TUCKER HIGH SCHOOL New Student Registration Requirements TUCKER HIGH SCHOOL New Student Registration Requirements I. Students Enrolling From Another Dekalb School MUST Present: a. Proof of residency – Current rental/lease contract, warranty deed and utility bill. In case of co- residency, an “Affidavit of Residence” will need to be completed. b. Withdrawal Records NOTE: students enrolling from another Dekalb school will be accepted on a contingency basis until documents are received from the previous school. II. Students Enrolling From Another School In Georgia MUST Present: a. Georgia Certificate of Immunization – Georgia Form 3032 or 3231 b. Georgia Certificate of Vision, Hearing, Dental – Georgia Form 3300 c. Withdrawal Form from previous school attended d. Transcript of high school credits e. Discipline Letter from previous school in the State of Georgia f. Birth Certificate g. Social Security Card or signed Objection Form h. Proof of Residency – current rental/lease contract, warranty deed and utility bill. In case of co- residency, an “Affidavit of Residence” will need to be completed. III. Students Enrolling From A School Out of State MUST Present: a. Georgia Certificate of Immunization – Georgia Form 3032 or 3231 b. Georgia Certificate of Vision, Hearing, Dental – Georgia Form 3300 NOTE: Any student enrolling from an out of state school MUST have health records listed above (a) and (b) changed into a GEORGIA Certificate of Immunization and a GEORGIA Certificate of Hearing, Vision, Dental. This can be done at any Dekalb County Health Department or at your physician’s office. c. Withdrawal Form from previous school attended d. Transcript of high school credits e. Birth Certificate f. Social Security Card or signed Objection Form g. Proof of Residency – current rental/lease contract, warranty deed and utility bill. In case of co- residency, an “Affidavit of Residence” will need to be completed. IV. This resulted in eleven consecutive posters, which when mounted side by side, create a %foot mural. The timeline exhibits and describes the millstones of physics in images and words. The timeline functions as a chronology, a work of art, a permanent open textbook, and a gigantic photo album covering a hundred years in the life of the community of physicists and the existence of the American Physical Society . Each of the eleven posters begins with a brief essay that places a major scientific achievement of the decade in its historical context. Large portraits of the essays’ subjects include youthful photographs of Marie Curie, Albert Einstein, and Richard Feynman among others, to help put a face on science. Below the essays, a total of over 130 individual discoveries and inventions, explained in dated text boxes with accompanying images, form the backbone of the timeline. For ease of comprehension, this wealth of material is organized into five color- coded story lines the stretch horizontally across the hundred years of the 20th century. In 1975, he served as a consultant to the Senate Committee on Veterans Affairs and became a professional senate staff member from 1975-1977. He was appointed as Administrator of the U.S. Veterans Administration by President Jimmy Carter in 1977 and served until 1981.
