NEW YORK STATE PUBLIC TRANSPORTATION SAFETY BOARD RAIL SAFETY SECTION ABBREVIATED REPORT CASE NUMBER: 9531 DATE OF ACCIDENT: October 14, 2007 CARRIER: MTA Long Island Rail Road TYPE OF ACCIDENT: Collision SYNOPSIS: On Sunday, October 14, 2007, at approximately 6:22 p.m., the conductor on board westbound train #8709 operating on the Montauk Branch east of Southampton Station reported to the train’s engineer that he heard a loud banging noise outside the rear end of the first passenger car. The eight car train was made up of six coach cars and two locomotives and was traveling at a speed of approximately 40 mph in an area where the maximum allowable speed is 65 mph. There were approximately 250 passengers on board the train and the train crew consisted of an engineer, a conductor, and an assistant conductor. Approximately eight minutes later, when the train arrived at the next scheduled stop in Southampton, the conductor detrained and visually inspected the outside of the train. The conductor found damage consistent with being struck by an object along the side of four of the six coach cars and an automobile wiring harness hanging from one of the coach cars. At 6:35 p.m., the conductor radioed the Long Island Rail Road Movement Bureau of the damage he discovered. The Movement Bureau ordered the train to continue on to Speonk Station and to hold there for MTA Police and LIRR supervisory personnel to further inspect the train. Passengers on board the train were discharged at Speonk Station at 6:48 p.m. -

NEW YORK STATE PUBLIC TRANSPORTATION SAFETY BOARD RAIL SAFETY SECTION ABBREVIATED REPORT CASE NUMBER: 9600 DATE OF ACCIDENT: December 13, 2007 CARRIER: MTA –Long Island Rail Road TYPE OF ACCIDENT: Grade Crossing ACCIDENT SYNOPSIS: At 2:51 p.m. on Thursday, December 13, 2007, LIRR train #2709 departed Montauk Station. The train, operated by an engineer, conductor, assistant conductor, and ticket collector, consisted of six bi-level coach cars and one locomotive. As the westbound train approached the Gleason Drive grade crossing in East Quogue, New York, the engineer reported seeing a vehicle without its lights on obstructing the tracks. The engineer repeatedly sounded the train’s horn and placed the train’sbrakes in emergency but could not avoid striking the automobile on the crossing. According to police reports, at approximately 4:06 p.m., the driver stated that while traveling northbound on Gleason Drive the vehicle skidded on the slick roadway and slid off the east end of the crossing pad surface becoming stuck. The driver claimed he attempted to free the stuck vehicle for approximately ten minutes but was unsuccessful. When the driver heard the horn of the approaching train, he exited the vehicle and moved a safe distance away. The vehicle was struck and knocked approximately forty feet across the crossing striking the signal case on the southwest side of the crossing and came to rest. The speed of the train at the time of the impact was estimated to be 55 MPH. Damage to rail equipment was minimal and consisted of damage to the brake piping, step ladder, snow plow and brake hoses. -

7367-7373 Derailment Case
NEW YORK STATE PUBLIC TRANSPORTATION SAFETY BOARD RAIL SAFETY SECTION PTSB CASES #7367 AND #7373 INVESTIGATION OF TWO DERAILMENTS INVOLVING THE MTA - LONG ISLAND RAIL ROAD ON #73 SWITCH IN JAY INTERLOCKING, JAMAICA, NEW YORK ON JANUARY 23, 2003 AND JANUARY 25, 2003 SYNOPSIS On Thursday, January 23, 2003 eastbound equipment train #3800 derailed in the Jay Interlocking which is located in Jamaica, Queens, New York. The rear wheel set on the third car and the lead set on the fourth car derailed as they passed over #73 switch. Two days later, on Saturday, January 25, 2003, eastbound equipment train #8738 derailed four wheels on the lead truck of the first coach car (the second car in the consist) as it too passed over #73 switch. Neither train was carrying passengers at the time it derailed. In both cases, at least one car passed over #73 switch before the derailments occurred. Both derailments caused moderate damage to the track and car equipment. The cause of the derailments was not determined until after the second derailment on January 25, 2003 when the investigation discovered that a newly installed switch point on #73 switch's right hand switch point was not manufactured to the correct specifications . The defective switch point and its companion stock rail were installed one day before the first derailment on January 22, 2003 . INVESTIGATION Accident Scene : The Jay Interlocking is located west of Jamaica Station, in Queens, New York. The station and interlocking is one of the railroads busiest hubs with all trains routed through this interlocking going to the various branch lines . -

2010 MTA Annual Performance Review
PERMANENT CITIZENS ADVISORY COMMITTEE TO THE MTA REPRESENTING MTA RIDERS SINCE 1981 2010 Annual Performance Review of The MTA and Its Operating Agencies Introduction The 2010 Annual Performance Review marks the fourth report by PCAC which evaluates the yearly initiatives and accomplishments of the MTA and its operating agencies. As in the past, we address the broad themes that affect the ability of the MTA to deliver quality service to the riders that we represent: leadership, transparency, accountability, service performance, communication, community outreach, security, capital program management, etc. An important guide in this review process is previous years’ assessments in order to gauge the progress made in policies and practices. Last year the MTA was fortunate to have sustained and capable leadership at Headquarters and the Agencies. However, the organization faced extreme financial pressures that required not only a fare hike but painful service cuts. Chairman and CEO Jay Walder has committed the MTA to “Making Every Dollar Count” and finding efficiencies within the system that will enable the MTA to overcome deficits without additional outside funding. The PCAC will be looking for performance measures and more accountability to accompany this slogan. Despite the gloomy financial environment, the MTA and the Agencies did make strides in a number of areas: an improved website; technology applications to operations and communication; streamlining the MTA Board Committee structure; progress in the long stalled installation of CCTV cameras in subways and countdown clocks; much greater transparency as a result of the capital program and performance dashboards on the website; and continued support of transit-oriented development projects.
