1.0 Introduction Madrid Engineering Group, Inc. (MEG) was founded in 1992 by the president and principal of the company, Mr. Larry D. Madrid, P.E., and has grown to become a multi-disciplined team of engineers, scientists, technicians, and support personnel. Our corporate office is centrally located in Florida in Polk County, City of Bartow. We are a registered professional engineering (EB6509) and geological (GB459) consulting firm. The purpose of the firm has remained unchanged since its inception: to provide high quality consulting and design engineering and construction support services to our clients throughout Florida and beyond. With over 7000 successfully completed projects, a staff of over 50 employees and field and laboratory equipment for exploration and testing, MEG has the expertise, experience and technical capabilities to solve problems brilliantly, and bring exceptional value to our clients. For over 20 years, we’ve helped our clients solve these complex problems and accomplish difficult tasks. General Engineering Approach Madrid Engineering Group is one of central Florida’s premiere geotechnical and materials engineering companies. Our project approach is designed to first, gain an understanding of the overall problems and objectives of an assignment; then to provide staff and equipment necessary to investigate and gather engineering data, and finally to design an “appropriate” solution for our clients in a cost effective manner. Once we have identified the objective(s) for each project, including costs, deliverables and deadlines, we will work with the Client to define a preferred method of communication, either by face-to-face meetings, phone calls, emails, online meetings, or a combination thereof. We also define a single point of contact for the Client as a means to ensure all communication is addressed in a timely and efficient manner.

During the 20 years of participating in challenging projects, MEG’s personnel has gained extensive experience evaluating subsurface site conditions and reliably quantifying soil and rock behavior.

We are experienced in the design and implementation of a wide range of field investigation programs, from the rehabilitation of existing dam structures to retrofitting on-site unstable soils. MEG has the necessary capabilities to perform deep exploration borings even when the study site has been deemed to have difficult access. We have developed state-of-the-art design parameters and construction recommendations for conventional and non- conventional designs. All analysis and design is supported by our in-house soils laboratory, construction materials laboratory, and geotechnical drilling fleet.

MEG’s approach to site improvement and subsurface stabilization has allowed for several innovative and cost effective solutions. MEG has successfully demonstrated the utilization of a proprietary material for chemical subsurface stabilization with no post settlement issues, and has gained the statewide respect of numerous private and public establishments.

MEG has also performed several sensitive site remediation and foundation stabilization programs prior to construction on marginally stable or saturated soils, which have been executed in nationally registered historic districts.

GEOLOGIC SITE CHARACTERIZATION MEG has licensed professional geologists on staff with a wealth of experience in site geologic and geomorphic characterization, , and geologic h a z a r d s identification. We work with geophysical tools such as ground penetrating radar (GPR) and electrical resistivity (ER) to view subsurface conditions using advanced technology. Our geologists have also completed numerous contamination assessments that delineated contaminant concentration and plume direction, as well as designed remediation plans in accordance with FDEP requirements. MEG geologists also work closely with our geotechnical engineers on special projects that may involve slope stability/landslide issues. Our geologists provide valuable input regarding the mass behavior and geologic characterization of soils and rocks.

GEOTECHNICAL RELATED PROJECTS These services have included a number of engineering investigations and permitting assignments for its clients. By listening to the needs of our clients, working hard to meet those needs, and by offering creative solutions, which meet or exceed technical and regulatory requirements, MEG has achieved a great measure of success. Listed below is a list of projects that provide an overview of the broad range of technical capabilities and innovation we offer our clients.

Geotechnical Innovation - Residential Project Using Geofoam – Lakeland, Florida We believe that Madrid Engineering was the first to design foundations using Geofoam for a project in Polk County, Florida. The two houses, Lots 61 and 62 of the Waterview Development were built on mined land adjacent to a mine pit near Lakeland Highlands Boulevard. The soil profile consisted of loose to medium dense to 8 feet below ground surface (bgs), then underlain by very soft waste phosphatic clay from 8 and 25 feet bgs, in turn underlain by loose sand to 40 feet bgs. Soft soils were too deep for wood pilings. MEG APPROACH We concluded that it was too expensive for pipe or concrete piles, and too expensive for post tensioned slabs (which would not have alleviated differential settlement). The houses to the east and west of the subject lots each had experienced excessive settlement, based on stairstep cracks observed on the exterior walls. We recommended and monitored installation of Geofoam. Polystyrene blocks were 2 feet high by 3 feet wide by 10 feet long each. Along the load bearing perimeter, they were stacked two units high (4 feet) to offset the weight of excavated soil and the house, creating a near “net zero” loading. To assure that buoyancy could not lift the house, the net load was designed to be positive but less than the previous load, effectively unloading the soil so that even though thick, highly compressible clay existed below the house, no settlement was induced.

MEG BENEFIT The project was extremely successful, with significant cost savings to the owner, reduced time of installation compared to post-tensioned slabs, and NO SETTLEMENT CRACKS to date.

Lake Hancock Sediment Sampling – Bartow, Florida MEG completed sediment sampling at over 170 locations on this 4,550-acre lake using a piston tube- type sampler to obtain continuous sediment cores ranging from a few inches to 8 feet long. Sample locations were obtained using GPS equipment in real time, and data was input into a handheld computer. Sediment samples were tested for organic content and solids content, along with numerous other parameters. Bulk samples were also obtained and used for landside testing of the consolidation behavior of the sediments, with and without flocculants. The study included a review of previous engineering studies; a field program consisting of thickness determinations and sediment sampling within the lake, and an environmental/geotechnical evaluation of potential sites for use as confined disposal facilities (CDFs); laboratory testing of sediment samples to determine physical and chemical characteristics for dredging and storage purposes; analysis of dredging technologies to determine the most economical/feasible method to dredge; and process engineering to optimize consolidation of the sediments.

MEG APPROACH – We used a clear PVC pipes with custom seals to visually confirm sample thickness and quality. Sampling locations were confirmed using handheld GPS equipment accurate to within +/- 5 feet. We investigated several sediment handling methods, including dewatering tubes.

MEG BENEFIT - Sediment sampling confirmed that more muck had accumulated in Lake Hancock since the previous evaluation 10 years earlier. MEG proposed several potential methods to remove the muck, which contributes to high nutrient loading and eutrophication. The report was comprehensive and was utilized by Polk County and the Southwest Florida Water Management District for planning purposes.

Mosaic Desoto Mine Railroad Spur - Desoto County, FL

The project consisted of the design of a new private railroad spur for the future Desoto Mine in Arcadia and Pine Level, Florida. The proposed alignment was approximately 11.3 miles long running generally from west to east beginning at the CSX railroad west of County Road 661 and terminating south of State Road 70 on the south side of the proposed plant site adjacent to the Dimare Property. The alignment crosses from the north side to the south side of State Road 70 just west of Bethel Farms Road. Generally, the alignment encounters several streams, wooded areas, orange groves and wetlands while traversing larger areas of open fields. The creek crossings where either trestles or storm water culverts are proposed include a tributary to Oak Creek, Horse Creek, Brandy Branch, Buzzards Roost and two unnamed stream crossings.

MEG APPROACH We developed Boring Location Plans, Performed Exploratory Soil Borings, performed Capacity Analyses (for vertical and lateral loads) for various proposed pile sizes, and provided general recommendations regarding the construction of the proposed railroad embankment. The owner was pressed for time, so MEG completed the over 300 soil borings, plans production, and the geotechnical report, in just one month.

MEG BENEFIT – Soil and rock data along the preliminary alignment were gathered and preliminary pile load capacity was calculated for budgetary purposes so the final layout and structure costs can be estimated for future construction.

5J International Project (Private Owner) – Proposed 5J Eco Youth Camp and Recreational Facilities - Polk County, FL The project consisted of an estimated 9,600 lineal feet of roadway with associated pavement parking, a multi-sport/activity recreational camp including multiple recreational fields/courts, a zipline with multiple tie/landing points, various single story structures including a maintenance building, concession stands, an office building, and “pod” areas each consisting of two (2) two-story dormitory buildings, a one-story cafeteria/multi-use building, and associated pool and courtyard areas. The site is a former phosphate mine and has been reclaimed to a “land and lake” setting. No development currently exists.

MEG APPROACH - We performed the review of field exploration plans, soil analyses, settlement analyses, geotechnical parameter selection for roadway and structure design, and report preparation. MEG BENEFIT – By utilizing MEG’s recommendations, the owner will be able to take a large site with very loose soils and/or soft soils from former phosphate mining operations to a higher use recreational complex for commercial use.

Pete’s Lane Geotechnical Investigation, Lakeland, Florida MEG completed a soils investigation for Pete's Lane consisting of approximately 4100 feet of existing unpaved roadway. MEG completed hand auger borings every 300 feet, obtaining soil samples at major strata changes. Laboratory testing was completed to classify soils, including natural moisture content, percent passing the No. 200 sieve, gradation analysis and organic content, and to determine the geotechnical and drainage properties. MEG also ran field permeability tests along the roadway. The report included recommendations for all new features of the site preparation; boring logs and test results. MEG BENEFIT Based on our test results, Polk County’s consultant engineer designed an innovative stormwater collection system that took advantage of the highly permeable sandy soils without compromising the sewer system performance. Ultimately, the

County achieved significant cost benefit by not requiring a stormwater retention pond system for the newly paved surface.

CSX Corings/Lab Support, Tampa, Florida – CSX planned an expansion of a loading facility at their Rockport shipping terminal. MEG performed continuous split-spoon sampling to 14 feet depth, Shelby Tube sample collection, monitor well and installations to 14 feet, double ring tests, geotechnical testing including organic content and particle size, constant head permeability testing, and determination of the angle, , and in-situ density. The site was a former dredge material fill area, but was deemed suitable for limited usage.

Cypress Point Landfill Geotechnical Assessment, City of Tampa, Florida - A prime property adjoining Tampa Bay was acquired by the City of Tampa for a proposed park, but only after Madrid Engineering Group, Inc., working with other consultants, investigated subsurface conditions at the site. Historical aerial photos indicated two non- native conditions. Part of the land surface had been created from spoil materials pumped in by dredging contractors who deepened the bay for boat traffic. Also, illicit miscellaneous waste materials were dumped at the site over several years, and covered with fill dirt. Over 100 soil borings were completed strategically across the site to assess the overall subsurface conditions, determine the type of debris, the depth and thickness of the debris, and determine if the debris contained hazardous materials. Based on the results, the City purchased the property and used the information to design park elements while limiting or eliminating the need to excavate any waste debris. Boring data was referred to for design stabilization of a parking lot and foundations for park facilities.

SR 29 Geotechnical Roadway , Deep Lake to Everglades City – FDOT District One - MEG was the lead geotechnical engineering consultant for the design phase of this project, completing hundreds of soil borings for a roadway widening project through an environmentally sensitive area.

Foundation Design, Lakeland Christian Church, Lakeland, Florida – MEG provided detailed geotechnical support for the design of a church facility on a site that included a former disposal site for porcelain tile. Due to organics and broken tile, the excavate and replace method and deep dynamic compaction were used to stabilize portions of the site.

Foundation Design, Industrial Ball Mill, Polk County, Florida - MEG designed foundations for a heavily loaded industrial process facility associated with phosphate mining and phosphoric acid production. Loads were as high as 250 tons per footing.

Polk Parkway Construction Monitoring – Lakeland – Florida’s Turnpike – MEG assisted in the construction monitoring of the Polk Parkway, a 21-mile limited access toll road. MEG provided support for installation of and settlement instrumentation used to monitor settlement and lateral spreading of the embankment sections, including those over waste phosphatic clay. We also observed the installation of wick drains used to significantly decrease the time of consolidation of the soft clay. This was the first usage of wick drains ever with waste phosphatic clay.

Cherry Street Manhole Repair, Plant City, Florida – Unacceptable pavement distress had developed on a heavily traveled street in Plant City. Soil borings revealed poor compaction adjacent to the manholes at 3 feet below the surface. MEG devised an innovative, low cost approach for the City of Plant City Engineering Department involving injecting low strength grout, which added strength to the soils and filled the voids. Conventional approaches would have required up the loose soil and re-compacting, which would have taken much more time and disrupted traffic. MEG developed and submitted compaction grouting specifications for the remediation of the subsurface soils adjacent to manhole structures located on Cherry Street. We also provided construction monitoring and quality control over the compaction grouting operation for each of eight manhole structures, including materials testing of the slurry grout mix. Field personnel submitted daily summary reports for the remediation operation.

Central Florida Disposal Class III and C&D Landfill – Ft Meade, Florida – From 1996 to 2006, MEG provided geotechnical, hydrogeological and environmental permitting services on two formerly mined sites for certification as Class III and C&D landfills. Field investigations included soft sediment delineation in three lakes with collection of undisturbed samples, SPT boring, hand augers, muck and slime probes. The work effort included: geotechnical reports for , soil profiles, boring location, as well as hydrogeological evaluations, stormwater management systems, operations plans and preparation of financial assurance estimates. These services included new permits, permit renewals, cell construction and landfill closure certifications.

Soil Survey in mined area in North Lakeland, Florida - MEG obtained over 50 soil samples, on over 800 acres of land on two sites. MEG completed laboratory testing and provided slope stability and foundation design on mined land. Also completed ecological study, environmental site assessments, and bathymetric surveys of seven lakes that included lake volumes, lake bottom muck probes, and lake contours.

Linebaugh Pit Surface Water Permit Modification, Tampa, Florida – The Linebaugh Avenue borrow pit, used for construction of embankment of the Veteran’s Expressway, was originally designed to be dug in the wet using dragline or dredge equipment. In order to dig in the dry the contractor requested that MEG modify the surface water permit to allow for temporary pumping and storage of the water to other portions of the system, allowing segments of the pond to be dug in the dry. MEG sized pumps, calculated operational requirements, and designed a rim dike system that protected adjacent wetlands from being dried out. The modification saved thousands of dollars and months of construction time. The permit modification was completed and accepted by SWFWMD in a record time of only two weeks.

Quarterly Clay Sampling for Florida Power Corporation Hines Complex, Bartow, Florida – MEG performed piston tube sampling of very soft to soft waste phosphatic clays at 7 settling areas undergoing dewatering phase. MEG also completed laboratory testing of the samples collected.

Stormwater Pond Design, Seminole County – Florida’s Turnpike – MEG, as a subconsultant to Keefer Veroski Consultants, completed auger borings, double ring Infiltration testing, SPT borings and a foundation design for a temporary construction office building and associated stormwater pond for the Seminole Parkway. Pond design included a mounding and recovery analysis using MODRET software.

Pavement Coring for Carpenter’s Way, Lakeland, Florida – MEG completed 18 asphalt cores along Carpenter's Way between Wedgewood Estates Boulevard and North Florida Avenue to determine asphalt and limerock base thickness. Results were recorded on a table and coring locations were shown on a site map. Results indicated extremely variable foundation conditions due to poor initial road construction.

2.2 SUBSIDENCE INVESTIGATIONS AND STABILIZATION DESIGN Sinkholes are a substantial problem in Florida, and other parts of the southeast US and the Caribbean. MEG has completed over 5000 subsidence investigations throughout Florida. MEG professional engineers and professional geologists are acknowledged experts in the State of Florida for geotechnical engineering, to include sinkholes, , , behavior of sediments, organics, and other soft soils, behavior of soils generated or disturbed by mining activities, and other related geotechnical issues. We have been called upon as geo-experts for over 100 legal cases. We provide the total engineering and geologic package, including: preliminary site visits; photo-documentation of damages; test pits for foundation assessment; non-intrusive geophysical surveys including but not limited to ground penetrating radar and electrical resistivity imaging; floor elevation surveys; standard penetration test and direct push cone penetration testing (SPT and CPT) borings; dynamic hand cone penetration tests for shallow soils; hand auger borings; and emergency response services. We also have an in-house structural group that provides evaluations and recommendations regarding the integrity of structures that are inspected for subsidence damage. Section 2.3 offers more details regarding the capabilities and expertise of our structural engineering group. MEG compiles and thoroughly evaluates all the data collected from each project site into a comprehensive professional report. Additionally, MEG designs grouting and underpinning sinkhole and structural stabilization plans. We are at the forefront of using alternative remedial programs such as polymer and slurry grouting for shallow applications. As a service to the client and to ensure the remediation program is conducted according to MEG specifications, we provide expert monitoring services. Upon completion of the stabilization program, MEG will prepare a professional summary report to the client. Reports are completed in an efficient, timely manner and are customized to the needs and requests for each client and sometimes for each individual policy requirement.

Our structural staff has provided technical expertise for clients in residential, commercial, industrial, institutional, municipal, and transportation markets. We also strive to maintain a holistic approach to civil engineering. Many projects may require not only a structural input (focus on elements above ground) but also a geotechnical input (focus on elements below ground) regarding subsurface soil conditions and their impact on the structure of interest. Our structural team works hand-in-hand with our in-house geotechnical engineers in order to provide high quality and sound civil engineering service.

STRUCTURAL ENGINEERING RELATED PROJECTS Listed below is an abbreviated project list of structural engineering related projects that provide an overview of the broad range of technical capabilities which we have to offer our clients.

MULTI-STORY CONDOMINIUM LIMITED STRUCTURAL INVESTIGATION AND REPAIR – Culbreath Key Condominiums Building 9, Tampa, FL This project consisted of the evaluation and assessment of a structural failure in progress that included one post tensioned concrete beam and three concrete columns of a 4-story, 500+ occupants, and residential building’s first floor structure. MEG performed a structural and geotechnical investigation at this site and found that field changes made during initial construction had resulted in introducing major structural flaws. Project responsibilities included site evaluation, floor elevation study, planning and oversight of geotechnical drilling, coordination of structural analysis with post- tensioning sub-consultant, and presentations of findings and repair recommendations to the Culbreath Key Condominiums Owner’s Association Board of Directors.

MEG APPROACH: MEG utilized various destructive and non-destructive techniques to detect and assess installation and location of post-tensioning strands in various structural elements in an effort to identify if changes had been made in those elements commensurate with noted field-expedient changes implemented in primary structural components. During the course of the investigation, MEG learned that a field installed concrete column did not have an appropriate foundation. MEG also evaluated soil conditions to assess any sub-surface contributions to the identified structural failure in progress.

FOUNDATION STABILIZATION AND ROOF FRAMING REPAIR – Tee It Up, Inc. Golf Driving Range Facility in Oviedo, FL This project consisted of foundation stabilization and roof framing repair based on the results of a subsidence investigation completed at the property. Due to the presence of highly organic soils below the building and nearby de-watering, a total of 15.5 inches of settlement occurred near the southeast corner of the structure. MEG provided the design and layout of the foundation stabilization and re-leveling plan in addition to localized roof framing repair. The plans included a total of 52 exterior helical auger pins and 41 interior/exterior slab helical auger pins.

MEG APPROACH: MEG investigated sub-surface conditions and found highly organic soils that were in the influence zone of the building foundation. While settlement had occurred over time, more rapid settlement was experienced due to dewatering activities for new construction on a neighboring property. Structural elements including foundations, concrete floor slabs, CMU bearing walls, bearing wall penetrations, and roof framing were assessed and evaluated for both structural and cosmetic damage that resulted from the movement. Because of the amount of movement that had been experienced, a plan for staged lifting and leveling was developed for implementation by the contractor.

MEG BENEFIT: MEG was able to assist the client in evaluating both long-term as well as recent settlement so that the owner could enter into discussions with the owner and contractor for the improvements being made on the adjoining parcel to devise an equitable division of costs to repair the damage. MEG was also able to evaluate not only the cause for settlement but damage to the building that resulted from the movement and develop appropriate repair plans to stabilize, re-level, and restore the building to its full use.

PACKAGING CORPORATION OF AMERICA – Packaging Corporation of America Facilities, Auburndale, FL The project consisted of a foundation design for heavy corrugating and gluing machinery trains used in the manufacturing process for cardboard. The machinery operates around the clock and imposes significant vibration and impact forces on the foundations in addition to very high dead and live loads. Base plate tolerances are extremely critical and mis- alignment results in process failures and unacceptable maintenance downtime. Initially, MEG was consulted to evaluate the cause for unacceptable foundation settlement in equipment originally installed on foundations that did not adequately account for in-situ soil conditions. Replacement foundations consisted of traditionally reinforced cast-in-place concrete pile caps on helical auger piles. Construction challenges required removal and replacement of foundations during very short maintenance downtime windows as longer periods for more conventional construction methods could not be tolerated. Designs developed for original equipment were modified and incorporated into plans for new equipment trains during various plant production changes, modifications, and upgrades. There were a total of 15 separate foundations on a total of 344 helical auger piles. This project also required the demolition and installation of a new concrete slabs on that are subject to heavy wheel loads from forklifts and development of joint designs that would withstand continual pounding from these wheel loads during normal operations.

MEG APPROACH : MEG evaluated numerous alternatives in an effort to minimize interrupting plant production and normal operations while implementing necessary repairs to equipment foundations. The selected alternative was both innovative and cost effective allowing production to resume in repaired areas within client dictated time constraints while enabling the contractor to continue his work simultaneously with little impact to normal plant operations and schedules.

MEG BENEFIT: The client did not have to stop operations, which was a significant positive impact to the owner as the retrofit was performed without causing a loss of profit. MEG’s limited access equipment and trained personnel, contribute to the standards and site specific safety protocol.

POLK COUNTY PARKS AND NATURAL RESOURCES – Port Hatchineha Seawall, Retaining Wall, and Sidewalk Repair at Lake Hatchineha, Haines City, FL This project consisted of the assessment, evaluation, and repair of a failing 500 foot long seawall and sidewalk at Port Hatchineha located adjacent to Lake Hatchineha in Haines City, FL. After acquiring the property, Polk County Parks and Natural Resources Division staff were concerned about pedestrian safety at failing sections of the sidewalk and sought MEG’s assistance in both evaluating the cause for failure and developing an appropriate repair plan. MEG provided the assessment and also a design for the retaining wall and sidewalk repair. The seawall itself, while being an obsolete design is reasonably functional and stable. However, over time, soils have washed through weep holes at the base of the seawall and created unstable conditions including void areas under the adjacent sidewalk and retaining wall. The repair design consisted of steel angle spreader beams supported by helical auger piles. The repair design was created to be implemented in a by-phase installation progression in order to re-level the sidewalk and retaining wall. The most elegant fix for the problem would require removal and replacement of the obsolete seawall and sidewalk system; however, funds to affect this repair were not available. MEG considered several alternatives and selected the most feasible repair that would restore functionality to the sidewalk and extend the life of the seawall until replacement of the entire system can be implemented. The beauty of MEG’s plan is that, for the most part repairs are relatively simple, can be implemented by section, and can be accomplished with in-house maintenance staff.

MEG APPROACH: Using various techniques, including minor destructive testing that allowed borescopes to be inserted into the area behind the seawall and side and non-destructive testing with sensitive metal detectors to locate reinforcing steel within the concrete, MEG was able to quickly identify areas that were becoming unstable. MEG staff were able to structurally analyze the sidewalk and retaining wall so that various repair options could be evaluated. We were able to determine that the concrete slab sidewalk was able to bridge over soil fill behind the seawall. This led to an innovative and cost effective plan to stabilize the sidewalk and retaining wall that could be implemented in stages by the County with mostly in-house forces.

MEG BENEFIT: By combining structural and geotechnical engineering investigations using in-house staff, MEG was able to affect a coordinated approach to assessing and identifying the problem. At the same time, this coordinated approach allowed us to develop an innovative repair plan that could be implemented as necessary to extend the life of this sidewalk and dock structure within a popular and heavily utilized County park facility.

Polk County North Central Landfill, Florida – Among other projects for Polk County, MEG has provided geotechnical support for the North Central landfill expansion involving a 59 acre lined cell. MEG also provided the quality assurance and quality control for the construction of this expansion. This included the 59 acres of and embankment earthwork, lined landfill cell, sand drainage layer installation, haul , pump stations, storm water drainage facilities, and miscellaneous services. The project was fast tracked and constructed on a 24-hour, 7-day a week schedule. The budgetary estimate for MEG’s QA/QC effort was in excess of $500,000.

Environmental Assessment, Monitoring, and Operation and Maintenance for EarthTech, MacDill Air Force Base, Florida – MEG has assisted and supervised delineation consisting of numerous hand auger soil borings with organic vapor analysis (OVA/FID) screening for petroleum sites and numerous hand auger soil borings with field immunoassay screening for PCB sites. MEG has supervised soil and groundwater contamination delineation by means of direct push technology. MEG has installed environmental monitoring at several sites and has coordinated and performed groundwater sampling events at these sites as well as others. Sampling includes field screening for lead using a colorimeter and analytical testing. MEG has monitored remediation by means of ORC (oxygen release compound) injection at various petroleum-contaminated sites. MEG assists in monitoring, repair, and upkeep of on-site remediation systems.

Base Wide Environmental Drilling, for EarthTech, Tyndall Air Force Base, Florida – MEG was contracted to perform difficult access environmental drilling to install deep and shallow monitor wells at Tyndall Air Force Base. The scope included environmental drilling through buried landfill debris, well installation, and well completion.

Environmental Drilling, for EarthTech, Avon Park Bombing Range, Florida – MEG has been awarded multiple contracts to perform environmental drilling to install deep and shallow monitor wells at Avon Park Bombing Range. The scope included environmental drilling, well installation, well completion, and well abandonment.

Remedial Action Plans, Reagent Yards, Central Florida - Prepared remedial action plans for four chemical reagent yards that had extensive contamination. RAPs provided for excellent levels of cleanup at lowest cost for the client. The plans were accepted and approved upon first submittal, without any additional requests for information by the regulatory agency.

Tank Closure, Phosphate Mine, Central Florida - Completed tank closure and managed initial remedial action (IRA) of two, 20,000 gallon diesel tanks and one 12,000 gallon unleaded gasoline tanks.

Contamination Assessment for Confidential Client, Central Florida - MEG supervised an initial remediation action (IRA) removal of nearly 1500 cy of diesel and gasoline contaminated soil at a high- use industrial fueling facility. After removing the soil, our engineers and scientists followed up with a detailed contamination assessment of the site consisting of numerous soil borings with organic vapor analysis (OVA/FID) screening, environmental monitor well installation, sampling and analytical testing of both soil and groundwater, and evaluation of double-plume contamination migration. Groundwater flow direction and precise measurement of contaminant levels were critical to evaluating the site, and recommending a least cost alternative for ultimate cleanup of the site.

Quarterly Sampling for Trans-Phos, Bartow, Florida - A large trucking maintenance facility in Polk County had underground storage tanks removed, along with contaminated soil. Unfortunately, groundwater levels were slightly above regulatory limits and the company was required to monitor groundwater analytical levels. Madrid Engineering Group, Inc. was called upon to obtain groundwater samples, water levels, and other field parameters, in strict accordance with a quality assurance plan. Samples were shipped to an approved analytical laboratory and results were tabulated (see example below). MEG prepared and submitted quarterly reports to the Florida Department of Environmental Protection (FDEP) for the monitor only plan (MOP). Results indicate a steady decline in pollutant levels.

Limited Environmental Assessment of Two Parcels, Bartow, Florida - Confidential client needed assessment for property transfer of parcels with limited access, heavy vegetation and extensive wetlands. MEG reviewed maps and historical environmental documents, and completed reconnaissance of the properties. Several waste dumpsites and potential environmental liabilities were cited.

Contamination Assessment, SR29, Collier County, Florida – FDOT District One - MEG was selected to provide Phase I environmental assessment of over 50 parcels along the corridor of the SR29 alignment, as part of the roadway widening design for the Department of Transportation, District One. Seven parcels were suspected of contamination, ranging from petroleum to pesticides, to PCB’s. The environmental assessment was completed in accordance with FDOT protocol.

Environmental Drilling for Polk Parkway, Lakeland – Florida’s Turnpike – MEG was selected as the preferred environmental specialty-drilling contractor for the Polk Parkway by both HDR Engineering (preliminary analysis) and Westinghouse Remediation Services, Inc. (final assessment and remediation contractor during construction phase activities). Over 15 parcels located throughout the project limits, were assessed for contamination using MEG personnel and expertise.

Environmental Permitting for Confidential Client, Central Florida - An unfortunate incident involving an off-site discharge of process water caused this industrial client to be placed under a consent order by the Florida Department of Environmental Protection. Madrid Engineering Group, Inc. was directed by the client to assess the damage to the site surface and groundwater, if any, and to determine a course of action to satisfy regulatory requirements and provide for future safety to the environment. Madrid Engineering acted on behalf of the client for all permitting and consent orders, made recommendations and supervised construction of facility improvements, which included construction of a designed retention pond to store all process and stormwater on site. Permitting of the site was required by Polk County, FDEP, and SWFWMD. Concurrently, Madrid Engineering also designed foundations for a new “heavily-loaded’ industrial manufacturing facility at the site.

Annual Water Quality Monitoring, Trucking Transfer Station, Central Florida - Provided quarterly testing and reporting to the Florida Department of Environmental Protection for 6 monitor wells at a former underground tank spill site.

Tampa International Airport (TIA) Expansion Environmental Assessment – Madrid Engineering Group was selected as a key subconsultant to both ECT, Inc. and CDM for the environmental analysis and remediation of the Drew Park area. This was the largest planned expansion of TIA for the next 25 years, and required testing and analysis of over 300 small businesses including manufacturing, repair, and dry cleaning facilities. MEG installed over $100,000 worth of monitor wells at various locations.

CEI RELATED PROJECT

Bruce B. Downs – Tampa, Florida The project consisted of the widening of approximately 3.3 miles of Bruce B. Downs Blvd., from Palm Springs Boulevard to Pebble Creek Drive. Specific project work includes embankment construction, new flexible pavement, milling and resurfacing, storm water utilities, and drilled shaft installation. MEG provided engineering technicians with CTQP certifications for Earthwork Level 1 and 2, and Concrete Field 1. Drilled Shaft Inspectors were provided as needed. MEG also provided laboratory testing services. We also designated a lead technician to ensure that all logbooks and plans were updated at the offices of the Owner’s inspection team on a daily basis.

Bartow Northern Connector (Ernest Smith Boulevard) – Polk County, Florida This project consisted of new construction of approximately 2 miles of four lane divided highway between US 98 and US 17. A new bridge over Saddle Creek on a driven pile foundation is also being constructed.

MEG APPROACH: MEG provided Contractor Quality Control testing and inspection services for this project. Specific work for this project included earthwork construction inspection and laboratory testing of soils, concrete field and laboratory testing, Quality Control Management services and pile driving inspection.

MEG BENEFIT: Due to MEG’s extensive experience in geotechnical exploration and soils engineering, when unanticipated groundwater and soil conditions, as well as difficult pile driving conditions were encountered by the Contractor, MEG provided geotechnical borings and laboratory testing support that ultimately allowed the Contractor and the Owner to successfully resolve those issues without significant design changes or cost increases.

Fried Chicken Company at LEGOLAND – Winter Haven, Florida This project consisted of the new construction of a restaurant facility at the LEGOLAND theme park.

MEG APPROACH: We provided geotechnical field exploration and engineering services, as well as construction materials testing and inspection services for this project.

MEG BENEFIT: MEG provided a limited- access drill rig for the geotechnical exploration which allowed the exploration to proceed with negligible disturbance to the existing park facilities and on an extremely accelerated schedule. MEG also worked closely with the Contractor to ensure all construction materials testing and inspection services were performed and accepted by the Owner in order for the facility to open on schedule.

State Road 60 Emergency Repairs – Mulberry, Florida This project consisted of repairing a section of S.R. 60 that was washed out due to a drainage failure.

MEG APPROACH: MEG provided Contractor Quality Control testing and inspection services for this project. Specific work for this project included earthwork construction inspection and laboratory testing of soils, and asphalt paving inspection.

MEG BENEFIT: MEG provided overnight inspection and testing services and 24-hour turnaround on laboratory results in order to allow the Contractor to proceed with construction and minimize inconvenience to the traveling public.

Culbreath Key Condominiums Building 9 Structural Evaluation and Repairs – Tampa, Florida This project consisted of determining the cause of structural damage observed in column and beam elements in the bottom floor parking garage level of a 4-story condominium located on Tampa Bay, and working with a Post-Tensioning specialty engineer, General Contractor and Remediation Contractor to remediate the structure and foundations

MEG APPROACH: MEG performed a geotechnical investigation and structural condition assessment to determine the extent of damages and likely cause of damage, and then performed on-site inspection and laboratory testing services during the remediation of the building foundation and superstructure.

MEG BENEFIT: MEG was able to determine the full e xtent of damages to the structure through a thorough investigation and determined that in addition to an inadequately constructed post- tensioned beam, differential settlement of the building foundations was exacerbating the distress observed in the superstructure. MEG’s on-staff structural engineering team worked closely with a Post-Tensioning specialty engineer to design and develop a structural remediation program consisting of column-jacketing, additional PT beam sections and grouting for the existing foundation to ensure the structure remained secure.

State Road 17 at Mountain Lake Cutoff Road – Lake Wales, Florida This project consisted of milling and resurfacing, widening and drainage improvements to approximately 0.3 miles of two lane roadway at the intersection of S.R. 17 and Mountain Lake Cutoff Road.

MEG APPROACH: MEG provided Contractor Quality Control testing and inspection services for this project. Specific work for this project included earthwork construction inspection and laboratory testing of soils and Quality Control Management services. MEG BENEFIT: Drainage improvements at the project site required the Contractor to maintain a minimum level of within the drainage basins during construction. During project closeout, testing by MEG and the FDOT indicated that the minimum hydraulic conductivity levels had not been maintained, however, MEG performed additional geotechnical exploration, field and laboratory testing to confirm that the drainage basins did meet the project requirements.

4.0 Field Exploration and Laboratory Testing

Madrid Engineering has its own in-house fleet of six drill rigs designed for depths up to and exceeding 100 feet below ground surface. MEG has low ground pressure, tracked rigs to meet difficult access requirements often found in roadway widening and drainage pond areas. Also, MEG can and has mounted its equipment on barges for open water drilling, and we have even mounted our drills on an airboat hull for the most demanding site access problems. MEG's drilling department staff of 20 people includes dedicated and experienced drill crew chiefs and helpers. Our drillers are fast, efficient, and hardworking and our safety record is outstanding. As needed, MEG will also coordinate drilling services with other drilling firms, where MEG provides the supervision and monitoring. We take great pride in the selection and monitoring of our drilling companies. We only work with drilling companies that provide OSHA 40-hour safety trained personnel, and include certified well drilling contractors with the water management districts in Florida. In addition, their equipment inventory must be constantly expanding to meet the needs of our clientele. For environmental drilling, a self-contained decontamination unit includes steam cleaner, water and power supply.

GEOTECHNICAL LABORATORY AND SOIL SAMPLING

Madrid Engineering Group, Inc. has an in-house geotechnical laboratory. Mr. Madrid has over 25 years of experience in conducting and supervising geotechnical laboratory testing and laboratory research on a wide range of soil properties. Standard equipment is available for classification tests on soils in accordance to ASTM procedures. Our engineers are experienced in coring through concrete and asphalt surfaces for core sampling or subsurface sample recovery. MEG’s laboratory testing programs have the capability to characterize subsurface conditions such as compressible soils beneath proposed or existing structures, weak foundations, and weak soil and rock slopes. To quantify soil behavior under the proposed or current construction conditions, we evaluate , compressibility, expansion potential, gradation, moisture content and density. Subsidence Inves ga ons (over 2000) – Various Insurance Carriers - Mr. Madrid has acted as lead geotechnical engineer for over 2000 subsidence inves ga ons for houses and commercial buildings. The work has covered all of central Florida, from the west coast to the east coast, as far north as Jacksonville and south to Sebring. Mr. Madrid supervises a team of professionals including engineers, geologists and soil scien sts, se ng guidelines and protocols for the inves ga ons, and personally completes a fi nal technical review of each inves ga on before sending the fi nal report to the client.

Subsidence Inves ga ons and Expert Witness Tes mony – Various A orneys – Mr. Madrid is recognized as a leading expert in Florida subsidence inves ga ons and has been listed as expert for both plain ff and defendant. Du es include senior engineering review of reports by other geotechnical engineers, geologists and geophysical professionals for a second opinion. He has been deposed, tes fi ed in court, and been involved as an expert in arbitrated cases.

Mini-Pile Founda on Analysis and Design, Residen al – Numerous – For sites that have been iden fi ed as having sinkhole ac vity as a cause of damage, Mr. Madrid has been the lead engineer for the design of remedial mini-pile systems to support the structure and minimize future se lement damage. Design parameters include individual pile capacity, spacing and loca on of piles, and cost es mates. Mr. Madrid is also the engineer in charge of changes to the installa on based on site-specifi c criteria that supercedes the standard installa on specifi ca ons.

Grout Remedia on Plans – Numerous – Low mobility cement grout is used to fi ll voids in the upper, weathered por on of the limestone and to provide compac on to raveled soils. Mr. Madrid has specifi ed numerous grout plans to remediate subsurface subsidence caused by sinkhole ac vity. Cooling Pond Expansion, Hines Complex, Bartow, Florida – Mr. Madrid was responsible for the geotechnical inves ga on and analysis of 11,000 LF of new embankment. Tasks completed under his direc on included soil borings, slope stability analysis, seepage analysis, construc on methods, and stabiliza on recommenda ons for so /loose soils.

PhosphoCrete Chemical Stabiliza on of Compressible Clays, Polk County, Florida – In 1992, Mr. Madrid was Principal Inves gator for a research project that involved the stabiliza on of highly compressible waste phospha c clay with unslaked lime. Sponsored by the Florida Ins tute of Phosphate Research (FIPR) and the Civil Engineering Research Founda on (CERF) in Washington D.C., the research concluded that chemical stabiliza on using this technique was feasible, though not cost-eff ec ve. Years later, Mr. Madrid completed addi onal research using in-house facili es and test plots on private land to show that the method was successful using a proprietary blending of other, less expensive pozzolanic materials, called PhosphoCrete. This technology has now been designed and used for commercial applica ons, and is currently being researched by the University of South Florida Geotechnical Engineering Program.

Founda on Design using GeoFoam, Lakeland, Florida - Mr. Madrid designed the fi rst uses of GeoFoam founda ons for two adjacent residen al lots, underlain by over 20 feet of very so , compressible waste phospha c clay. The GeoFoam, which is a structural expanded polystyrene (EPS) material weighing 2 pcf, literally unloaded the soil to off set the weight of the house, thereby reducing the poten al for se lement to an acceptable limit.

Chemical Stabiliza on of Founda on Soils for Vibratory Equipment, Winter Haven, Florida – Packaging Corpora on of America (PCA) required stabiliza on of their corrugator and gluing machines, each weighing over 40,000 lbs, due to se lement of very loose subject to intense vibra ons from the equipment. Rather than moving the equipment off -line, which would shut down the plant for several weeks, Mr. Madrid devised a method to strengthen the soils, both in compression and tension, by injec ng hydrophobic polyurethane chemical grout that cements the sand par cles together to form a rock-like mass beneath the machines.

High Strength Geotex les and Geogrids for Founda on Stabiliza on, Mulberry, Florida – Mr. Madrid provided a unique site stabiliza on method to minimize diff eren al se lement of a proposed car wash facility that was underlain by over 20 feet of compressible soils. Near-surface sands were probed for so soil deposits near the surface that were then excavated and replaced with sand. Sand was compacted and two layers of high strength geotex les or geogrids were installed to provide a systems approach to minimize diff eren al se lement. The owner saved over $25,000 as compared to installa on of concrete pilings.

Polk County North Central Landfi ll Phase II Expansion, Eaton Park, Florida – Mr. Madrid was one of the lead geotechnical engineers for the design of a 60-acre landfi ll expansion at this facility. Under his direc on, MEG completed SPT borings and CPT soundings to determine subsurface profi les; and laboratory tes ng to determine soil classifi ca ons, strength, and compressibility parameters for landfi ll design. The design also included a sinkhole risk analysis. Mr. Madrid was also a senior consul ng engineer for construc on phase inspec on and monitoring ac vi es for this landfi ll.

Confi den al Landfi ll Sinkhole Risk Analysis, Polk County, Florida – In 2008, Mr. Madrid provided a sinkhole risk analysis for a confi den al client regarding a poten al expansion of an exis ng landfi ll. It was Mr. Madrid’s opinion that a sinkhole lineament existed that had not been adequately addressed, and posed a higher risk than was

McIntosh Power Plant Ash Landfi ll, City of Lakeland, Florida – Lead geotechnical engineer for the analysis of two poten al sites. Completed site suitability, soil borings, groundwater monitor well installa on and profi ling, bathymetric surveys, stability analysis, etc.

Class III Landfi ll Geotechnical Analysis, WasteCorp of Central Florida, Ft. Meade, Florida – Lead geotechnical engineer for founda on design of a 20-acre landfi ll site on a reclaimed phosphate pit. Founda on analysis included extensive slope stability analysis and waste materials characteriza on. Poten al fi ll materials included power plant ash.

C&D Landfi ll Expansion – Pembroke Landfi ll, Ft. Meade, Florida – Ver cal expansions of exis ng cells and opening of new cells required detailed materials characteriza on, slope stability analysis, cover material tes ng and design, and extensive coordina on with FDEP regulators. Mr. Madrid analyzed a unique soil stabiliza on technique using concrete rubble to improve strength in the underlying clays.

Phosphogypsum Cover CQA Engineer, Bartow, Florida - Provided closure construc on quality assurance for 90-acre facility, including lining of three ponds with 60-mil HDPE geosynthe c liners.

Annual Inspec on of Dams – Castcrete Facility, Thonotosassa, Florida – Mr. Madrid provides an annual inspec on of stormwater facili es for this heavy industrial facility that produces various pre-cast concrete products. Due to the size of the facility, the volumetric capacity of the ponds, and the height of some berms over the surrounding area, the stormwater ponds are actually classifi ed as “dams” and subject to addi onal scru ny according the FDEP guidelines.

Na onal Maglev Ini a ve System Concept Development – Es mated project cost $2 million - Assistant project manager for geotechnical and environmental aspects of a magne cally levitated vehicle and associated engineer- ing systems. This research project for the Federal Government involved development of concepts and design pa- rameters for a mass transporta on vehicle capable of a velocity of 300 MPH using electromagne c propulsion and levita on. Mr. Madrid supported the interdisciplinary team of scien sts and engineers to develop such a system for possible u liza on to connect urban areas of the United States.

County Road 540 Widening and New Construc on – Mr. Madrid was senior project manager and principal geotechnical engineer for three phases of expansion of this road project. The fi rst was the crossing of an unreclaimed phosphate pit called Triangle Lake. This “lake” was only accessible for explora on by MEG’s drill, mounted on an airboat hull, and pulled across the lake by a front end loader. A bridge was constructed on concrete pilings based on the informa on provided by the geotechnical inves ga on. Phase II and Phase III of the road were expansions of the exis ng two-lane road over a distance of 6.1 miles, and involved retaining walls, and fi ll sec ons, reten on pond design, founda ons for ligh ng and signaliza on, and roadway soil survey analysis.

Veterans Expressway, Tampa, Florida - Es mated Project Cost: $ 332,000,000 - General Consultant for geotechnical and hazardous/petroleum materials for this proposed 12-mile roadway facility. Larry Madrid was a senior consultant and coordinator for all geotechnical ac vi es. Coordinated and reviewed geotechnical subconsultant design work elements for each of the 7 sub-sec ons of the roadway; reviewed borrow pit availability for embankment construc on; and managed a team of geotechnical engineers that provided wave equa on analysis and construc on monitoring of test piles for over 50 bridge structures. Also supervised projects involving abandonment and redevelopment of SWFWMD deep wells and reloca on of several FDEP monitoring wells near the Expressway. Polk Parkway, Lakeland, Florida - Es mated Project Cost: $ 365,000,000 - Mr. Madrid’s experience on the Polk Parkway goes back to 1984, when he was hired as a staff engineer to work on the corridor selec on and geotechnical analysis from SR37A to past Hwy 98 (approximately 3 miles). The work involved hundreds of soil borings and underground mapping of complex mined areas. Later, when the project responsibility was assumed by the FDOT, he was the Project Manager for geotechnical design of roadway and bridge structures in Sec on 2 of the Parkway (approximately 3 miles). Mr. Madrid was the chief geotechnical engineer for this sec on, and supervised the soil survey and bridge founda on designs. Du es also included project management and engineering input into the design of two wetland mi ga on areas totaling 160 acres, and the enhancement of exis ng wetlands in the vicinity of the proposed toll road. This project involved assessment of the exis ng parcels, determina on of seasonal high water levels and wetland boundaries, and integra on of the proposed constructed wetlands with the exis ng wetlands surrounding the area. Both surface and groundwater hydrologic studies were completed. Mr. Madrid managed a mul -disciplinary group of professionals, including soil scien sts, hydrogeologists, civil engineers, geotechnical engineers, and wetland ecologists. Mr. Madrid was also a technical consultant to the other geotechnical engineering company, PSI, who was responsible for the remaining 16 miles of the road.

Cape Coral Bridge, Ft. Myers, Florida - Es mated Project Cost: $12,600,000 – Mr. Madrid was responsible for fi eld geotechnical inves ga ons, which included barge drilling adjacent to the exis ng bridge and soil borings along the mainland approaches. He was also in charge of laboratory tes ng, founda on design, and test pile monitoring on this 3400-foot long bridge. 24-inch square, pre-stressed concrete piles with a design capacity of 200 tons each were dynamically tested with pile driving analyzer (PDA) equipment. Results of the test pile driving confi rmed that the soil condi ons were rela vely uniform across the site, which allowed the produc on piles to be cast the same length; thereby off ering signifi cant cost savings to the client.

Lake Panaso ee Restora on (SWFWMD) - Mr. Madrid was a lead geotechnical engineer for the restora on of this 4820-acre lake in Sumter County, Florida. He was responsible for collec on of sediment and upland soil samples, laboratory tes ng and characteriza on, and dredged volume predic ve modeling. Phase II (of 6 phases), involves removal of approximately 4.8 million cubic yards of sediment that has silted in the lake. The total budget for the project is $26 million.

Coleman Landing Pilot Dredging Project (Florida Game & Freshwater Fish Commission) – This was the test pilot project for Lake Panaso ee, and involved dredging a 300-foot wide channel for boat access to the lake, and determined the feasibility of dredging the rest of the lake. Mr. Madrid directed the geotechnical inves ga on of the sediment and upland disposal areas, and the preliminary design of the disposal area and return water system.

Unnamed Lake Feasibility Study (Confi den al Client) – Mr. Madrid has inves gated the feasibility of using a proprietary water addi ve to s mulate microbial diges on of organic sediments. In the future, this process may be used to reduce the volume of organic sediments within lakes without the need for hydraulic dredging.

Lake Maggiore Restora on (City of St. Petersburg) – Mr. Madrid was a technical consultant for the contractor that was low bidder for this 400-acre lake with 2.2 million cubic yards of sediment.

Banana Lake Restora on, Lakeland, Florida - Es mated Project Cost: $2,000,000 - This SWIM project was the fi rst major lake restora on undertaken in the state of Florida. A major environmental eff ort which took over 4 years to complete, tasks included feasibility studies, conceptual design, environmental permi ng, plans prepara on, bidding, and construc on monitoring. Involved in all phases of the project, and the engineer of record. Mr. Delashaw currently serves as the Chief Geotechnical Engineer for Madrid Engineering Group and is the project principal and/or technical lead for most geotechnical projects performed by MEG. His primary experience includes project management and technical direc on of geotechnical engineering projects, geotechnical fi eld inves ga ons, and analyses for numerous public and private sector clients. Much of his over 21 years of geotechnical engineering experience has been gained from working on projects in the areas of dam design and evalua on, shallow and deep founda on systems, earthworks and site development, soil improvement techniques, sinkhole inves ga on and evalua on, roadway soil surveys, pavement design, and various pipeline projects, including the use of trenchless construc on technologies. The project site consisted of both undisturbed and previously mined areas on a site near the Polk Parkway in Auburndale. MEG provided the geotechnical explora on and design including fi eld explora on plans, shallow founda on soil capacity analyses, se lement analyses, geotechnical parameters provided for roadway and structure design, and report prepara on. MEG is also contracted to provide QA/QC CMT tes ng during construc on for the contractor.

Geotechnical Principal-in-Charge, Harden Boulevard Access Road, Polk County (2011): Polk County planned to construct an approximately 600 foot long access road that progresses east from Harden Blvd. to provide access to a new development (residen al and/or commercial) in Lakeland, Florida. The proposed alignment passes through former phosphate mined lands that have undergone varying degrees of reclama on and approximately 2/3 of the alignment included very so waste phospha c clay deposits near the surface and to depths of up to 35 feet. MEG was requested to perform a geotechnical inves ga on and provide a geotechnical design to provide a stable, permanent pla orm for roadway embankment construc on along the proposed access road alignment. MEG’s design included CSV columns combined with a load transfer blanket (LTB) consis ng of mul ple layers of biaxial geogrid interbedded with compacted sandy fi ll. The system will distribute and transfer roadway, embankment, and vehicle loads to more competent soils at depth. CSV stands for “cement stabilized ver cal” columns, and is an in situ stabiliza on method which essen ally provides a rigid ver cal element through the so soil much like a pile with an added benefi t of improving the soil mass within the CSV treated area also. The LTB acts much like a bridge deck and transfers the more uniform embankment loads above it through the LTB into the individual CSV columns below it. The combined system provides a more uniform load response to the roadway embankment.

Project Principal, Publix Supermarkets General Merchandise Expansion Facility, Lakeland, Florida (2010): Senior geotech review for geotechnical site evalua on, site development, and founda on recommenda ons. Geotechnical Engineer, Herbert Hoover Dike, Major Rehabilita on and Repair Project, Reach 1A Construc on and Design Support Services, Port Mayacca, Florida (2006-2007): Quality Assurance consulta on and inspec on services to support USACE related to soil-cement-bentonite cutoff wall and other construc on ac vi es. Geotechnical Engineer/Resident Quality Assurance Engineer, Florida Power CorporaƟ on, Polk County, Florida (1992-1996): Involved with geotechnical aspects of the ini al phase 3,000 MW Hines Energy Complex power plant on phosphate mined lands including supervision of fi eld and laboratory tes ng programs, engineering analyses including large-strain and conven onal consolida on se lement, stability of phospha c clays under various solid waste disposal scenarios, and dam design including seepage and stability analyses. Professional Profile

Mr. Jordan serves Madrid Engineering Group, Inc. (MEG) as Chief Structural Engineer and Sr. Project Manager. His responsibilities include technical review and oversight of engineering projects and design activities; provides leadership to technical and support staff; serves a project principal; training of technical staff; and assists with formulation, development, and implementation of marketing strategies for the firm.

For ten years prior to joining MEG, Mr. Jordan served URS Corporation Southern as Sr. Civil Engineering Project Manager and manager of the Bartow, Florida office. His responsibilities included directing the activities of a staff of engineers, designers, and technicians; creating and implementing client service area policy; and establishing goals and objectives for this client service area. Other responsibilities included providing direct or indirect oversight for all civil engineering and planning projects, including PD&E and corridor studies, roadway design, bridge design, construction engineering and inspection (CEI) services, site design, structural design, drainage, water resources, utility systems, permitting activities, insurance claims and adjustment evaluations, right of way acquisition and condemnation, engineering studies, and other client service area activities.

Mr. Jordan has been certified as an expert in litigation and provided expert witness services and testimony in numerous Right-of-way acquisition and condemnation cases, construction claims and product liability cases, design errors and omissions, and others. Projects and cases include Dale Mabry Highway in Tampa, County Line Road in Lakeland, Meadowlands Wastewater Treatment Plant in Lakeland, Wimmer Construction Pipe Failure, Southern Machinery in Lakeland, Averett Septic Tank Company in Lakeland, Kreider Salvage Company in Desoto County, Davis Monuments in Lakeland, White Construction Company in Lakeland, and Highlands Fairways Utilities among others.

Mr. Jordan’s duties have included supervising roadway and bridge design projects; site design; drainage projects; water distribution and sewer collection system projects; water and sewage treatment system projects; and environmental projects; residential, commercial, and industrial structures; forensic investigations, and other types of engineering projects Representative roadway projects include County Line Road, Shepherd Road, Ewell Road, SR 50, US 17-92, US 17, Edgewood Drive, Kings Highway, and others. All of these involved upgrading 2-lane facilities to 4- and 6-lane roadways with a mix of urban and rural sections each extending several miles in total length. Project experience - Structural Engineering Forensic Evaluations, Insurance Damage Claims, Multiple Insurance Companies (1989-Present): Directly responsible for conducting numerous structural investigations, including forensic evaluations to determine probable failure chains for multiple insurers throughout the State of Florida. Project Manager, Polk County Engineering, 21st Street Bridge, Winter Haven, FL (1990 -1995): Directed staff and supervised design for constructing a replacement bridge over a man-made canal, northwest of the City of Winter Haven. Design elements included piling and abutments, approach slabs, bridge deck, utility supports, re-alignment of approach roadways, permitting, maintenance of traffic, and demolition of the existing bridge. Project Manager, Polk County Engineering, Study of 15 Secondary Road Bridges, Polk County, FL (1989): Conducted a detailed study of 15 secondary road bridges at various locations in Polk County. Bridge spans, construction methodology, age, and maintenance status were all different. The purpose of the study was to determine which of the bridges was functionally obsolete and no longer capable of providing service for, principally, school bus traffic.

Project Manager, City of Treasure Island, and Bascule Bridges, Treasure Island, FL (1985-1989): Served as the engineer of record for conducting biennial bridge inspections and developing condition ratings for 3 sets of causeway bridges and the central bascule bridge over the Intracoastal Waterway. Responsible for preparing repair plans to correct deficiencies discovered during the inspections. During this same period, major renovations to the steel portions of the bascule bridge were designed including replacement of major portions of the steel trusses, main girders, lift ram anchor bolts, and nose locks. Construction Engineering and Inspection (CEI) services were provided for all construction projects completed during this period.

Project Engineer, Numerous Clients, Industrial Plant Design, Worldwide, (1975-1983): Served as a design project engineer for completing all structural and civil engineering portions of large complex industrial projects involving many technical disciplines. Duties included structural design of large buildings utilizing various types of construction materials, foundations, and specialized structures to support process and mechanical devices for industrial complexes and clients in numerous countries around the world. Project Manager, City of Bartow/CRA Church Street Traffic Calming, Bartow, Florida (2008): Directed all staff activities for conducting an existing conditions analysis, survey, data collection, coordination with stakeholders, traffic study, existing conditions analysis, and development of preliminary design plans for 1,500 lineal feet of residential street within one of the City of Bartow’s preeminent Historic Districts. The project’s objective was to develop plans consistent with the City and CRA’s goal for calming the rate and volume of traffic utilizing this residential street as an access corridor instead of a destination. Tools evaluated included landscaping, hardscaping, geometric changes, curbing, speed tables, and other traffic calming devices.

Project Manager, City of Bartow/CRA Main Street/2nd Ave./1st Ave. Intersection Improvement, Bartow, Florida (2008): Directed all staff activities for conducting the traffic study necessary to evaluate alternatives for improving this complicated offset intersection. The traffic study proceeded jointly with Church Street Traffic Calming since both function together. Work elements included traffic counts, turning movement counts, vehicle splits, field inventory for peak hour evaluations, crash data, future vehicle demand, existing conditions analysis, modeling of traffic conditions with various types of intersection geometry and influence from neighboring streets. Deliverables included modeling data, development of concept plans, traffic study report, and recommendations for proposed alternative.

Project Manager, FDOT District One Districtwide Minor Design, Bartow, Florida (2000-2003): Oversaw this on-call services contract for minor design projects within the jurisdiction of FDOT District One. Projects have included redesign of the SR 60 - US 27 Interchange in Lake Wales and a county-wide sidewalk project in Highlands County. Work elements include survey, roadway design, drainage design, signalization, lighting, structures, signing and marking, traffic maintenance plans, and other construction drawing plan set requirements.

Project Manager, Carter Road Park Drainage Reconstruction, Polk County, Florida (1997): Responsible for evaluating the mechanism causing failure of large (90 inch and greater) subsurface drainage piping inthis heavily used park in Polk County. Once the failure mode had been identified, plans and specifications for system reconstruction were prepared along with construction documents. During construction, full-time CEI services were also provided to verify adherence with construction plans. Project Manager / Principal General Consultant Districtwide CEI Enhancement, FDOT District One, Florida (1996- 1998): Managed districtwide construction engineering and inspection (CEI) services for FDOT District One. Assigned inspection projects included landscaping and irrigation, streetscape, sidewalk, lighting, signing, and historic building restoration.

Project Manager, Marcum Road Widening Project, Polk County, Florida (1998-2001): Oversaw this major roadway widening project for Polk County that envisions widening nearly two miles of roadway from two- lanes to five- lanes. Work elements include corridor analysis and lineament studies to determine the optimum alignment and typical section while accommodating major Lakeland Electric overhead transmission lines and Florida Gas Transmission high pressure natural gas transmission lines. Other work elements include detailed design, plans production, permitting, and CEI.

Project Manager, Study and Evaluation of 35 Problem Intersections, Polk County, Florida (1999): Managed the study and evaluation phase of 35 problem intersections in Polk County that were half of the 70 intersections included in a major transportation bonding program. Work elements included field evaluation of each intersection, reviewing available traffic and accident data, and devising a plan to effect improvements within existing right of way or that required minimal right of way acquisition. Cost estimates were also developed to account for proposed improvements along with illustrative sketches. A report was developed for each intersection to document the study.

Project Manager, Four Intersection Improvement Projects, Polk County, Florida (1999-2000): Managed the design phase of four problem intersections included in Polk County’s Transportation Improvement Program. Work elements included field survey, roadway design, drainage improvements, signalization, utility accommodation, signing and marking, traffic maintenance plans during construction, right of way mapping, permitting, and other construction drawing plan set requirements.

Project Manager, Pine Level Railroad, Desoto County, Florida (1989-1991): Performed and directed a corridor study for the extension of an industrial railroad spur from CSX mainline northwest of the city of Arcadia for more than six miles to a proposed industrial complex. Project Manager, Pine Level Road, Desoto County, Florida (1989-1001): Performed and directed a corridor study for the creation of a new 6 mile roadway between SR 70 and SR 72 west of the city of Arcadia to provide access to a proposed industrial complex. The study encompassed environmental assessments, alignment and geometrics, bridges and other structures, intersections, signalization, constructability, and cost estimates. Project Manager, Howard Frankland Bridge Erosion Control Study, FDOT District Seven, Hillsborough County, Florida (1996): Charged with developing a plan for controlling erosion of the Howard Frankland Bridge causeways during major storm events. The study evaluated hydro-dynamic storm conditions that would result in over-topping of the causeway bulkheads, alternative methods for sealing leaking bulkhead joints, and evaluating various types of salt tolerant vegetation for the roadway shoulders.

Project Manager, Downtown Bartow Streetscape, FDOT District One, Bartow, Florida (1997): Provided all construction engineering and inspection (CEI) services for this inter-modal Surface Transportation Enhancement Act (ISTEA) project. Specific CEI duties included providing full time inspection services as the representative of FDOT to ensure full compliance with all construction plans and specifications; reviewing shop drawings; monitoring Equal Employment Opportunity (EEO) compliance by the contractor; maintaining a daily log of construction activities and materials installed; assisting the contractor with plan interpretation; verifying installed quantities; verifying landscaping materials; verifying installation of special brick borders, tree grates, and period lighting fixtures; checking maintenance of traffic (MOT) items; and completing final estimates.

Project Experience - Civil Engineering

Project Manager, Pine Level 630 kV Transmission Line, Desoto County, Florida (1989-1991): Performed and directed a corridor study for the creation of a new 630 kV transmission line linking a generator in a proposed industrial complex with the state power grid. The study evaluated alternate alignments and connection points for several corridors up to 20 miles in length and encompassed environmental assessments, alignment and geometrics, maintenance, constructability, and cost estimates.

Project Manager, Plant City Commerce Center Utility Extensions, Plant City, Florida (1995-1996): Managed this infrastructure project that involved extending water, wastewater, and reuse lines from the city of Plant City over 3 miles to provide services to newly annexed areas of the city. The project required evaluating alternative corridors, alignments, environmental assessments, constructability, and cost estimates. Once the corridor was chosen, final design of the selected alternate proceeded and resulted in preparation of construction plans and specifications, contractor bid documents, and permits. Subsequently, construction engineering and inspection services (CEI) and construction contract administration services were provided.

Project Manager, Derby Ditch Water Control Structure, Polk County, Florida (1993): Oversaw the design of a flood control structure and water quality improvement project near the city of Auburndale in Polk County, Florida. The project included evaluating system hydraulics, design of a weir and control structure, design of a water management system of ponds, vegetation design for water quality improvement and ease of maintenance, preparation of construction plans and specifications, and providing construction engineering and inspection (CEI) services.

Project Manager, Statewide Stormwater Management Plan, FDOT Central Office, Florida (1992-1993): Managed this project that resulted in developing a statewide stormwater management plan to be implemented in each district in compliance with Federal NPDES Stormwater requirements.

Project Manager, Districtwide Environmental Permit Compliance, FDOT District Five, Florida (1994-1998): Responsible for this districtwide project that required developing procedures for inspecting, monitoring, maintaining, and reporting in place environmental systems to maintain compliance with permitted conditions. Project Manager, Districtwide NPDES Implementation, FDOT Districts One, Five, and Seven, Florida (1994-1998): Responsible for NPDES implementation projects that included developing a comprehensive GIS system to track permit conditions, performing permit compliance evaluations, developing public information programs, developing methods for evaluating improvements in water quality, providing training for FDOT maintenance and construction personnel, and developing annual reports.

Project Manager, Winston Peaking Station, City of Lakeland, Polk County, Florida (2001): Provided site evaluation, location, and permitting assistance to the City of Lakeland for this 50 MW peaking generator station located on Airport Road on the west side of the city and north of Lakeland Linder Regional Airport. Services included land use studies, site utilities studies, stormwater studies, development standards, and assistance with site review and approval process.

Project Manager, Sun Air Stormwater Pump Station Study, Polk County, Florida (2007): Managed the detailed study of a land-locked lake in a subdivision that had flooded roads, wastewater lift station, and other infrastructure during both 2004 and 2005. During the flooding event, Polk County Natural Resources had installed temporary pumps to alleviate the situation. The project team was tasked with evaluating construction of a permanent pump station, determining 100-year flood elevations, determining a pipeline route, and estimating costs.

Project Manager, Sun Air Stormwater Pump Station Design, Polk County, Florida (2008): Managed the preparation of design plans and easement documents to implement the recommendations of the Sun Air Stormwater Pump Station Study and obtaining permits. Permitting involved interacting with two water management districts for approval since the District dividing line separated the subdivision. Design included converting an existing wastewater lift station to become the emergency pump station since it was being abandoned infavorof constructing a new wastewater pump station for Polk County Utilities.

Project Experience - Site Developments

Project Manager, Polk County Administrative Complex Parking and Drainage Study, Polk County, Florida (2007- 2008): Managed and directed a comprehensive parking and drainage study to evaluate constructing additional ground level parking areas for the Polk County Administration Building, Courthouse, Sheriff’s Offices and other administrative areas. Program elements included conducting a detailed traffic study, usage, signage, consolidation and relocation of existing stormwater management areas, removal of landscape island, restriping, construction of stormwater vaults, and other system improvements.

Project Manager, Cypress Youth Soccer Complex, City of Lakeland, Florida (2007-2008): Managed site design for a proposed 6-field soccer complex to be developed on a 40-acre site previously mined for phosphate rock and subsequently reclaimed. Site design included extensive geotechnical evaluation to determine locations for relic phosphatic waste clays, mapping of environmentally sensitive areas, conducting a transportation study for potential impacts to local road networks, site utility design, drainage, landscaping, irrigation, lighting, permitting, developing detailed cost estimates, developing a phasing plan, and other program elements.

Project Manager, Hunt-Fountain Park Drainage Design, Polk County, Florida (2007): Managed the design plan preparation to implement the recommendations of an earlier study to overcome persistent drainage problems at a regional Polk County Leisure Services park in northwest Polk County.

Project Manager, IMC-Agrico Park Boardwalk Expansion, Polk County, Florida (2006): Managed design project to expand the boardwalk facility along the Peace River and construct the next two phases of the boardwalk master plan. Project elements included field survey to recover the master planned alignments, environmental review of the alignments, boardwalk structural design, preparation of design plans, obtaining environmental and construction permits or approvals, and providing construction administration services.

Project Manager, Lake Blue Park Design, Polk County, Florida (2006): Managed design team implementing the recommendations of the feasibility study for this new park facility. Project Manager, Snively Elementary School Recreational Complex, Polk County, Florida (1994-1995): Oversaw planning, construction plans preparation, permitting, and CEI services. This is a joint use facility shared by Snively Elementary School during the day and available to the community in the afternoon, evenings, weekends, and holidays. The park features basketball courts, ball fields, soccer fields, infant playground, jogging trails, covered picnic areas, and restroom facilities. Planning and design features included a facility layout that minimized removal of existing trees, coordination and integration into the existing school grounds, security and after-hours access, permitting and stormwater management, ADA compliance, utility extensions, and landscaping. Park development was the result of a cooperative agreement between Polk County Parks and Recreation Division, Polk County School Board, and the federal government through a Community Development Block Grant.

MEG Qualifications

MEG has completed the construction of our new 750 SF construction materials laboratory including a secure storage area for nuclear gauges and a concrete curing room.

The addition will work in conjunction with our existing laboratory facilities thus doubling our lab size. The new laboratory is equipped with state-of-the-art soil and concrete testing equipment, including new Forney and ELE digital loading frames for LBR and concrete testing, automatic Proctor hammer, Humboldt tri-axial permeability apparatus and climate-controlled concrete curing facility. It will also house our Nuclear Soil Density Gauge, which MEG is now licensed by the State of Florida, Department of Health to own, store, transport and use. Currently, MEG uses Troxler Model 3430 /density gauges to measure in-situ soil density and moisture content. The MEG laboratory and field personnel are equipped and certified to perform all standard soil and concrete testing required by FDOT project specifications.

We have pursued and obtained accreditation for various soil testing methods based on the guidelines of AASHTO R18, Standard Recommended Practice for Establishing and Implementing a Quality Management System for Construction Materials Testing Laboratories. The accrediting body is the Construction Materials Engineering Council, Inc. (CMEC), a not-for-profit organization based in Central Florida whose stated goals are to provide accreditation, education, and certification to the construction and engineering industry in Florida and elsewhere. Accreditation has been granted for the test methods listed on the following page.
