The CHNA process undertaken by Ochsner Medical Center, with project management and consultation by Tripp Umbach, included extensive input from persons who represent the broad interests of the community served by the hospital facility, including those with special knowledge of public health issues, data related to vulnerable populations and representatives of vulnerable populations served by the hospital. Tripp Umbach worked closely with leadership from Ochsner Medical Center and a project oversight committee to accomplish the assessment. 2 Community Health Needs Assessment Ochsner Medical Center Tripp Umbach Community Definition While community can be defined in many ways, for the purposes of this report, the Ochsner Medical Center (OMC) community includes the Ochsner Medical Center, Ochsner Medical Center – Westbank Campus, and Ochsner Baptist, a Campus of Ochsner Medical Center facilities and their community, zip‐code definitions.
