CHAPTER 1 ABOUT THE INSTITUTE 1. Introduction 1.1 Chhattisgarh’s disabled population has witnessed a whopping rise of 48% from 2001 to 2011 with the number of disabled going up from 4.19 lakh to 6.24 lakh during the period. These include 2.31 lakh males and 1.88 lakh females.

1.2 The Composite Regional Centre, Rajnandgaon is an extension of the Swami Vivekananda National Institute for Rehabilitation, Training and Research (SVNIRTAR) Orissa. It was established on 25th June, 2016 to extend services that cater to multiple and diverse disabilities, under one roof. Today, it is fully-equipped and functional to provide rehabilitative and educational services to all types of disabilities. It is located at Old Hospital Campus Rajnandgaon, CRC offers end-to-end solutions for the Divyangjan in Chhattisgarh. Its services that range from the academic to clinical, it continues to be committed to further the cause of the Divyangjan. Persons having different disabilities, served by CRC, include persons with Locomotor Disability, Blindness, Low-vision, Hearing Impairment, Mental Retardation, Mental Illness, Leprosy Cured, Cerebral Palsy, Autism Spectrum Disorder, and Multiple Disability of all ages. Special emphasis is given to early intervention. A team including Assistant Professor Speech & Audiology, Assistant Professor Clinical Psychology, Assistant Professor Special Education, Lecturer in Occupational Therapy, Lecturer in Physiotherapist, Orientation & Mobility Instructor, Physical Medicine & Rehabilitation Specialist, Prosthetic & Orthotic Engineer, Rehabilitation Officer, Special Educationist, and Vocational Instructor work at the Centre on principles of trans-disciplinary approach.

1.3 The basic aim of CRC, Rajnandgaon is to create resources and infrastructure required for developing services for persons with disabilities including human resource development and research. The Centre is for facilitating capacity at local levels as well rather than letting it centralize in urban areas. CRC, Rajnandgaon building was equipped with good infrastructure and all assessment and therapeutic material to provide the best services to persons with disabilities. The teams of professionals are providing their services in different areas such as rehabilitation medicine, clinical & rehabilitation, parental training, speech, hearing & communication, special education, training for persons with deaf and blindness, physiotherapy & occupational therapy, early Intervention, prosthetics & orthotics, sensory integration, and community based rehabilitation.

2. Aims & Objectives

2.1 The Main Aims & Objectives of CRC: i. Manpower Development- Long Term, Short term courses, Training for rehabilitation personnel such as ANM, Anganwadi, Nurse, Multipurpose Rehabilitation Therapists & other personnel for the rehabilitation for Divyangjans. ii. Vocational Training, placement & rehabilitation of Divyangjans. 2.2 To achieve these objectives the CRC has Ten major Departments/Sections i.e. Administration, Accounts & Finance, Academics (HRD), Prosthetics & Orthotics, Physiotherapy, Occupational Therapy, Special Education, Clinical Psychology, Speech & Hearing, Vocational. 3. Mission: Through constant professional endeavors, empowering the Persons with Disabilities (Divyangjans) to access state of art rehabilitation services. 4. Vision: To improve the quality of life of Persons with Disabilities (Divyangjans) through providing quality professional services of Composite Regional Centre for Skill Development, Rehabilitation & Empowerment of Persons with Disabilities (CRC), Rajnandgaon . 5. Value Statement:

6. Management:

Composite Regional Centre for Skill Development, Rehabilitation & Empowerment of Persons with Disabilities (CRC), Rajnandgaon is active under the Administrative control of Swami Vivekananda National Institute of Rehabilitation Training & Research (SVNIRTAR), Cuttack, Odisha; CRC has to follow the Two Councils i.e. General Council (GC) & Executive Council (EC) comprising the representatives from Union & State Governments, renowned Social Workers, Orthopedics & Rehabilitation professionals, etc. govern the activities of the Institute. The Secretary to the Government of India, Department of Empowerment of Persons with Disabilities (Divyangjan), Ministry of Social Justice and Empowerment (MSJ&E), Government of India is the President of General Council & The Joint Secretary (DEPWD) to the Government of India, Department of Empowerment of Persons with Disabilities (Divyangjan), Ministry of Social Justice and Empowerment (MSJ&E), Government of India is the Chairperson of Executive Council. The Director, SVNIRTAR Cuttack, Odisha is the Member Secretary of both the Councils & is Head of SVNIRTAR as well as CRC’s Activity under the Administrative control of SVNIRTAR, Cuttack. The List of members of GC & EC are mentioned in Appendix – “A” & “B” respectively.

7. Executive Council (EC) & General Council (GC) Meetings:

As per the Bye-Law of SVNIRTAR, Cuttack; EC is to be conducted four times in a year and GC is to be held once in a year for framing the policies, monitoring the progress of activities & new initiatives etc. EC consists of 5 Members and The Joint Secretary (DEPwD), Department of Empowerment of Persons with Disabilities (Divyangjan), Ministry of Social Justice and Empowerment (MSJ&E), Government of India is the Chairperson. The GC consists of 8 members & the Secretary (DEPwD), Department of Empowerment of Persons with Disabilities (Divyangjan), Ministry of Social Justice and Empowerment (MSJ&E), Government of India is the President.

9.3 Assistance to Disabled Persons (ADIP) Scheme:

The ADIP scheme was introduced in the year 1981 for the Persons with Disabilities and implemented by the Ministry of Social Justice & Empowerment, Govt. of India. The Mail objective of the scheme is to help the disabled persons and bring suitable, durable, scientifically manufactured, modern, light weight aids & appliances within their reach, which is essential for social, economical & Vocational rehabilitation of the disabled persons. Details are available under chapter of Prosthetics & Orthotics.

9.4 Other Important Activities: I. Republic Day: Republic day is being celebrated every year in our Center’s Premises. II. 05th International Yoga Day: Observed by all staff, students and beneficiaries on dated 21st June 2019. III. 03rd Foundation Day: The Centre celebrated “03rd Foundation day” on dated: 25th June 2019 by distributing Prosthetic & Orthotic appliances to Divyangjan, Launching of Website of CRC, Rajnandgaon & Distribution of Skill Training Certificate to the divyangjan in the trade of 1) Automotive Service Technician (Two & Three Wheeler), 2) Hand Embroiderer through Mayer of Rajnandgaon IV. Swachha Bharat: The center celebrated “Swachha Bharat Pakhwada form dated 19th July to 31st July, 2019 in the centre’s premises. During the programme, Swachha Bharat Pakhwada was celebrated by the Staff & Student of the centre by organizing various cleanliness programmes to spread strong message of clean India. V. Independence Day Independence day is being celebrated every year in our Center’s Premises. VI. Viswakarma Puja: The Centre celebrated “Viswakarma Puja” by the staff and students.

VII. International Disability Day for the PwDs: CRC, Rajnandgaon celebrated the International Disability Day for the PwDs on 3rd December 2019 in collaboration with Social Welfare Deptt, Rajnandgaon by distributing Aids & appliances to Divyangjan at the centre.

Department of Special Education

The essence of Human Resource Development is education, which plays a significant and remedial role in balancing the socio-economic framework and the Constitution of India also ensures equality, freedom, justice and dignity of all individuals and implicitly mandates an inclusive society for all including the children with disabilities.

One of the primary needs of children with special need (CWSN) is that of conducting ongoing assessment. Educational assessment basically involves finding out current functioning of the child or youth in terms of development, language learning skills, Activities of Daily Living Skills (ADL), school subject knowledge and ruling out associated problems. Such assessment is undertaken by the unit of Special education by using formal and informal tests, adapted tools and self devised tools.

The outcome of the assessment is used for the following purposes: 1. Parent guidance 2. Referral 3. School placement 4. Identifying individualistic educational needs of the child 5. Providing individualized educational support.

Empowering parents by providing information in schemes, concessions facilities and exemptions under PWD Act & National Trust Act Catch them Young' is the crux of success in the education and overall rehabilitation of the children with special needs. It is well accepted that language input given during the critical period of language development helps the child in its proper and speedy acquisition of language. Language forms the base on which the educational base is built upon. ORIENTATION & MOBILITY

Orientation and mobility (O&M) training designed to promote safe, independent movement of persons with blindness or visual impairment. Orientation training is designed to help such individuals determine their location in space and how to plan travel from one point to another. Mobility training involves teaching visually impaired persons to move safely from one place to another, which often requires the use of a mobility device, such as a long cane. Using a combination of orientation and mobility techniques, persons who are blind or visually impaired can learn to move around their homes, communities and the world at large with greater independence. There are a variety of skills that help persons with visual impairments to achieve greater independence. Orientation and Mobility Specialist provide instruction or recommendations to maximize independent movement, services include:

Department of Clinical Psychology

 Clinical psychology is a profession where applies principles and procedures to understand, predict and alleviate intellectual, emotional, psychological and behavioral problems of the individuals.  The Department of Clinical Psychology is involved in clinical services by carrying out individualized, detailed diagnostic and intervention based psycho-educational assessments of children at risk, and those with various types of communication disorders and developmental disabilities including mental retardation, autism, speech delays, learning disabilities, and multiple disabilities.

 Individual and group counseling services, vocational guidance and counseling for adult living are given regularly for needy individuals with disabilities or their family members.  Providing IQ assessment reports for Disability Certification to help them for securing various benefit or concessions available by the Central/State governments for persons with disabilities.  Parent and teacher training programs are regularly conducted for target caregivers of children with special needs.  Highlights of the departmental activities are conducting CRE programs to update professional knowledge and skills of master trainers working in the field of disability rehabilitation and special education.  Conducting Post Graduate Diploma in Rehabilitation Psychology (PGDRP) course (in the process) which aimed to prepare rehabilitation counsellors who promote understanding of the situations and needs of people with disabilities and perform a vital role in the personal, vocational and educational adjustment of persons with disability within self- advocacy and community development model.

Department of Speech & Hearing

Speech and Hearing department is one of the two major branches in Speech and Hearing. The department is routinely involved in evaluation, diagnosis, counseling and management of persons with various types of speech ,language and communication disorders and evaluation and diagnosis of hearing loss, prevention of hearing loss, hearing aids trial and fitting auditory training program , counseling of patients to care of hearing aids and its maintained and rehabilitation of the individuals with hearing impairment and evaluation of speech, language and communication disorders include detailed assessment of clients with speech problems ,language problems (such as delayed language development, Mental Retardation, Cerebral Palsy, Specific Language Impairments and Aphasia) and related disorders (such as swallowing and oral motor difficulties). A battery of appraisal procedures starting from case history to assessment through special tests .Counseling, guidance, home training program, services are also provided to clients and their families in their local language according to the locality on various issues related to evaluation and management of persons with speech and language disorders on a regular basis.

Department of Prosthetic & Orthotics

Concept of Prosthetic & Orthotic Department:

The Department of Prosthetics and Orthotics provides Rehabilitation Service to the persons with locomotors disabilities caused by amputation, Poliomyelitis, Cerebral palsy, Congenital skeletal limb deficiencies , spinal cord injury & peripheral nerve injury etc those who unable to stand and walk properly require Prosthetic and Orthotic aids and appliances.

The rehabilitation assistive aids / appliances were planned, designed, fabricated and modified and fitted to disabled taking into consideration of individual's requirement at institute and outreach camps, Research, teaching / training and outreach service programs were part of aid services. The type of aids and appliances provided are: -  Lower and Upper extremity Orthosis for supporting weak or deformed limb to prevent or correct deformity.  Spinal orthosis to support or immobilize the spine to relieve the pain, to prevent deformity.  Lower and Upper limb prosthesis (artificial limbs) to restore function and cosmetic appearance, provide ambulation.  Prefabricated aids/ appliances (Crutches, Walkers, Wheel chairs, Tricycles etc.) for mobility.

Fitment of rehabilitation of aids & appliances-

Physical disabilities restrict the individual to perform activities as a productive member of the Society. The factor influence the productivity of a Person with Disabilities due to nature of disabilities which constraint a person’s capacity for self directed purposeful movement. Walking is the most common form of Mobility for Human beings and the mode is walking most desired by the people for movement. The Prosthetic and Orthotic Devices and other rehabilitation aids assist the Person with Disabilities to achieve a level of performance that at best approaches that of the able bodied person. The department has well equipped Prosthetics and Orthotics Workshop, Prosthetic and Orthotic professionals who design, fabricate and fit suitable and appropriate prosthetics and orthotics devices for Person with Disabilities.

Prosthetic Devices:

Prosthetic Devices (Artificial Limbs) are functional and cosmetic replacement of the missing part of the amputated limb.

Lower Limb Prosthesis:

The amputee who lost part of his Lower limb can use artificial limb for walking. The department of Prosthetics and Orthotics fabricate and fit Partial Foot Prosthesis, Symes Prosthesis, Bellow Knee Prosthesis, Through Knee Prosthesis, Above Knee Prosthesis, Hip Disarticulation Prosthesis and Extension Prosthesis and make fitments to Person with Disabilities of various level of amputation.

Upper Limb Prosthesis:

The Upper Limb Prosthesis are designed and fabricated for Upper Limb Amputees to restore grasp function and cosmetic appearance. The various types of Upper Limb Prosthesis designed and fabricated by the department are Partial Hand Prosthesis, Carpometacarpal Prosthesis, Wrist Disarticulation Prosthesis, Below elbow Prosthesis, Elbow Disarticulation Prosthesis, Above Elbow Prosthesis and Shoulder Disarticulation Prosthesis.

Orthotic Devices:

The Orthotic Devices are generally used for management of various Orthopaedic and Neuromuscular disorders including congenital deformity of lower limb, upper limb and spine. The devices are used for wide spectrum of physical disabilities. They are used to correct the deformity, to maintain the correction and support the unstable part of the body. The various types of Lower Limb Orthosis are Foot Orthosis, Ankle Foot Orthosis, Knee Orthosis, Knee Ankle Foot Orthosis, Hip Orthosis, and Hip Knee Ankle Foot Orthosis etc. The types of Upper Limb Orthosis are Wrist Hand Orthosis, Elbow Orthosis and Elbow Wrist Hand Orthosis etc. The Spinal Orthosis like Sacroiliac Belt, Lumbo-Sacral Belt, Lumbo-Sacral Corset and Thoraco- lumbosacral Orthosis (PP) are also provided.

Department of Physiotherapy

Physiotherapy is a branch of modern treatment that deals with the disease and disorders by physical means such as Exercise, Mobilization, Manipulation, Traction, Electricity, Magnetism, Heat, Light, Water, Ice, Ultrasound etc. It is not merely application of physical modalities but it includes total client care i.e. evaluation, assessment, treatment, planning, application and advise. It deals with Musculoskeletal, Neuromuscular, Cardiopulmonary, Paediatrics, Geriatrics and all surgical conditions. Department of Occupational Therapy

Occupational therapy is the only profession that helps people across the lifespan to do the things they want and need to do through the therapeutic use of daily activities (occupations). Occupational therapy practitioners enable people of all ages to live life to its fullest by helping them promote health, and prevent—or live better with—injury, illness, or disability.

Common occupational therapy interventions include helping children with disabilities to participate fully in school and social situations, helping people recovering from injury to regain skills, and providing supports for older adults experiencing physical and cognitive changes.

Occupational therapy services typically includes:

Department of Vocational

Introduction:

Vocational training plays a crucial role in the social and economic development of every country. Vocational training involves teaching people to acquire a particular skill meant to prepare them for a particular occupation.

Employment is an important ingredient for combating poverty and achieving inclusion for all persons, including persons with disabilities (PWD). Employment increases self-sufficiency, social connectedness and creates a sense of self worth and dignity for individuals.

For persons with disabilities, employment is considered a vital step towards their rehabilitation and empowerment in that it provides a sense of belonging, independence, and increases their social inclusion and overall well-being.

Although persons with disabilities can perform virtually every job, and with the enabling environment and support services, majority of persons with disabilities can be as productive as persons without disabilities, there are limited employment opportunities for them.

Since 1989 Social & Vocational Rehabilitation section started functioning under the Department of Rehabilitation Medicine & Surgery for providing Socio-Vocational Rehabilitation services to Persons with Disabilities and Teaching activities to degree students of PT, OT & PO.

Objective:

 Act as a centre for developing vocational training module for Divyangjans.  Aims at inducing self-confidence, self care activities and creating awareness about their residual abilities, worthiness & usefulness.  Provide training opportunities to develop skills & knowledge so as to enhance the scope of employability.  Encourage for self-employment/ wage employment.  Guide about placement services.  Provide supportive services such as counselling & guidance to develop desirable personality.  Create awareness and sense of responsibility in the employers & community for rehabilitation of Divyangjans.  Providing vocational training for disabled male and female youth, providing assistive devices for them trade tools for self employment and referring them for open employments.  Referring children with special needs for inclusive education.  Providing sign language interpreter services to facilitate the routine work of the persons with hearing impairment.  Referring various requests including assistance for necessary actions.

Upcoming Trainings:-

Composite Regional Centre (CRC), Rajnandgaon is going to commence Skill Development Training Programme in three Trades i.e. 1. Mobile Repairing, 2. Assistant Beauty Therapist & 3. Hand Rolled Agarbatti Maker with intake capacity of 30 beneficiaries in each Trade under SIPDA Scheme in collaboration with Leading Reforms Consultant Pvt. Ltd, Raipur one of the MSJ&E empanelled Organization and authorized to implement Skill Development Training Programme for Persons with Disabilities in Chhattisgarh.

Placement & Employment Services:

After Successful completion of Skill Training Programme in 2 Trades such as 1) Automotive Service Technician (Two & Three Wheelers) & 2) Hand Embroiderer; 20 nos. of Trainees have been placed in self employment to earn their livelihood during 2019-20.

Service & Activities:

1. Socio- Economic assessment of persons with Locomotor Disability and family. 2. Arrangements/ Linkage for social, educational, vocational, recreational services including training, self employment, placement, promotion of SHGs of PWDs etc. 3. Public awareness, education, training & orientation at various operational levels, conferences & workshops etc. 4. Conduct/ participate training & teaching programmes for professionals as well as Persons with Disabilities and their families. 5. Productions of awareness kits, materials, posters, leaflets, booklets. 6. Liaison with NGOs/GOs for implementation of programmes/schemes for the total rehabilitation of the disabled persons i.e. for Education, Vocational Training, Self employment and Placement, Referral & Extension services. 7. Planning, preparing project Proposal, MOU, and developing strategies for Skill Development Training. 8. Monitoring & Inspection of Skill Training Programs conducted by NGOs. 9. Department of Administration Introduction

The Department maintains discipline in the work place for achieving the organizational objectives. The department ensures the day to day functioning of the Institute by coordinating, monitoring and controlling the systems and procedures. The department envisages strict adherence to rules and regulations in all the activities of the Institute. The main function of the department has been divided through various sections like: 1. The CRC, Rajnandgaon continued to follow the reservation policy framed by Govt. of India.

As on 31st March 2020; CRC, Rajnandgaon had total of 16 (Out of 19) Sanctioned contractual posts.

Introduction

The Institute is fully funded by the Department of Disability Affairs, Ministry of Social Justice and Empowerment, Govt. of India. Accounts Department of the Institute keeps the records of the financial Transactions of the Institute. Pay Roll and day to day Accounts are maintained through software. The Department keeps the records of Grant in Aid received from the Ministry of Social Justice and Empowerment, Government of India and the utilization of the same. This Department discharges a pivotal role in monitoring the financial transactions as per the laid down procedure of GFR and other guidelines issued by the Government of India. Compilation of Annual Accounts within the time frame and get the Accounts Audited by Comptroller & Auditor General of India is also an important function of the Department.

2. Post Graduate Diploma In Rehabilitation Psychology (PGDRP)

Out Reach Services / Activities:- To provide the rehabilitation services to the Persons with Disabilities in rural, tribal, remote and interior areas, the Institute has been conducting rehabilitation camps in collaboration with District Administrative and NGOs to provide aids and appliances in different district of Chhattisgarh.

i. Assessment Camp:

In the first phase assessment Camps are conducted to asses and evaluate the Person With Disabilities and their needs for various types of Aids and Appliances. This will facilitate to derive the exact number of aids and appliances required for particular camp. After procurement of the required number of aids appliances they were distributed to the Person With Disabilities under Rehabilitation camps.

ii. Assessment cum Distribution camp:

Assessment cum Distribution camps are conducted to asses and evaluate the Person With Disabilities and at the same time aids and appliances were provided to the Person With Disabilities at the camp site. If the number of PWD exceeds the numbers of aids and appliances, the balance aids and appliances were provided in the second phase under Rehabilitation camp.

iii. Rehabilitation Camp:

Rehabilitation camps are organized in a planned manner and the aids and appliances were distributed at the Camp based on the list of the Person with Disabilities identified in the earlier assessment camp.
