Level B Mapping delineates a Stamford Aquifer Protection Areas preliminary aquifer protection area, providing an estimate of the Darien land area from which the well draws its water. Level A Mapping Greenwich Bureau of Water Protection and Land Reuse delineates the final Aquifer Protection Area, which becomes the regulatory boundary for land use controls designed to protect the well from contamination. As Level A Mapping is completed for July 21, 2014 each well field and approved by DEEP, it will replace the Level B Mapping. 1.0 Project Description and Location

The State of Connecticut Action Plan for Community Development Block Grant Program Disaster Recovery submitted a Plan to the U.S. Department of Housing and Urban Development (“HUD”) as part of a receipt of $71,820,000 of federal funding under the Community Development Block Grant – Disaster Recovery (CDBG-DR) Program.

The funding was authorized under The Disaster Relief Appropriations Act of January 29, 2013. The allocation of the Funding to the State is intended to address immediate unmet housing and economic revitalization needs in those counties and jurisdictions that were most severely impacted by .

Pursuant to the National Environmental Policy Act (NEPA), Freeman Companies, LLC has prepared the following environmental documentation for rehabilitation of the property located at 42 Neptune Avenue in Norwalk, Connecticut. We are preparing the environmental documentation in accordance with the HUD regulations 24 CFR Part 58. This project is within Connecticut Department of Housing’s (DOH) Owner Occupied Rehabilitation and Rebuilding (OORR) Program.

The property is a single family residence located at approximately 41.083458, Latitude and -73.409748 Longitude. The residence, constructed in 1923, is located on the south side of Neptune Avenue between Channel Avenue and Park Lane. The property is located within an AE flood plain.

The project will entail the following: x Raising of the residence 2 feet above base flood elevation (BFE)

2.0 Explanation of Categorical Exclusion

A Categorical Exclusion in accordance with 24 CFR Part 58.35 refers to a category of activities for which no environmental impact statement or environmental assessment and finding of no significant impact under NEPA is required, except in extraordinary circumstances. Because the project involves building rehabilitation that does not increase the unit density of the building or change land use, the project is categorically excluded under 24 CRF 58.35(a)(3)(i).

3.0 Statutory Checklist

This project is determined to be Categorically Excluded according to 24 CFR 58.35(a)(3)(i). Projects may be additionally subject to review under related federal laws and authorities as determined by completing a statutory checklist. The following checklist and documentation of the findings of the checklist are incorporated into this Environmental Review Record in compliance with 24 CFR 58.

Tier 1 of a 2-step Tiered Environmental Review has already been conducted by DOH, and this Statutory Checklist shall be considered Tier 2.

The Statutory Checklist indicates whether the activity does or does not affect the resources under consideration. Status “A” indicates that the project does not require formal consultation with an outside agency and does not affect the resource in question. Status “B” indicates that the activity requires formal compliance consultation with the oversight agency or affects the resource. The documents and/or information sources used in making the determination are listed in the checklist. A compliance determination is provided following the checklist.

The checklist is included as Appendix A.

4.0 Agency Consultation and Mitigation Measures Required

Based on the completion of the checklist, the following agencies were consulted and inspections performed.

4.1 Department of Economic and Community Development - State Historic Preservation Office

The State Historic Preservation Office (SHPO) is responsible for overseeing the governmental program of historic preservation for Connecticut’s citizens. SHPO administers a range of federal and state programs that identify, register and protect the buildings, sites, structures, districts and objects that comprise Connecticut's cultural heritage.

In accordance with 24 CFR 58.5(a) Historic Properties, since the property, due to its age, may be eligible for listing on the National Register of Historical Places, a request for a review of the status of the site relative to historic or cultural resources will be submitted directly by the Connecticut Department of Housing.

4.2 City of Norwalk– Planning and Zoning Department

The City of Norwalk was consulted in regards to inland , coastal zone management and local zoning approvals. According to the compliance assistant at the department of Planning and Zoning, Mr. Jim Bova, a coastal area management review would be required for the project if the renovation exceeds the footprint of the existing building. In addition approvals from inland wetlands as well as local planning and zoning may also be required.

4.3 Lead

A lead inspection was performed at the property Freeman Companies on November 11, 2016. Based on the inspection several building components were determined to contain concentrations of lead greater than 1.0 milligrams of lead per square centimeter of paint. See Appendix D “Hazardous Building Materials Survey for Proposed Renovations”, for the list of building components.

Rehabilitation/renovation/repair activates that disturb any of these areas will be subject to the requirements of 40 CFR 745.80 through 745.92 (EPA’s Lead Renovation, Repair and Painting Rule). If these components are to be disposed during rehabilitation then a Toxicity Characteristic Leaching Procedure (TCLP) sample of the demolition waste stream should to be collected in order to determine disposal requirements.

4.4 Asbestos

An asbestos inspection was performed at the property by Freeman Companies on November 11, 2016. Based on the results of the inspection, none of the tested materials were identified to contain asbestos.

Any suspect material encountered during renovation that is not identified in this report as being non- asbestos containing material, should be assumed to be asbestos containing material unless sample results prove otherwise.

4.5 Radon

Radon testing was not performed at the site per instructions from Amaya Architects.

4.6 Mold

On November 11, 2016, Freeman Companies performed a visual assessment, and laboratory testing, for the presence of suspect mold and water intrusion. Based on the findings of the assessment and laboratory testing, the following water damaged material was identified:

5.0 Determination

For Categorically Excluded actions pursuant to §58.35(a), the project cannot convert to “Exempt” since one or more authority requires compliance, including but not limited to consultation with or approval from an oversight agency, performance of a study or analysis, completion of remediation or mitigation measure, or obtaining of license or permit.

Mr. Charles D. Brink possesses over 20 years of experience performing and leading environmental assessment and investigation projects. He has overseen numerous hazardous material investigations and performed dozens of Phase I Environmental Site Assessments. National Wetland Inventory Maps are produced by the U.S. Fish and Wildlife Service, a sub-department of the U.S. Department of the Interior. In 1974, the U.S. Fish and Wildlife Service developed a criteria for wetland classification with four long range objectives:

· to describe ecological units that have certain homogeneous natural attributes, · to arrange these units in a system that will aid decisions about resource management, · to furnish units for inventory and mapping, and · to provide uniformity in concepts and terminology throughout the U.S.

High altitude infrared photographs, soil maps, topographic maps and site visits are the methods used to gather data for the productions of these maps. In the infrared photos, wetlands appear as different colors and these wetlands are then classified by type. Using a hierarchical classification, the maps identify wetland and deepwater habitats according to:

· system · subsystem · class · subclass · modifiers

(as defined by Cowardin, et al. U.S. Fish and Wildlife Service FWS/OBS 79/31. 1979.)

The classification system consists of five systems:

1. marine 2. estuarine 3. riverine 4. lacustrine 5. palustrine

The marine system consists of deep water tidal habitats and adjacent tidal wetlands. The riverine system consists of all wetlands contained within a channel. The lacustrine systems includes all nontidal wetlands related to swamps, bogs & marshes. The estuarine system consists of deepwater tidal habitats and where ocean water is diluted by fresh water. The palustrine system includes nontidal wetlands dominated by trees and shrubs and where salinity is below .5% in tidal areas. All of these systems are divided in subsystems and then further divided into class.

National Wetland Inventory Maps are produced by transferring gathered data on a standard 7.5 minute U.S.G.S. topographic map. Approximately 52 square miles are covered on a National Wetland Inventory map at a scale of 1:24,000. Electronic data is compiled by digitizing these National Wetland Inventory Maps.

1.0 INTRODUCTION

Freeman Companies, LLC was retained by Amaya Architects to conduct a pre-renovation asbestos, lead- based paint, and mold growth investigative survey amongst suspect materials associated with proposed renovations to the residence at 42 Neptune Avenue, Norwalk, Connecticut. The asbestos survey was conducted in conformance with the Environmental Protection Agency (EPA) National Emissions Standards for Hazardous Air Pollutants (NESHAP) 40 CFR Part 61.

Freeman Companies, LLC was contracted to perform an investigative survey of all accessible interior and exterior materials. These areas included the living spaces, attic, and accessible roofing areas. Non- destructive sampling techniques were conducted. Every attempt was made to locate all suspect materials, however additional materials may be discovered above ceilings, below floors, or behind walls during demolition or renovation activities.

2.0 ASBESTOS INSPECTION

The following asbestos survey section presents the survey results, methods, and conclusions based on survey findings. Appendix A further illustrates each type of suspect asbestos-containing material identified, their location(s), and whether the materials are classified as an asbestos containing material (ACM) or non- ACM based upon the analytical results. All samples were submitted to EMSL Analytical, Inc., a Connecticut Licensed Laboratory.

As required by the U.S. Occupational Safety & Health Administration (OSHA), the U.S. Environmental Protection Agency (EPA), and the State of Connecticut Department of Public Health (DPH), sampling was performed by a DPH-certified asbestos inspector (see Appendix C). Sampling was done in a manner to prevent airborne fiber release and in accordance with the protocols specified in EPA 40 CFR Part 763. Samples were placed in appropriately labeled containers that were sealed, labeled and submitted to the laboratory for analysis. The samples were submitted for petrographic analysis using the EPA-endorsed Polarized Light Microscopy with Dispersion Staining (PLM/DS) method. The percentage of asbestos present in each sample was determined by the visual area estimation technique.

Samples were collected using a wet technique to prevent airborne fiber release. Each suspect material was sampled using a decontaminated knife to cut through its entire thickness to ensure that a complete cross section was obtained. The sample was then placed in an appropriately labeled container, which was sealed, labeled and submitted to the laboratory for analysis.

3.0 LEAD PAINT SURVEY

The lead paint survey was conducted with an X-ray fluorescent (XRF) direct reading instrument in accordance with the Department of Housing & Urban Development (HUD) testing guidelines. These protocols were developed for residential or day care facilities and were adopted by the Connecticut Childhood Lead Poisoning Prevention Regulations (CLPPR). The lead paint reports were prepared using the CLPPR threshold of 1 mg/cm2.

EPA’s Lead Renovation, Repair and Painting Rule (RRP Rule) requires that firms performing renovation, repair, and painting projects that disturb lead-based paint in homes, child care facilities, and pre-schools built before 1978 have their firm certified by EPA (or an EPA authorized state), use certified renovators who are trained by EPA approved training providers and follow lead safe work practices. These guidelines define the regulated level of lead paint (Toxic Level of Lead) as paint containing greater than 1.0 milligrams lead per square centimeter (mg/cm2) of surface as measured on-site by an XRF analyzer or more than 0.50 percent lead by dry weight as measured by Atomic Absorption Spectrometry (AAS).

Additionally, all paints containing detectable amounts of lead are regulated under OSHA. This action is taken because OSHA regulates lead in construction based on airborne exposures and it cannot be ensured that lead paint with concentrations of lead less than 1.0 mg/cm2 or 0.50% mass will not result in exposures exceeding the OSHA standard.

Results of the lead paint survey indicated several surfaces painted with a lead concentration of 0.00 mg/ cm2, several surfaces painted with a lead concentration ranging between 0.00 mg/ cm2 and 1.0 mg/ cm2 (lead containing paint), and several surfaces painted with lead concentrations exceeding 1.0 mg/ cm2 (lead based paint). Results are listed below. Figure 1 provides identification for rooms, and sides of walls tested.

5.0 MOLD GROWTH

Freeman Companies LLC observed localized mild mold growth residue on the ceiling of the second floor. The visually impacted surfaces were swab sampled and analyzed via direct microscopic examination in order to determine mold type and relative concentration of the mold. The samples were labeled and delivered with a Chain of Custody to EMSL Laboratory in Wallingford, Connecticut. The samples were analyzed by direct examination optical microscopy for mold species identification and quantification. EMSL is an AIHA approved Laboratory.

Laboratory testing confirmed the presence of Cladosporium mold in high concentrations. Hyphal fragments were also present indicating active mold colonies. Cladosporium is very common on wet building materials such as gypsum board, acrylic painted walls, wood, wallpaper, and carpeting. Cladosporium species have the ability to trigger allergic reactions to sensitive individuals. Prolonged exposure to elevated spore concentrations can elicit chronic allergies and asthma.

Information contained in this report is based on site observations, sample results relevant to the scope of work for this survey. Conclusions of this report are based on the survey, study, and/or investigation. This is not to be interpreted as a complete compilation of all existing information pertaining to the site conditions.

It should be noted that mold / fungal conditions observed during this investigation may change based on any number of influencing factors and/or environmental variables such as fluctuations in indoor and outdoor temperatures, humidity and seasonal changes in sunlight. These factors can influence the spread and concentration of molds as they change. This report is not intended to guarantee that the investigated site is, or is not, free from conditions, which could pose a threat or hazard to human health or safety. Should further research on the site be conducted, any additional data should be submitted to Freeman Companies for review and revisions as necessary.

7.0 CONCLUSIONS and RECOMMENDATIONS

In accordance with the OSHA regulations (29 CFR Part 1926.1101 and 1910.1001), all potential contractors bidding on work must first be informed of the results of this survey.

The non-destructive asbestos inspection conducted did not locate any materials identified as ACM. Any new materials that may be encountered during renovation must be presumed as ACM until testing proves otherwise. All materials identified as negative for asbestos may be removed at will and disposed of as standard construction debris. In accordance with EPA NESHAPS regulations (40 CFR Part 61), building owners must remove all ACM from a facility (or area of a facility) before any activity begins that would break up, dislodge, or similarly disturb the ACM. In the State of Connecticut, a licensed asbestos abatement contractor must perform all asbestos-related activities, including the renovation/demolition portion of the work that includes asbestos disturbance. Disturbance of asbestos-containing material can only be done by trained and licensed individuals. It should be noted that the State of Connecticut regulations governing asbestos abatement does not distinguish between friable and non-friable material. Therefore, full containment procedures are required for any interior abatement work (removal, encapsulation, or enclosure) involving both Category I and/or Category II non-friable asbestos-containing materials.

The lead paint survey identified several surfaces painted with either lead based paint (>1.0 mg/cm2) or lead containing paint (>0 and <1.0 mg/cm2). EPA’s RRP Rule requires that firms performing renovation, repair, and painting projects that disturb lead-based paint in homes, child care facilities, and pre- schools built before 1978 have their firm certified by EPA (or an EPA authorized state), use certified renovators who are trained by EPA approved training providers and follow lead safe work practices. Additionally, contractors will be required to comply with all applicable OSHA regulations including 29 CFR 1926.62, "Lead Exposure in Construction: Interim Final Rule" and 29 CFR 1926.59, "Hazard Communication for the Construction Industry". In addition, pre-disposal lead-waste testing requirements must be complied with.

Freeman Companies LLC recommends contracting a qualified mold remediation contractor to clean mold affected surfaces prior to renovations.
