Simply go to our enrollment section located in the center of this catalog and complete the 3 easy steps. to help you GETTING YOUR SUPPLIES IS EASY: Ostomy Supplies: Requires little or no upfont costs because we accept assign- ment on most ostomy products covered by Medicare. You are only responsible for the 20% co-pay and annual deductibles required by Medicare. If you have a secondary insurance, they may cover these costs for you, requiring no payment from you at all. Urological Supplies: DO require payment upfront, because we do not accept as- signment on these products. We will submit your claim to Medicare on your be- half, and reimbursement will be sent directly to you. Shipping: • Standard shipping is free for orders that we accept assignment on. • On unassigned orders, standard shipping is free on orders over $95.00.
