Your health care will be from doctors in our network. If your primary care physician (PCP) says you need services outside of our network, s/he can call us. Provider incentive plan You are entitled to ask if we have special ®nancial arrangements with our providers that can affect the use of referrals and other services you might need. Primary Care Physicians Family Medicine: Adults and children Pediatrics: Children Internal Medicine: Adults General Practice: Adults and children OB/GYN: (who speci®cally request Adult women to act as a PCP) Physician©s Assistants: Adults and children Nurse Practitioners: Adults and children Geriatrics: Adults Clinics: (to include IHS, FQHC, RHC) Adults and children To ®nd a provider, go to the Table of Contents. The providers are listed by their specialty. (Specialty is the area of medicine in which this doctor is highly trained. The doctor trains in this area of medicine after medical school.) When you ®nd the specialty you want, go to the page listed. Then you can pick the provider who meets your needs. Members may get care from a resident doctor or a nurse practitioner if supervised by a PCP. Remember to take your member ID card with you. If there is no chapter in the Table of Contents for the type of provider you want, look in the chapter for Ancillary Providers. Providers that do not have speci®c chapters are listed there. These listings are based on the provider©s application. We verify certi®cation and hospital affiliations when a provider joins our network. We check every three years (or sooner). We verify names, phone numbers, addresses, languages, limits and gender when a provider joins our network or gives us new information. myuhc.com/CommunityPlan CSKY.20.999.26.9.009 Cultural competency and accessibility indicators Description: Example of Display: Wheelchair access Transportation P = Parking spaces are accessible with ramps and curb cutouts between the Parking parking lot, office and at drop off locations. EB = Handrails are provided on both sides of the ramp. Doorways are accessible, Exterior open wide enough to accommodate a wheelchair or scooter and have door Building handles that are easy to use. IB = The interior doors open wide enough to accommodate a wheelchair or scooter Interior Building and door handles are easy to use. Elevators, stairs, and ramps have handrails and are easy to use. The elevator is available for public/patient use at all times the building is occupied. The elevator has enough room for a wheelchair or scooter to maneuver. 1 1 = Home infusion pharmacy 2 2 = Retail pharmacy 3 3 = 90 day supply 4 4 = LTC pharmacy 24-hour pharmacy R = The restroom is accessible and the doors are wide enough to accommodate a Restroom wheelchair or scooter and are easy to open. The restroom has enough room for a wheelchair or scooter to turn around and close the door. There are grab bars which allow easy transfer from wheelchair to toilet. The sink is easy to get to and the faucets, soap, and toilet paper are easy to reach and use. E = The entrance to the exam room is accessible, with a clear path. The doors open Exam Room wide enough to accommodate a wheelchair or scooter and are easy to open. The exam room has enough room for a wheelchair or scooter to turn around. T = The exam table moves up and down and the scale is accessible with handrails Exam Table/ to assist people with wheelchairs and scooters. The weight scale is able to Scale accommodate a wheelchair.
