It primarily affects the front legs, but in some cases the hind legs can be affected too. Discomfort in the region of the navicular apparatus can be caused by pain in this region, by derogation of the navicular bone itself, or by the surrounding soft tissues. This thesis describes a case of a horse that was presented to the department of Surgery at the Faculty in Merelbeke (UGent). The clinical examination pointed out that the horse was very stiff and lame. Radiographic imaging was done foremost due to the severity of the symptoms, to see if there were any subtle or obvious changes in the hoof. Since this was not the case for this horse, local analgesia was safe enough to use to locate the region from where the pain originated. Because palmar digital nerve block on all four legs was positive, and the intrabursal anaesthesia of the most lame limb was positive as well, the horse was diagnosed with navicular syndrome. The owners decided to try a conservative therapy, before going over to advanced medical imaging such as magnetic resonance imaging. The therapy consisted of an intrabursal injection with a corticosteroid (methylprednisolone) combined with corrective shoeing and trimming. After 5 months the owners were contacted to check up on the horse. It had responded very well to the treatment and was still sound. Samenvatting Hoefkatrol of podotrochleïtis is een veel voorkomend probleem, vooral beschreven bij warmbloeden van middelbare leeftijd. In de vakliteratuur wordt podotrochleïtis vaak vermeld als een probleem dat voornamelijk voorkomt in de voorvoeten. Desondanks worden er op de Faculteit in Merelbeke (UGent) jaarlijks een 30-35 gevallen van paarden die aan de vier voeten mank zijn gediagnosticeerd, waarvan er een deel zelfs wordt aangeboden met de zwaarste kreupelheid komend uit de achtervoeten. Podotrochleïtis is een aandoening waarbij het probleem zich lokaliseert in de regio podotrochlearis van de hoef. Het kan gaan om simpelweg pijn in die regio, vroege of al meer uitgesproken botveranderingen van het straalbeen, of aantasting van de omgevende weke delen. In deze thesis wordt een casus besproken van een paard met viervoetsmanker conditie. Het dier werd kreupel en stijf bij de vakgroep Heelkunde en Anesthesie van de huisdieren aan de Faculteit in Merelbeke (UGent) gebracht. Het werd enkele dagen gehospitaliseerd om de uitgebreide klinische onderzoeken en herhaaldelijke lokale anesthesieën correct te kunnen uitvoeren. Het gebruik van lokale anesthesie wordt erg gewaardeerd als diagnostische methode om aan te kunnen tonen waar de pijn in het lidmaat vandaan komt. Lokale anesthesie in de bursa is erg specifiek; wordt de bursa positief bevonden, kan men vervolgens nog aanvullende diagnostische methodes, zoals medische beeldvorming, gebruiken om het onderzoek verder toe te spitsen op de oorzaak van de pijn in die regio. Bij het paard in deze casus werden er nauwelijks radiografische veranderingen aangetroffen. Het is vaak zo dat op radiografische opnames niet veel veranderingen te zien zijn, of dat deze niet significant genoeg zijn om de pijn in de regio podotrochlearis te verklaren. Andere medische beeldvormingstechnieken, zoals magnetic resonance imaging (MRI), geven vaak betere beeldkwaliteit, van zowel het straalbeen als van de omgevende weke delen. De eigenaars van het paard besloten om eerst een therapeutische behandeling te proberen, vooraleer over te gaan naar MRI. Op basis van de positieve lage anesthesie op alle vier de benen, en een positieve bursa anesthesie van het meest manke lidmaat, werd het paard conservatief behandeld voor hoefkatrol. Het paard werd lokaal behandeld in de bursa podotrochlearis met een middellang-werkend glucocorticoïd (methylprednisolone). Om laminitis te voorkomen werd deze behandeling in 2 stappen uitgevoerd, eerst de voorvoeten en enkele dagen nadien de achtervoeten. Ook werd er een correctief beslag aangebracht op alle 4 de hoeven. Een rustperiode werd aangeraden waarna de werklast gradueel kon worden opgebouwd. Er zijn meerdere behandelingen beschreven voor podotrochleïtis, maar het gaat hier voornamelijk om tijdelijke oplossingen die het paard een hoger comfort kunnen geven. De uitkomst van de gekozen behandeling of combinatie van verschillende behandelingen dient afgewacht te worden. Het effect van de behandeling is individu afhankelijk en kan moeilijk voorspeld worden. Vijf maanden na de lokale behandeling werden de eigenaren gecontacteerd, waaruit bleek

Reid Hanson, DVM, DACVS, DACVECC Auburn University Abstract: Navicular syndrome is a chronic and often progressive disease affecting the navicular bone and bursa, deep digital flexor tendon (DDFT), and associated soft tissue structures composing the navicular apparatus. This syndrome has long been considered one of the most common causes of forelimb lameness in horses. Diagnosis of navicular syndrome is based on history, physical examina- tion, lameness examination, and peripheral and/or intraarticular diagnostic anesthesia. Several imaging techniques (e.g., radiography, ultrasonography, nuclear scintigraphy, thermography, computed tomography [CT], magnetic resonance imaging [MRI]) are used to identify pathologic alterations associated with navicular syndrome. Radiographic changes of the navicular bone are not pathognomonic for navicular syndrome. Additionally, not all horses with clinical signs of navicular syndrome have radiographic changes associated with the navicular bone. Therefore, newer imaging modalities, including CT and especially MRI, can play an important role in identifying lesions that were not observed on radiographs. Navicular bursoscopy may be necessary if the clinical findings suggest that lameness originates from the navicular region of the foot and if other imaging modalities are nondiagnostic. The navicular bone is a baby carrot sized and shaped bone tucked behind your horse’s coffin bone within the hoof. The deep digital flexor tendon (DDFT) courses over the navicular bone to insert on the caudal (back) portion of the coffin bone. In between the navicular bone and the DDFT sits the navicular bursa, a synovial “cushion” that allows the tendon to glide smoothly over the flexor surface of the navicular bone. Navicular syndrome/disease used to encompass any lameness in the foot that could not be attributed to something definitive, like an abscess or laminitis. Now with more advanced imaging modalities, including digital x-ray and MRI, navicular syndrome is defined by specific degenerative changes to the navicular bone and the deep digital flexor tendon. Pathogenesis The exact pathophysiology of navicular syndrome is unknown. Multiple factors are believed to be involved. Distal limb conformation, and the resulting biomechanics, play a large role. A “broken back” hoof-pastern axis, underrun heels, and long toes can put excess pressure on the navicular bone, causing a low level of damage, which over time leads to degeneration. Certain breeds such as warmbloods, Quarter Horses, and Thoroughbreds, are predisposed to developing navicular syndrome. Horses tend to be 8-10 years old at onset of clinical signs. Clinical Signs Classic navicular syndrome presents as an intermittent, mild forelimb lameness.

Duesterdieck-Zellmer Abstract: Intravenous regional limb perfusion (IVRLP) with tiludronate is a commonly used treatment for distal limb orthopedic disease in horses, but doses and protocols are anecdotal. IVRLP exposes articular cartilage within the perfused area to tiludronate, raising concerns about safety of this treatment, as high tiludronate concentrations (≥19mg/L) were harmful for cartilage in vitro. This is the first study to evaluate synovial fluid tiludronate concentrations following IVRLP to determine safety for articular cartilage in horses. Synovial fluid cytology variables and tiludronate concentrations were evaluated in the navicular bursa, coffin and fetlock joints following IVRLP of one front limb with low dose (0.5mg, n=6) or high dose (50mg, n=6) tiludronate. The contralateral limb was perfused with saline as a control. Synovial fluid samples were taken 1 week prior and 30 minutes following IVRLP from all structures and 24 hours post-perfusion from coffin and fetlock joints. Synovial fluid tiludronate concentrations were lower in limbs perfused with 0.5mg in all synovial structures (metacarpophalangeal joint = 3.7 ± 1.5 mg/L, distal interphalangeal joint = 16.3 ± 1.9 mg/L, navicular bursa = 6.0 ± 1.9 mg/L) than in limbs perfused with 50 mg (metacarpophalangeal joint = 0.04 ± 0.02 mg/L, distal interphalangeal joint = 0.12 ± 0.06 mg/L, navicular bursa = 0.08 ± 0.03 mg/L) at tourniquet release.

    Hoof Care Horses that are housed in stall or small pens should have their feet picked out daily. This will aid in examination for rocks, sticks or other foreign objects. The removal of mud and organic matter will also reduce the risk for infection or “thrush.” Thrush is a foul-smelling bacterial infection that may soften the foot, and in severe cases, invade deeper tissues. Your horse should be trained to readily pick up its foot when asked. Using a hoof pick, clean the foot from toe to heel, being sure to clean the commisures or sulci on each side of the frog, and sulcus of the frog itself. It is a good idea to clean out the feet after you ride. Carry a hoof pick with you on trail rides, so that you can readily remove rocks or stick from your horse’s hoof. Extremely dry or brittle feet may cause excessive chipping of the hoof wall. There are several commercial hoof dressings on the market that may be useful. Oral hoof supplements, usually containing biotin and methionine, may aid in hoof growth. Very soft soles may be tender and cause the horse to be lame when walked over gravel. Topical dressings, such as Keratex, iodine, or Venice Turpentine, may be useful in toughening the sole. Talk to us for product recommendations. Trimming and Shoeing Generally, horses are trimmed and/or shod in a 6-week cycle. This guideline may vary due to individual hoof growth differences. It is important to train your horse to stand obediently for the farrier.

It is possible, however that a number of clinical signs are evident to the careful observer some 18 to 24 months before the onset of lameness. It is this state that will be referred to as the ‘pre-navicular syndrome’. The almost total reliance on radiographic evidence of areas of radiographic lucency, increase in number, or change in shape of foramina in the navicular bone for the purposes of diagnosing navicular disease often diminishes the prospects of a complete cure, as these are seen in the latter stages of the disease. The subsequent administration of anticoagulant drug therapy (or Isoxsuprine) would seem to be palliative rather than curative - and any improvement temporary - as long as the root mechanical causes are neglected. Farriers and veterinary surgeons should recognise the signs of change in the horse’s behaviour, gait, feet, and shoe-wear to affect an early diagnosis of a developing syndrome. This clinical condition will, if left untreated, develop into navicular disease. It is my contention that, with intelligent analysis of the signs that are apparent in the pre-navicular syndrome and with the help of regular shoeing to restore the correct mechanical balance of the limbs and feet of affected horses, the development of the syndrome can be prevented and a permanent and effective cure can be obtained. Etiology Navicular disease is a chronic progressive forelimb lameness and is the final stage of a clinical syndrome.
