Tinea Infections: Athlete’s Fo ot, Jock Itch and Ringworm What is tinea? Tinea is caused by a fungus that grows on your skin, hair or nails. As it grows, it spreads out in a circle, leaving normal-looking skin in the middle. This makes it look like a ring. At the edge of the ring, the skin is lifted up by the irritation and looks like a red and scaly rash. To some people, the infection looks like a worm is under the skin. Because of the way it looks, tinea infection is often called “ringworm.” However, there really is not a worm under the skin. How did I get a ringworm/tinea? You can get a fungal infection by contact with person or environment. Some fungi live on damp surfaces, like the floors of showers or locker rooms. You can even catch a fungal infection from your pets. Dogs and cats, as well as farm animals, can be infected with a fungus. Often this infection looks like a patch of skin where fur is missing (mange). What areas of the body are affected by tinea infections? Fungal infections are named for the part of the body they infect. Tinea corporis is a fungal infection of the skin on the body. If you have this infection, you may see small, red spots that grow into large rings almost anywhere on your arms, legs or chest. Tinea pedis is usually called “athlete’s foot.” The moist skin between your toes is a perfect place for a fungus to grow. The skin may become itchy and red, with a white, wet surface. -

Some of the most commonly encountered diseases of the integument are dermatomycoses. Human or animal contact may be the source of all types of tinea infections, occasional candidal infections, and some other types of superficial or deep fungal infections. This narrative review focuses on the epidemiology, clinical features, diagnosis and treatment of anthropophilic dermatophyte infections primarily found in North America. -

Therapeutic Class Overview Antifungals, Topical INTRODUCTION The topical antifungals are available in multiple dosage forms and are indicated for a number of fungal infections and related conditions. In general, these agents are Food and Drug Administration (FDA)-approved for the treatment of cutaneous candidiasis, onychomycosis, seborrheic dermatitis, tinea corporis, tinea cruris, tinea pedis, and tinea versicolor (Clinical Pharmacology 2018). The antifungals may be further classified into the following categories based upon their chemical structures: allylamines (naftifine, terbinafine [only available over the counter (OTC)]), azoles (clotrimazole, econazole, efinaconazole, ketoconazole, luliconazole, miconazole, oxiconazole, sertaconazole, sulconazole), benzylamines (butenafine), hydroxypyridones (ciclopirox), oxaborole (tavaborole), polyenes (nystatin), thiocarbamates (tolnaftate [no FDA-approved formulations]), and miscellaneous (undecylenic acid [no FDA-approved formulations]) (Micromedex 2018). The topical antifungals are available as single entity and/or combination products. Two combination products, nystatin/triamcinolone and Lotrisone (clotrimazole/betamethasone), contain an antifungal and a corticosteroid preparation. The corticosteroid helps to decrease inflammation and indirectly hasten healing time. The other combination product, Vusion (miconazole/zinc oxide/white petrolatum), contains an antifungal and zinc oxide. Description of Condition: Onychomycosis is a common nail condition. It is a fungal infection of the nail that differs from bacterial infections (often referred to as paronychia infections). It is very common for a patient to present with onychomycosis without a true paronychia infection. It is also very common for a patient with a paronychia infection to have secondary onychomycosis. Factors that can cause onychomycosis include: (1) environment: dark, closed, and damp like the conventional shoe, (2) trauma: blunt or repetitive, (3) heredity, (4) compromised immune system, (5) carbohydrate-rich diet, (6) vitamin deficiency or thyroid issues, (7) poor circulation or PVD, (8) poor-fitting shoe gear, (9) pedicures received in places with unsanitary conditions. Nails that are acute or in the early stages of infection may simply have some white spots or a white linear line. Chronic nail conditions may appear thickened, discolored, brittle or hardened (to the point that the patient is unable to trim the nails on their own). Skin diseases are indicators of HIV/AIDS which correlates with WHO clinical stages. In resource limited environment where CD4 count is not readily available, they can be used in assessing HIV patients. The study aims to determine the mucocutaneous manifestations in HIV positive patients and their correlation with WHO clinical stages. A prospective cross-sectional study of mucocutaneous conditions was done among 215 newly diagnosed HIV patients from June 2008 to May 2012 at adult ART clinic, Wesley Guild Hospital Unit, OAU Teaching Hospitals Complex, Ilesha, Osun State, Nigeria. There were 156 dermatoses with oral/oesophageal/vaginal candidiasis (41.1%), PPE (24.4%), dermatophytic infections (8.9%), and herpes zoster (3.8%) as the most common dermatoses. The proportions of dermatoses were 4.5%, 21.8%, 53.2%, and 20.5% in stages 1–4, respectively. -

ABSTRACT Sixty-five compounds, including general microbicides, fungicides, antibiotics, and fumigants were screened for their sporicidal effect on Ascosphaera aggregata. The chalkbrood spore, the only readily accessible stage of the fungus, was unaffected by most of the materials tested. Only the halogenated general microbicides, iodine, hypochiorite, and stabilized dry chlorine, demonstrated consistent sporicidal activity at low concen- trations. Suggestions are made as to possible disease control strategies. Antifungal Drugs
Antifungal Drugs Antifungal or antimycotic drugs are those agents used to treat diseases caused by fungus. Fungicides are drugs which destroy fungus and fungistatic drugs are those which prevent growth and multiplication of fungi. Collectively these drugs are often referred to as antimycotic or antifungal drugs. General characteristics of fungus: Fungi of medical significance are of two groups: Yeast Unicellular (Candida, Crytococcus) Molds Multicellular; filamentous consist of hyphae (Aspergillus, Microsporum, Trichophyton) They are eukaryotic i.e. they have well defined nucleus and other nuclear materials. They are made of thin threads called hyphae. The hyphae have a cell wall (like plant cells) made of a material called chitin. The hyphae are often multinucleate. They do not have chlorophyll, can’t make own food by photosynthesis, therefore derive nutrients by means of saprophytic or parasitic existence. Cell membrane is made up of ergosterol. Types of fungal infections: Fungal infections are termed as mycoses and in general can be divided into: Superficial infections: Affecting skin, nails, scalp or mucous membranes; e.g. Tinea versicolor Dermatophytosis: Fungi that affect keratin layer of skin, hair and nail; e.g. Tinea pedis, rign worm infection. Candidiasis: Yeast infections caused by (Malassezia pachydermatis), oral thrush (oral candidiasis), vulvo-vaginitis, nail infection. Systemic/Deep infections: Affecting deeper tissues and organ they usually affect lungs, heart and brain leading to pneumonia, endocarditis and meningitis. Systemic infections are associated with immunocompromised patients, these diseases are serious and often life threatening due to the organ involved. Some of the serious systemic fungal infections in man are candidiasis, cryptococcal meningitis, pulmonary aspergillosis.
