A total of 189 CAD patients were consecutively enrolled in this prospective cohort study and treated by aspirin, with or without clopidogrel. Venous blood was obtained and detected by TEG. Arachidonic acid (AA) inhibition rate was (61.83±26.30)% and AR occurred in 62 patients (32.8%). Additionally, adenosine diphosphate (ADP) inhibition rate was (41.83±32.07)% and clopidogrel resistance occurred in 67 patients (35.4%). Values of R time and K time were 6.75±1.82 minutes and 1.69±0.73 minutes, respectively, and α angel was 62.50±10.47°. Maximal amplitude (MA) and AA-induced and ADP-induced platelet-fibrin clot strengths were 62.85±7.39 mm, 30.81±14.93 mm, and 39.28±13.70 mm, respectively. Multivariate analysis showed that age and white blood cells (WBC) were indepen- dent factors for predicting higher risk of AR, while hemoglobin (Hb) was an independent factor for absence of AR. Furthermore, the combination of age, WBC, and Hb displayed a good predictive value for AR. TEG could assess the antithrombotic effect of aspirin, with or without clopidogrel treatment in CAD patients. Combination of age, WBC, and Hb demonstrated a good predictive value for AR risk. Our objective was to review the evidence for their role in diagnosing early coagulopathies, guiding blood transfusion, and reducing mortality in injured patients. Methods: We considered observational studies and randomized controlled trials (MEDLINE, EMBASE, and Cochrane databases) to February 2014 that examined TEG®/ROTEM® in adult trauma patients. We extracted data on demographics, diagnosis of early coagulopathies, blood transfusion, and mortality. We assessed methodologic quality by using the Newcastle-Ottawa scale (NOS) for observational studies and QUADAS-2 tool for diagnostic accuracy studies. Results: Fifty-five studies (12,489 patients) met inclusion criteria, including 38 prospective cohort studies, 15 retrospective cohort studies, two before-after studies, and no randomized trials. Methodologic quality was moderate (mean NOS score, 6.07; standard deviation, 0.49). With QUADAS-2, only three of 47 studies (6.4%) had a low risk of bias in all domains (patient selection, index test, reference standard and flow and timing); 37 of 47 studies (78.8%) had low concerns regarding applicability. Studies investigated TEG®/ROTEM® for diagnosis of early coagulopathies (n = 40) or for associations with blood-product transfusion (n =25)ormortality(n =24).Most(n = 52) were single-center studies.

It should be kept in mind that the TEG is a global test of coagulation and therefore the need for additional haemostatic tests should be evaluated when applicable.

The pathogenesis of RVO remains uncertain, but it is believed to be multifactorial and to depend on both local and systemic factors, which can be divided into vascular, platelet, and hypercoagulable factors. The vascular factors include dyslipidaemia, high blood pressure, and diabetes mellitus. Regarding the platelet factors, platelet function, mean platelet volume (MPV), platelet distribution width (PDW), and platelet large cell ratio (PLCR) play key roles in the diagnosis of retinal vein occlusion and should be monitored. Nevertheless, the role of a hypercoagulable state in retinal vein occlusion remains unclear and requires further studies. Therefore, the following article will present the risk factors of RVO associated with coagulation disorders, as well as the acquired and genetic risk factors of thrombophilia. From November 2017 until June 2019, 109 additional patients were prospectively enrolled. For this study, in-hospital thrombotic and hemorrhagic complications (both CNS and non-CNS) were tabulated, and receiver operating characteristic (ROC) curve analysis was used to identify threshold values of the PRU and MA- ADP for predicting each type of complication.

Luddington, Ph.D.1 ABSTRACT The thromboelastography trace provides a graphical and numerical representation of the viscoelastic changes associated with fibrin polymerization. When used with whole blood, the shape of this trace is a composite of the effects of white and red cell content and composition, platelet number and function, fibrinogen concentration, as well as coagu- lation protein function and balance. The trace is also influenced by pharmacological agents such as anticoagulants, antiplatelet therapy, and coagulation factor supplementation. As such the main role of this technology has been as a point-of-care device to guide transfusion of blood components. Recently the technology has moved from the cardiac and hepatic surgical settings, where most of the early work was focused, into other areas of hemostatic monitoring. New applications for pharmaceutical monitoring and patient screening are being explored. This review gives a broad overview of the applications of the technology. In particular it considers the factors that most influence the characteristics of the trace, be they preanalytical, analytical, or clinical.
