(E.) 266 of 2021 PREVENTION AND CONTROL OF DISEASE (COMPULSORY TESTING FOR CERTAIN PERSONS) REGULATION Compulsory Testing Notice I hereby exercise the power conferred on me by section 10(1) of the Prevention and Control of Disease (Compulsory Testing for Certain Persons) Regulation (the Regulation) (Chapter 599, sub. leg. J) to:— Category of Persons (I) specify the following category of persons [Note 1]:— (a) any person who had been present at Citygate (only the shopping mall is included), 18–20 Tat Tung Road & 41 Man Tung Road, Tung Chung, Lantau Island, New Territories, Hong Kong in any capacity (including but not limited to full-time, part-time and relief staff and visitors) at any time during the period from 10 April to 26 April 2021; (b) any person who had been present at Zaks, Shop G04 on G/F & Shop 103 on 1/F, D’Deck, Discovery Bay, New Territories, Hong Kong in any capacity (including but not limited to full-time, part-time and relief staff and visitors) at any time during on 11 April 2021; (c) any person who had been present at Novotel Citygate Hong Kong, 51 Man Tung Road, Tung Chung, Lantau Island, New Territories, Hong Kong in any capacity (including but not limited to full-time, part-time and relief staff and visitors) at any time during the period from 10 April to 11 April 2021; (d) any person who had been present on the following premises in any capacity (including but not limited to full-time, part-time and relief staff and visitors) at any time on 21 April 2021:— (1) 13/F, Cameron Commercial Centre, 458–468 Hennessy Road, Wan Chai, Hong Kong; (2) 8/F, Immigration Tower, 7 Gloucester Road, Wan Chai, Hong Kong; (e) any person who had been present at Ling Liang Church Sau Tak Primary School, 35 Man Tung Road, Tung Chung, Lantau Island, New Territories, Hong Kong in any capacity (including but not limited to full-time, part-time and relief staff, students and visitors) at any time during the period from 8 April to 4 May 2021; (f) any person who had been present at Annunciation Catholic Kindergarten, 11 On Yin Street, Tsuen Wan, New Territories, Hong Kong in any capacity (including but not limited to full-time, part-time and relief staff, students and visitors) at any time during the period from 22 April to 3 May 2021; (g) any person who had been present at Wellcome, Shop 9A, Phase 1, Allway Gardens, Tsuen Wan, New Territories, Hong Kong in any capacity (including but not limited to full-time, part-time and relief staff and visitors) at any time on 22 April, 24 April, 25 April or 2 May 2021; (h) any person who had been present at Hymn Piano Company, Block E, Allway Gardens, 187 Tsuen King Circuit, Tsuen Wan, New Territories, Hong Kong in any capacity (including but not limited to full-time, part-time and relief staff and visitors) at any time on 22 April 2021; (i) any person who had been present at Yip’s Children’s Choral & Performing Arts Centre, Unit 2302-2303, 23/F, Tower 2, Metroplaza, Kwai Fong, New Territories, Hong Kong in any capacity (including but not limited to full-time, part-time and relief staff and visitors) at any time on 24 April 2021; (j) any person who had been present at Discovery Park (only the shopping mall is included), 398 Castle Peak Road Tsuen Wan, Tsuen Wan, New Territories, Hong Kong in any capacity (including but not limited to full-time, part-time and relief staff and visitors) at any time on 26 April 2021; (k) any person who had been present at Curry Lounge at Shop S, G/F, SeaView Crescent, 8 Tung Chung Waterfront Road, Tung Chung, Lantau Island, New Territories, Hong Kong in any capacity (including but not limited to full-time, part-time and relief staff and visitors) at any time on 10 April 2021; (l) any person who had been present on the following premises in any capacity (including but not limited to full-time, part-time and relief staff, students and visitors) for more than 2 hours at any time during the period from 21 April to 4 May 2021:— (1) The Evangelical Lutheran Church of Hong Kong Wo Che Lutheran School, 11 Hip Yan Street, Wo Che Estate, Sha Tin, New Territories, Hong Kong; (2) Leung Kui Kau Lutheran Primary School, 5 On King Street, Sha Tin, New Territories, Hong Kong; (3) Kowloon Tong School (Primary Section), NKIL 2721, Surrey Lane, Cumberland Road, Kowloon Tong, Kowloon, Hong Kong; (4) Delia School of Canada, Tai Fung Avenue, Taikoo Shing, Quarry Bay, Hong Kong; (5) St. John the Baptist Catholic Primary School, 29 Yee On Street, Kwun Tong, Kowloon, Hong Kong; (6) Ng Wah Catholic Primary School, 5 Choi Hung Road, San Po Kong, Kowloon, Hong Kong; (7) S.K.H. St. Christopher’s Nursery (Kwai Chung), Unit No. 1, Podium Level 3, Kwai Chung Shopping Centre, Kwai Chung Estate, New Territories, Hong Kong; (8) Tai Po Baptist Kindergarten Tin Chak Estate Branch, G/F, Chak Sing House, Tin Chak Estate, Tin Shui Wai, New Territories, Hong Kong; (9) Po Leung Kuk Yick Kwai Fong Kindergarten, Level 2, Greenland Garden, Tuen Mun Town Lot 242, Tuen Mun, New Territories, Hong Kong; (10) Anchors International Nursery, Kindergarten Premises, Constellation Cove, 1 Hung Lam Drive, Tai Po, New Territories, Hong Kong; (11) Po Leung Kuk Ng Tor Tai Kindergarten, G38 on G/F & F38 on 1/F, Commercial Complex, Sui Wo Court, Sha Tin, New Territories, Hong Kong; (12) The Church of Christ in China, Hong Kong Chi To Church Kei Pok Kindergarten (Tseung Kwan O), G/F, Kwong Ying House, Kwong Ming Court, Tseung Kwan O, New Territories, Hong Kong; (13) Shun Tak Fraternal Association Leung Lee Sau Yu (Shatin) Kindergarten, G/F, Wings A & B, Hin Tak House, Hin Keng Estate, Sha Tin, New Territories, Hong Kong; (14) Deborah English Kindergarten (Tseung Kwan O)/Deborah International Pre-School/Play School (Bauhinia Garden), Kindergarten Block, Bauhinia Garden, 11 Tong Chun Street, Tseung Kwan O, New Territories, Hong Kong; (15) CCC Mongkok Church Jeannette Kindergarten, G/F, 1/F & 3/F, Mongkok Christian Centre, 56 Bute Street, Mongkok, Kowloon, Hong Kong; (16) Hong Kong Baptist University – Kindergarten, 3/F–4/F, Madam Chan Wu Wan Kwai SCE Tower, Baptist University Road, Kowloon Tong, Kowloon, Hong Kong; (17) Semple Kindergarten, G/F to 3/F, 10 Willow Street, Tai Kok Tsui, Kowloon, Hong Kong; (18) ABC Pathways International Kindergarten (Whampoa Garden), Shop No. G25, G/F, Site 11, Whampoa Garden, 6 Tak Hong Street, Hung Hom, Kowloon, Hong Kong; (19) Po Leung Kuk Yick Chark Fung Kindergarten, G/F, Po Yan House, Po Lam Estate, Tseung Kwan O, New Territories, Hong Kong; (20) Po Leung Kuk Tam Au-Yeung Siu Fong Memorial Kindergarten, Shop 3B, G/F, Site 7, Whampoa Garden, Hung Hom, Kowloon, Hong Kong; (21) The Christian and Missionary Alliance Scholars’ Anglo-Chinese Kindergarten, 100 Shing Tai Road, Heng Fa Chuen, Hong Kong; (22) Yuen Long Public Middle School Alumni Association Lau Leung Sheung Memorial Kindergarten, Wings A & B, G/F, Yiu Chak House, Tin Yiu Estate, Tin Shui Wai, Yuen Long, New Territories, Hong Kong; (23) Fanling Baptist Church Lui Ming Choi Kindergarten, 3 Sha Tau Kok Road, Fanling, New Territories, Hong Kong; Requirements and Procedure for the Test (II) require every person who falls within the above category to undergo a polymerase chain reaction-based nucleic acid test for the coronavirus disease 2019 (the Specified Test) in compliance with the requirements and procedure set out below:— (a) for a test at mobile specimen collection stations:— (i) attend one of the mobile specimen collection stations [Note 2] set up by the Government on or before 7 May 2021; (ii) undergo the Specified Test, including providing a sample, in accordance with the instructions given by the staff at the stations; and (iii) keep the SMS (mobile phone text message) notification containing the result of the test, and provide the relevant SMS notification for checking by a prescribed officer when the prescribed officer requires the person to provide information about his or her undergoing the Specified Test; OR (b) for a test at Community Testing Centres:— (i) attend one of the Community Testing Centres [Note 3] set up by the Government on or before 7 May 2021 during its opening hours; (ii) undergo the Specified Test, including providing a sample, in accordance with the instructions given by the staff at the centre; and (iii) keep the SMS (mobile phone text message) notification containing the result of the test, and provide the relevant SMS notification for checking by a prescribed officer when the prescribed officer requires the person to provide information about his or her undergoing the Specified Test; OR (c) for a test with a specimen bottle obtained:— (i) obtain a specimen bottle from one of the 121 post offices, 47 general out-patient clinics of the Hospital Authority or vending machines set up at 20 MTR stations [Note 4]; (ii) collect deep throat saliva sample in the specimen bottle as per the guidelines provided together with the specimen bottle; (iii) return, by 7 May 2021, the specimen bottle with the sample to one of the designated collection points [Note 4]; and (iv) keep the SMS (mobile phone text message) notification containing the result of the test, and provide the relevant SMS notification for checking by a prescribed officer when the prescribed officer requires the person to provide information about his or her undergoing the Specified Test; OR (d) for a test using a specimen bottle distributed to the buildings specified in Part (I) above by the Centre for Health Protection (CHP) (if applicable):— (i) obtain a specimen bottle from the CHP; (ii) collect relevant sample in the aforesaid specimen
