Sri Lanka’s national road network is administered by the Road Development Authority while other roads are administering by the Provincial Councils and local government authorities. Almost all rural roads are under these Provincial Councils and local government authorities. However, the condition of this rural road network is very poor and as a result it does not provide sufficient road accessibility for the rural people in the country. Furthermore, the existing poor road condition has become a major cause for the poor livelihood condition of the people. Therefore, road infrastructure is one of the prioritized components for sustainable development of Sri Lanka. 2. The Government of Sri Lanka has requested financing from the World Bank to undertake a Provincial Project to upgrade and rehabilitate the dilapidated provincial road infrastructure. Provincial roads are the key connectors between the rural and the national networks. The proposed project for improvement of provincial roads in the Uva province and the Ampara district in the Eastern Province, Sri Lanka is being prepared to provide road users with improved sustainable road transport by enhancing quality, durability, and efficiency, of the provincial road network, which would be the key to economic and social development in rural Sri Lanka. 3. Both provinces are predominantly rural and land is the main asset of the people.

The overall project scope has six components and the marketing and enterprise development is the fifth component. The marketing component of the Dry Zone Livelihood Partnership Programme (DZLiSPP) is aimed at solving the marketing issues such as uncertainty and unpredictability of agricultural products and seasonal fluctuations of prices that discourage prices in harvesting. Hence, one of the approaches of DZLiSPP was to build links with the producer and the buyers and they have established two vegetable collection centers at Wangiyakumbura and Ruwalwela as a public-private partnership programme with Cargills PLC. The partnership is expected to help increase the farm gate price for farmers, better market access, reduce the involvement of intermediaries, and provide training facilities for farmers thereby improving their standards of living. Further, the farmers are benefited by the welfare fund. The benefits for the company are to access quality farm produce, facilitate direct purchase and reduce transaction cost.

    Concept Note CONSULTATIVE DISCUSSIONS TO SUPPORT ESTABLISHMENT AND IMPLEMENTATION OF SHARED CARE CLUSTER SERVICES IN THE 9 DISTRICTS BASED ON THE HEALTH SERVICE REORGANIZATION POLICY AND THE ESSENTIAL SERVICE PACKAGE ADB Financed Health System Enhancement Project, Sri Lanka Background The ADB financed Health System Enhancement Project (HSEP) as described under Output 1.4 (section on project description in the Project Administration Manual of the HSEP), intends to support the policy reform related to the establishment of 9 shared care clusters on a phase one pilot basis. The phase one implementation is based on the recently drafted ‘essential service package’1 and the recently approved ‘reorganizing health care delivery for universal health coverage policy’2. The hospital clusters (one base hospital (secondary care hospital) linked with feeder divisional hospitals and primary medical care units (primary curative care facilities) and their catchment areas (based on health seeking behavior identified by Grama Niladhari divisions) ) are already identified by the respective Provincial and Regional Directors of Health Services and their teams in each of the 9 districts of Moneragala, Badulla, Nuwera Eliya, Matale, Kandy, Rathnapura, Kegalle, Anuradhapura and Polonnaruwa, where the HSEP is implemented (Annex 1). The administrative reforms including the directive required to establish shared care clusters are expected to be issued by the Ministry of Health, Nutrition and Indigenous Medicine and the required documentation and processes that need to be followed for providing high quality services will also need to be documented prior to launching a cluster service to the defined catchment population. The services that will be shared across each of the clusters will also be agreed prior to the establishment of the shared care clusters.
