The Corangamite Health Collaborative is an independent group of public health agencies working together to improve health outcomes and service delivery for people in the Corangamite Shire. The ‘Collaborative’ consists of Corangamite Shire, Cobden District Health Services, South West Healthcare, and District Healthcare Services and Terang and Mortlake Health Service. Agencies share the Collaborative’s Vision Statement:

“Through collaboration, we will improve the health and wellbeing of the communities of Corangamite” The ‘Collaborative’ seeks to engage an experienced and expert allied health interdisciplinary leader who can demonstrate superior consolidated skills and has professional credibility in their field of practice. The successful applicant will encompass a passion for increasing allied health workforce development with the aim of developing a coordinated and sustainable allied health service model that is integrated with the other service providers of the Corangamite Shire.

The position will be based at Terang & Mortlake Health Service and the successful applicant will report to the CEO with governance reporting to the Corangamite Health Collaborative CEO group.

The successful applicant will have excellent written and verbal communication skills as well as strategic planning, project management and change management experience, preferably in the health sector. A degree in an allied health discipline is essential.

CORANGAMITE HEALTH COLLABORATIVE The Corangamite Health Collaborative is an independent group of public health agencies working together to improve health outcomes and service delivery for people in the Corangamite Shire.

Corangamite profile Corangamite Shire is located on 's Great South Coast (Figure 1). It is home to approximately 17,000 residents and attracts more than 1.5 million visitors each year. It includes the towns and districts of Camperdown, Cobden, , Lismore, and District, , Princetown, Simpson, Skipton, Terang and Timboon. The Shire relates to the sub-regional centres of and Colac and the regional centres of and Ballarat.

Allied Health Leadership Position

Key principles and values

The collaborative operates within a set of values and governance principles reflecting the independence of member organisations, fairness, respect and equity. Key principles guiding the work of the collaborative are contained in the Memorandum of Understanding (MOU) 2014, signed by each organisation, summarised below:

Members have agreed to strive to achieve a strategic, whole-of-Shire approach to: • meeting the health care needs of Corangamite communities; and • retaining health care resources within the Shire where it is appropriate to do so.

To work cooperatively to provide high quality, sustainable health services which:

• clearly integrate with each other; • retain health care resources within the Corangamite Shire, if quality and sustainability are achievable; and • enable Corangamite residents to access services as close to home as possible.

Member organisations will not provide services to their communities if those services cannot be designed for sustainability and quality.

Where services cannot be provided locally by one or more parties:

• clinically-appropriate referral of patients between Parties is likely to improve service viability and regional self-sufficiency; • a joint venture or other collaborative service arrangement involving two or more Parties may be appropriate; or • service provision by other providers on an in-reach basis to the Corangamite Shire may be appropriate. If referral of patients to services outside the Shire is necessary, where possible it will be via planned, documented, reliable and efficient pathways.

The parties have also agreed the following:

• where appropriate, collaborate in the provision of support services (clinical and non- clinical) and functions such as quality improvement

• undertake collaborative service and workforce planning and development

• consult with each other before making decisions that may impact health care provision in Corangamite communities

• systematically share relevant information

Unless exceptional circumstances apply, services provided through inter-Party agreements will be billed on a cost-recovery basis. The Parties will support the Collaborative to establish strong links with a range of relevant stakeholders including • Western Victoria Primary Health Network

• Department of Health and Human Services

• South West Primary Care Partnership

• Ambulance Victoria

• Colac Area Health
