation of chronic parenchimatic parotitis were examined (42 – (47%) with active course and 46 – (53%) with inactive). The exacerbation occurred on the background of acute infectious processes or coincided with the exacerbation of one of the chronic diseases. The first manifestations occurred in spring (55%) and autumn (36%) periods, 44% of children were hospitalized with other diagnoses. The presence of pathological conditions during pregnancy and birth defects in their mothers were recorded more often 3,5 and 3,3 times, respectively, compared with control. 70% of children received mixed and artificial feeding and were more likely to become ill. Conclusions: The severity of clinical manifestations of inflammation and disorders of the general condition depended on the activity of the course of chronic parenchimatic parotitis and were more pronounced when active. -

Measles is a leading cause of morbidity and mortality in developing countries. Over the past decade measles has been making a resurgence in the United States. Vaccination with MMR or MMRV is highly effective in preventing the disease. Introduction The Virus The measles virus is a single-stranded, enveloped ribonucleic acid (RNA) virus of the genus Morbillivirus within the family Paramyxoviridae. 1 The virus enters the respiratory epithelium of the nasopharynx and regional lymph nodes resulting in viremia and dissemination to the skin, respiratory tract, and other organs. The peak incidence of measles in temperate areas is late winter and early spring. Transmission Measles is a highly contagious virus with an estimated 90% secondary infection rate in susceptible domestic contacts. Measles is spread by droplets from respiratory secretions and close personal contact or direct contact with infected nasal or throat secretions. Measles virus can be transmitted by an infected person from 4 days prior to the onset of the rash to 4 days after the rash erupts. -

A retrospective analysis was conducted on patients who visited the Department of Oral Medicine from March 2019 to March 2020.Clinically diagnosed cases of salivary gland diseases which included salivary gland neoplasms, xerostomia, necrotizing sialometaplasia, mucocele, ranula, sjogren’s syndrome, sialodochitis, sialadenitis were included in the study.The details of each case were reviewed from an electronic database.From the study we found that 17 patients were diagnosed with salivary gland disease.The most commonly observed salivary gland disease was mucocele of the lip with a frequency of 41.17% in the study population followed by xerostomia (17.65%).Salivary gland disease can occur due to variable causes and might significantly affect the quality of life and daily functioning.Only with a thorough knowledge of the subject it is possible to detect the diseases of the salivary gland in their early stage and manage them more efficiently.
