shape with surrounding erythema and generally puritic Painful or burning is not characteristic and suggests vasculitis Individual lesions usually fade in 24-48 hours, new lesion may develop simultaneously at other sites Vasculitis lesions are palpable and usually nonblanching, span several days and often have residual hyperpigmented changes Angioedema typically is nonpruritic, brawny, nonpitting edema, without well defined margins and without erythema Urticaria versus Vasculitis Angioedema Chronic Evaluation History Relationship of episodes to the following: Ingestion (medication or food) Time of day Menstrual cycle Physical stimuli Exertion Occupational exposure Stress o Physical o Emotional/Mental Chronic Evaluation Testing Most often not indicated for chronic cases Targeted labs based on clinical suspicion may include: CBC with differential, sedimentation rate and/or C-reactive protein, liver enzyme, and thyroid stimulating hormone level Recurrent angioedema in absence of urticaria Evaluate for hereditary angioedema, acquired C1 inhibitor deficiency, or ACE-I associated angioedema if clinically indicated Physical Urticaria Subgroup of patients with tendency to have flares from environmental stimuli on inflammatory cells Mechanism of mast cell activation uncertain Estimated 0.5% of population is affected Lifetime prevalence 4-6% Comprises 20-30% of all cases of CU Variable resolution of disease 13-16% at 1 year, 50% after 5 years Dependant on subtype, age of onset, and severity Physical Urticaria Aquagenic Rare condition, 0.3% of CU Trigger is direct contact with any water source independent of temperature Confirmed with 35°C water applied to skin for 30 minutes Cholinergic 11% of young adults, 2-5% of CU “pinpoint” (1-3mm) hives surrounded by larger flares associated with increased core body temperature Common factors: exercise, sweating, emotion, hot baths/showers, saunas Severity ranges from mild puritus to serious life-threatening reactions Physical Urticaria Cold 2% of CU Pruritis and swelling with cold stimulus exposure Systemic reaction associated with systemic cold exposure (aquatic activities) Confirmed by applying cold stimulus to skin with wheal-and-flare appearing during skin re-warming Treatment is avoidance, pharmacotherapy in some case (ie: epinephrine for systemic reaction history) Delayed-pressure 1-2% of CU Swelling, that can be painful, delayed onset 4-6 hours (occasionally 12-24 hours) after pressure exposure Common factors: working with tools, sitting on a bench, constricting garments Confirmed by 15# weight suspended over shoulder for 15 minutes Often very difficult to treat, conventional antihistamine dosing frequently not efficacious Physical Urticaria Dermatographia Most common physical urticaria, 2-5% of general population, 10% of CU Wheal-and-flare quickly with pressure applied to skin Confirmed by stroking skin with firm object Exercise-provoked Occur in two conditions Cholinergic Exercise-induced anaphylaxis – 2 types o Provoked by exercise o Exercise temporally related to food or medication (ASA) ingestion • Two subgroups – specific food trigger and non-specific • Wheat & celery most common o Management includes – avoiding exercise several hours after eating, carry injectable epinephrine, exercise with partner, and wearing medical ID jewelry • If symptoms begin, immediate cessation of activity Physical Urticaria Solar 0.4-0.5% of CU Quick appearance (generally 1-3 minutes) with sunlight exposure Confirmed with phototesting with different wavelengths Vibratory 0.1% of CU Pruritis and swelling to vibratory stimulus Can be familial Confirmed by exaggerated response with skin exposure to a vortex mixer Chronic Management Non-pharmacologic Avoidance of exacerbating triggers NSAIDs, heat, tight-fitting clothing, and physical factors if indicated Pseudoallergens- substances that induce intolerance reactions o Additives, vasoactive substances, spices This retrospective study aims to characterise the features of erythroderma and identify the associated causes of this condition in our population. Materials and Methods: We reviewed the clinical, laboratory, histological and other disease-specifi c investigations of 225 inpatients and outpatients with erythroderma over a 7.5-year period between January 2005 and June 2012. Results: The most common causative factors were underlying dermatoses (68.9%), idiopathic causes (14.2%), drug reactions (10.7%), and malignancies (4.0%). When drugs and underlying dermatoses were excluded, malignancy-associated cases constituted 19.6% of the cases. Fifty-fi ve percent of malignancies were solid-organ malignancies, which is much higher than those previously reported (0.0% to 25%). Endogenous eczema was the most common dermatoses (69.0%), while traditional medications (20.8%) and anti-tuberculous medications (16.7%) were commonly implicated drugs. In patients with cutaneous T-cell lymphoma (CTCL), skin biopsy was suggestive or diagnostic in all cases. A total of 52.4% of patients with drug-related erythroderma had eosinophilia on skin biopsy. Electrolyte abnormalities and renal impairment were seen in 26.2% and 16.9% of patients respectively. Relapse rate at 1-year was 17.8%, with no associated mortality.

The antiinfl ammatory properties of the H1 antihistamines could be of relevance in chronic urticaria, though it is not clear to what degree they infl uence the fi nal therapeutic result. Before moving on to another therapeutic level, the advisability of antihistamine dose escalation should be considered, involving increments even above those approved in the Summary of Product Characteristics. Physical urticaria, when manifesting isolatedly, tends to respond well to H1 antihistamines, with the exception of genuine solar urticaria and delayed pressure urticaria.

    10 Chronic Urticaria as an Autoimmune Disease Michihiro Hide, Malcolm W. Greaves Introduction Urticaria is conventionally classified as acute, intermittent and chronic (Grea- ves 2000a). Acute urticaria which frequently involves an IgE-mediated im- munological mechanism, is common, its causes often recognised by the patient, and will not be considered further. Intermittent urticaria – frequent bouts of unexplained urticaria at intervals of weeks or months – will be dis- cussed here on the same basis as ‘ordinary’ chronic urticaria. The latter is conventionally defined as the occurrence of daily or almost daily whealing for at least six weeks. The etiology of chronic urticaria is usually obscure. The different clinical varieties of chronic urticaria will be briefly considered here, and attention will be devoted to a newly emerged entity – autoimmune chronic urticaria, since establishing this diagnosis has conceptual, prognostic and the- rapeutic implications. Contact urticaria and angioedema without urticaria will not be dealt with in this account. Classification of Chronic Urticaria The clinical subtypes of chronic urticaria are illustrated in the pie-chart of Fig. 1. The frequency of these subtypes is based upon the authors’ experience at the St John’s Institute of Dermatology in UK. Whilst there may well be mi- nor differences, it is likely that the frequency distribution of these subtypes will be essentially similar in most centres in Europe and North America (Grea- ves 1995, 2000b). Last reviewed by Robbie Pesek, MD September 14, 2016. Key Points Urticaria and angioedema are common problems and can be caused by both allergic and non- allergic mechanisms. Prompt diagnosis of hereditary angioedema (HAE) is important to prevent morbidity and mortality. Several new therapeutic options are now available. Patients with urticaria and/or angioedema should be referred to an allergist/immunologist for symptoms that are difficult to control, suspicion of HAE, or to rule out suspected allergic triggers. Definition, Assessment, and Diagnosis Definitions Urticaria is a superficial skin reaction consisting of erythematous, raised, blanching, well- circumscribed or confluent pruritic, edematous wheals, often with reflex erythema.1-3 Urticarial lesions are typically pruritic, and wax/wane with resolution of individual lesions within 24 hours. Angioedema is localized swelling of deep dermal, subcutaneous, or submucosal tissue resulting from similar vascular changes that contribute to urticaria.1,2 Angioedema may be pruritic and/or painful and can last for 2-3 days depending on etiology.3 1 Urticaria alone occurs in 50% of patients and is associated with angioedema in 40% of patients. Isolated angioedema occurs in 10% of patients.1,2 Hereditary angioedema (HAE) is a disorder involving defects in complement, coagulation, kinin, and fibrinolytic pathways that results in recurrent episodes of angioedema without urticaria, usually affecting the skin, upper airway, and gastrointestinal tract.4 In children, acute urticaria is more common than chronic forms.

    URTICARIA AND ANGIOEDEMA What are the aims of this leaflet? This leaflet has been written to help you understand more about urticaria and angioedema. It tells you what they are, what causes them, what you can do about them, and where you can find out more about them. What is urticaria and angioedema? Urticaria is common, and affects about 20% of people at some point in their lives. It is also known as hives or nettle rash. The short-lived swellings of urticaria are known as weals (see below) and typically any individual spot will clear within 24 hours although the overall rash may last for longer. Angioedema is a form of urticaria in which there is deeper swelling in the skin, and the swelling may take longer than 24 hours to clear. An affected individual may have urticaria alone, angioedema alone, or both together. Both are caused by the release of histamine from cells in the skin called mast cells. When angioedema occurs in association with urticaria, the two conditions can be considered part of the same process. When angioedema occurs on its own, different causes need to be considered. There are different types of urticaria of which the most common form is called ‘ordinary or idiopathic urticaria’. In this type no cause is usually identified and often patients have hives and angioedema occurring together. Ordinary urticaria with or without angioedema is usually divided into ‘acute’ and ‘chronic’ forms. In ‘acute’ urticaria/angioedema, the episode lasts from a few days up to six weeks. Chronic urticaria, by definition, lasts for more than six weeks.
