Welcome, Action Sheet WM welcomed all to the meeting. WM gave the group an overview of the development of Chapelton from its initial design phase to the ongoing building work, and further future development. For more information on this, please refer to briefing note. Minutes from meeting 19/05/2016 approved. Action Sheet : Actions related to engagement closely linked and may be merged into a single action which addresses all. Ensure deadlines for actions are updated. Local Community Plan - update The draft Local Community Plan 2016-2019 is being presented to the CP Board for endorsement 29th June 2016. The plan was agreed by the LCPG 23/06/2016. Further action planning will be an ongoing process, with the plan template being in an editable format. Actions will be presented in a condensed form for ease of use by the LCPG. Monitoring will be done using Covalent. Training is to be provided on this, facilitated by central Community Planning Team at Woodhill House. 3. Review of Empowerment Joint Meeting The group looked at the identified strengths/opportunities and barriers being faced in Kincardine & Mearns as identified at the joint meeting last November. The group identified the points they felt were most important to focus on next. They were: Ability to build on strengths/good practise Communication – identified as key barrier These will now be considered in relation to the Community Plan, and further development work. 4. Updates KDP are involved in 6 Community Action Plans at present in Johnshaven, Stonehaven, Portlethen, Drumoak, Newtonhill & Gourdon, see briefing note May 2016 for more detail. WM discussed with the group the changing role of the HSCI LRG. The lifespan of this group is officially over, however, in the absence of local management structure, there is value in retaining the group, though meeting less frequently with a remit to be decided. The group is now being facilitated by Paul Gleisner. SC informed the group of presentations being held on fire safety at Inverbervie and Mackie Academy, and about a forthcoming joint exercise on water rescue involving RNLI, Coastguard & SFRS. A review of Community Planning is in the initial stages looking at the CP Board, CP Executive and CP structures. 5. Budget Editable template for LCP 2016 – 2019 £200 - agreed. Shared News IH informed the group about an enterprise in Marykirk providing high speed internet connection. Community Councils, Local Rural Partnerships, Skills Development Scotland, Scottish Fire and Rescue Service, Police Scotland, NESTRANS, NHS Grampian, Scottish Enterprise and our community had experienced in the recent hospitalisation of his mum. After GP refusal to put his mum within the virtual ward, she was, unfortunately, hospitalised. During her stay at ARI she fell. Upon discharge, she went home but could not be independently mobile. Upon contacting the GP surgery it was found they had no knowledge of the release from hospital of MM’s mum, and no knowledge of her ongoing medical needs. A district nurse upon visiting, was quickly able to determine a care plan suitable, and ensure steps put in place to address her needs. MM informed the LCPG of this story to highlight a disconnect between hospital discharge and knowledge of discharge by the local GP practise. Prior to discharge a care plan should have been in place ensuring the right level of care at the right time, and not being reliant on the family organising this. 8.
