Title of Elective: Family Sub-Internship

General Description of course: During this four week rotation the student will spend two weeks as an active team member of the Dublin Methodist inpatient service and two weeks working alongside Family Medicine in the OhioHealth Primary Care Physicians office in Dublin. Students will participate in both general medicine inpatient care and ambulatory family medicine in preparation for their PGY-1 year as a Family Medicine resident.

Length: 4 weeks

Hours: Ambulatory Family Medicine: 8a-5p M-F Inpatient Internal Medicine: 7a-5p M-F. You may work weekend days during this portion of the rotation. Work with your team to determine the best days for this experience.

Instruction Type/Format: 2 weeks of inpatient ward experience (one-on-one or small group instruction), 2 weeks of ambulatory experience (one-on-one instruction).

Responsibilities: • Work up and follow inpatients. • Care for panel of outpatients. • Patient presentations. • Reading of literature and research.

Learning objectives: Inpatient Internal Medicine • Participate on an internal medicine team with an advanced degree of independence and responsibility in preparation for the PGY-1 year. • Perform an initial assessment of patients under consideration for admission to the internal medicine service. • Complete a full history and physical exam. • Develop a problem list and differential diagnosis. • Write admission orders. • Follow daily progress, daily notes. • Review literature on patient problems. • Implement diagnostic and therapeutic plans taking into account evidence-based information and patient preferences. • Work and communicate effectively with subspecialty consults and other members of the care team. • Address issues of risk vs. benefit, prevention, nutrition, ethics, pharmacology and interface between inpatient/outpatient care. • Recognize the value of continuity of care as patients make a transition into and out of the hospital. • Consider positively a career toward primary care and, particularly, family medicine.

Ambulatory Family Medicine • Collaborate with a family medicine attending in the care of patients with an advanced degree of independence and responsibility in preparation for their PGY-1 year. • Demonstrate accurate and complete history taking skills. • Demonstrate accurate and complete physical examination skills. • Demonstrate accurate and complete differential diagnostic skills. • Demonstrate completion of a complete and accurate office visit documentation. • Demonstrate appropriate diagnostic and therapeutic plan development. • Demonstrate advanced interpersonal communications skills with patients. • Demonstrate skills to develop a complete preventive health plan for patients. • Demonstrate skills to complete a focused primary care patient visit. • Consider positively a career toward primary care and, particularly, family medicine.

Assessments to be used: Teaching feedback: Formative verbal feedback by attending physicians at least once per week. Final Grade: Composite written evaluation from attending physicians.

Teaching and Learning opportunities: Didactics and resources: Hands on teaching occurring during the structured care of patients. Participation in Dublin Methodist Family Medicine didactics, Tuesday and Thursday 12:15-1:00p and Wednesday 12:30-4:30p.

Clinical assignments/ clinical experiences: Detailed information will be sent regarding your schedule and where to report two weeks prior to the start of the rotation.

If this is your first OhioHealth rotation and you have not had CareConnect Training (Epic EMR) or completed the OhioHealth Learning Modules; all times, processes and locations to complete this training will be sent to you from the Department of Medical Education.

Students who have completed CareConnect training and the OhioHealth Learning Modules may be assigned annual OhioHealth Learning Modules for completion.

You will work with a variety of physicians including Dublin Methodist Family Medicine residents on this rotation. Family Medicine residents and administrative support staff at each location will work with your schedule and physician availability to create a schedule for you. Prerequisites and requirements for enrollment: Successful completion of Part 1. Rotation is by permission only and will be approved only if completed application forms are submitted a minimum of 4 weeks prior to rotation start date. Student must be applying to the Dublin Methodist Family Medicine Residency Program and submit USMLE 1 or COMLEX 1 and USMLE 2CK scores and COMLEX 2CE scores (if available), a current CV, and proof of active ACLS certification.

Student capacity: 1-2 students during each four week block, July through January.
