I understand that during the college tour, there will be some free time for students to explore a campus before or after the tour. Schedule permitting, there may also be time for a visit to the beach and/or a college town or other location where students would have time to walk around, have a meal, and see the sights. Students will be given boundaries and must utilize the “buddy system” (at least three people from the tour together at all times). ______My child has permission to leave the larger group to walk around with friends from the tour, as explained above, during free time that may occur during the trip. I understand that my student must follow the Behavior Expectations listed below, and I have discussed these expectations with my student. ______All of the above is true AND my student has permission to meet with a friend/relative who is NOT a part of the tour. The names of these friends/relatives and their contact information (phone, address, visit times, etc.) are listed on a separate paper turned in to Seize the Day College Tours or the trip chaperones before departure. Behavior Expectations: 1) During free time, students must follow the “buddy system”. Students are not to walk around alone. 2) Campus tours are not optional. They are the focus of the trip. Students are expected to be courteous to the tour guides while on tours. 3) Students may not enter any vehicle or form of transportation other than our charter bus during the trip. 4) Members of the opposite gender are not allowed in each other’s rooms at any time. 5) Students will have a reasonable curfew each night, which will be determined by the Seize the Day College Tours trip leaders. Students MUST remain in their rooms after curfew and must remain reasonably quiet. 6) Students must stay within given boundaries during free time and behave in a mature, respectable manner. 7) Possession of alcohol or illegal drugs is strictly prohibited. Any student found to be under the influence of or in possession of any type of alcohol or illegal drug will be sent home at the expense of the student/parents. No refunds given. 8) Students need to be respectful towards one another, leave the hotel rooms and charter bus neat, and have a positive attitude. *Remember, you may, in your own life, have more personal freedoms than you will on this trip. These expectations are based on the common good of the group. Please respect them so our tour will run smoothly and we will continue to be welcomed at these campuses and hotels on future trips. I also agree to the above checked option regarding supervision for my student. ______Terms and Conditions I, ______, have registered for a college trip organized by Seize the Day College Tours (SDCT). I agree to all of the following terms, conditions, and understandings, which shall be binding upon me and upon SDCT. I agree to abide by the rules and regulations set by SDCT for the health, safety, welfare and management of tour participants. I agree to indemnify and hold harmless SDCT from any claims arising out of my participation in the tour program. 2. SDCT is engaged in the business of arranging and conducting escorted tours of college campuses. SDCT performs a wide variety of services in connection with these tours. SDCT as the principal shall be responsible for making arrangements for ground and/or air services, accommodations, appointments, events, activities, SDCT college planning materials, etc.. SDCT cannot be responsible, however, for such occurrences as an airline's failure to honor a confirmed reservation or to provide the transportation it has contracted to perform. I release SDCT (its employees, agents, school, etc) from any claims arising from the failure of any airline, ground transportation company, hotel, , or other supplier of tour services, arrangements, or accommodations, to perform as set forth in any tour literature, except where there has been actual negligence on the part of SDCT (or its employees or agents). In the event of missed connections, mechanical or weather related delays, whether due to the student’s own tardiness or the non-performance of an airline or other provider of tour services, I will be responsible for any additional expenses for lodging, meals, and transportation. 3. I acknowledge that SDCT cannot be responsible to me for events beyond its control, such as (without limitation) strikes, war, delays, weather, acts of God, terrorism, or governmental restrictions; or for the actions or omissions of any persons or entities outside its control, such as (without limitation) airlines, surface transportation companies, hotels, , and other suppliers of tour services, arrangements, or accommodations. I release SDCT (its employees, agents, school) from any claims arising from such events, actions, or omissions. Because SDCT has no control over such events and circumstances, it cannot be responsible for any claims which may arise as a result. If I wish to obtain , baggage protection insurance, or other insurance coverage, it is my responsibility to do so. 4. Each tour group is led and supervised by an experienced consultant and may be assisted by one or more persons experienced in counseling high school students regarding college selection and admissions. SDCT reserves the right to make any alterations in the itinerary, hotel accommodations, dates, transportation, food arrangements, sightseeing, tour leadership, and makeup of tour membership as it, in its sole discretion, may deem necessary for the success of the trip. If such events or circumstances arise, I agree to abide by SDCT judgments, and decision. I release SDCT (and its employees and agents) from any claims arising from the cancellation of my visit to any city or college campus. All schedules and fares quoted are based on current costs of operation, and are subject to confirmation or adjustment before the tour departs. Additional expenses, if any, shall be borne by the tours' member's parents or guardian: conversely, refunds, if any, will be made to the tour member's parents or guardian. 5. I agree to abide by the final itinerary established by SDCT for my tour program. I will not depart from SDCT's established itinerary at any time, either on my own or with any other tour participants, unless I submit a written document signed by my parents or legal guardian to SDCT and such document is approved by SDCT prior to departure. If I submit a written document and it is approved by SDCT, I agree to honor its terms and conditions and to follow the travel plans which it specifies. The approved document shall also become a part of this agreement, whether or not it is physically attached. If I depart from SDCT's established itinerary at any time during my tour (whether or not I have first submitted a written document authorizing the departure), I agree to be fully responsible for my own safety and well-being, as well as for my own conduct. I will make no claims against SDCT (or its employees or agents) as a result of any events which may occur prior to the actual departure of my tour or during any time that I depart from SDCT's established itinerary for my tour. SDCT cannot be responsible for my well-being during the time I am traveling from my home to the point of departure for my tour, or at any time during my tour when I am absent from SDCT-supervised activities. 6. I agree to conduct myself in a responsible manner at all times during my college tour program, follow the established itinerary, and to refrain from the use of alcohol or illegal drugs. In addition, I will comply with all group rules and standards which are announced verbally or in writing by SDCT. SDCT shall have complete discretion to terminate my participation in the college tour program and to send me home at my own expense (or at the expense of my parents or legal guardian) if I engage in any conduct which violates this Agreement, violates the group rules and standards announced by SDCT, endanger the success of the tour or the welfare of other tour members and/or tour leaders, or is detrimental to the reputation of SDCT. If I am sent home for any of these reasons, I will not be entitled to any refund. The manner and means of transporting the member home shall be determined exclusively by SDCT and all expenses incidental to the termination of the tour membership (including the entire costs of the transportation of the tour member back home) shall be borne completely by the tour member and/or the tour member's parent or guardian. 7. I agree to accept responsibility for my own conduct and judgment at all times during my college tour program, regardless of whether I am participating in a SDCT-supervised activity. SDCT is not responsible for loss of travel documents, valuables, or for loss of or damage to luggage or belongings. I accept responsibility for any financial obligations I may incur and for any damages or injuries I may cause. 8. I agree to complete the Medical Information Form as provided by SDCT and have indicated on this form any medical conditions which might affect my participation in the college tour program. Tour members must be covered by medical and baggage insurance by their parents or guardians as SDCT cannot assume any medical costs or baggage losses incurred while on tour. Trip cancellation insurance is available to protect against sudden illness of you or immediate family resulting in a doctor's orders against taking the trip. Further information is available upon request. I will notify SDCT of any prescription drug use on the Medical Information Form which will be signed by my parent or legal guardian. I understand my failure to complete the form prior to the college tour departure will preclude me from participating in the program. I acknowledge that the information contained in the Medical Information Form and Student Registration Form is true and accurate to the best of my knowledge and that I will indemnify and hold harmless SDCT from any claims should any of the information contained in the Medical Information Form and Student Registration Form are false. If I become ill or incapacitated at any time, I authorize SDCT to take whatever action it deems necessary to preserve my health and safety, including (without limitation) obtaining medical treatment for me and/or transporting me home, all at my expense (or at the expense of my parents or legal guardian). I agree to reimburse SDCT for any expenses it incurs for the purpose of preserving my health or safety. 9. I agree that SDCT shall have the right to cancel any tour program for insufficient participation or for other reasons which it considers appropriate. In the event a tour is cancelled, SDCT's sole obligation will be to provide me with a full refund. Should the tour be cancelled after it has begun, SDCT's sole obligation shall be to provide me with a refund of the cancelled portion of the tour program. I agree that I will have no right to a refund for any accommodations, meals, or services which I do not use once my tour has begun. 10. SDCT's fees for the college tour include: air and/or ground transportation, hotel lodging, some meals, student led campus tours, meetings with admission representatives at some campuses, materials, and listed activities. If I wish to cancel my reservation for a SDCT tour, I must do so in writing prior to departure. My right to a refund will depend upon the date that my written notice of cancellation is received by SDCT. If my written notice is received at least thirty (30) days prior to departure, then SDCT will refund all of my money, minus a processing fee of $50.00 plus any direct expenses incurred, e.g. non-refundable plane tickets, airline deposit, university housing deposits. If my written notice is received between 29 and 14 days prior to departure, then SDCT will refund all of my money minus $200 plus any expenses incurred. Should I cancel less than 14 days prior to the tour departure date I will not be entitled to any refund. 11. This agreement shall be effective upon my acceptance by SDCT as a participant in one of its campus tour programs. This agreement may not be modified except in writing. No oral modifications of this Agreement shall be valid. 12. SDCT program is designed to provide an educational and enjoyable experience for students to compare a variety of schools. SDCT is not affiliated with any college or university, and has no involvement in or influence upon any college’s admission decisions. SDCT may use photographs, videotapes, and testimonials of students in publicity materials. 13. I certify that I am the parent or legal guardian of the applicant named above. I have read the above Agreement and will be bound by its terms and conditions, both on my own behalf and on behalf of the applicant.
