Intrusion of Incisors to Facilitate Restoration: The Impact on the Periodontium Names of Investigators Date Background and Purpose This 60 year old male had severe attrition of his maxillary and mandibular incisors due to a protrusive bruxing habit. The patient’s restorative dentist could not restore the mandibular incisors without significant crown lengthening. However, with orthodontic intrusion of the incisors, the restorative dentist was able to restore these teeth without further incisal edge reduction, crown lengthening, or endodontic treatment. When teeth are intruded in adults, what is the impact on the periodontium? The purpose of this study was to determine the effect of adult incisor intrusion on the alveolar bone level and on root length. Materials and Methods We collected the orthodontic records of 43 consecutively treated adult patients (aged > 19 years) from four orthodontic practices. This project was approved by the IRB at our university. Records were selected based upon the following criteria: • incisor intrusion attempted to create interocclusal space for restorative treatment or correction of excessive anterior overbite • pre- and posttreatment periapical and cephalometric radiographs were available • no incisor extraction or restorative procedures affecting the cementoenamel junction during the treatment period pretreatment pretreatment Materials and Methods We used cephalometric and periapical radiographs to measure incisor intrusion. The radiographs were imported and the digital images were analyzed with Image J, a public-domain Java image processing program developed at the US National Institutes of Health. -

ABDULLAH, S. KANAGASINGAM* & D.A. LUKE ABSTRACT The aim of the study was to determine the frequency, size and location of apical and lateral foramina on anterior teeth. A total of 100 anterior teeth consisting of maxillary and mandibular incisors and canines were fixed in 10% formalin. Periodontal tissue remnants were mechanically removed and teeth were stained in 2% aqueous silver nitrate. The teeth were dried and examined using a Leica MZ 7.5 zoom stereomicroscope. The size of apical and lateral foramina and their distance from the anatomical apex of the tooth were measured directly using a calibrated eyepiece scale. Accessory foramina more than 1.8 mm from the apex were regarded as lateral foramina. Eighteen percent of teeth possessed more than one apical foramen. Seven teeth (three maxillary centrals, three maxillary canines, one mandibular lateral) had 11 lateral foramina each. The mean diameter of the lateral foramina was 0.14 mm (SD = 0.08) and their mean distance from the apex was 4.49 mm (SD = 2.63, range 1.9-10.5 mm). Multiple foramina were most common on maxillary canines and least common on maxillary laterals. The mean diameter of apical foramina for all teeth possessing a single foramen was 0.35 mm (SD = 0.10) and the mean apical foramen diameter for all teeth with multiple apical foramina was 0.22 mm (SD = 0.08). -

The aim of this study was to evaluate the influence of apical foramen widening on the healing of chronic periapical lesions in dogs’ teeth after root canal filling with Sealer 26 or Endomethasone. Study design. Forty root canals of dogs’ teeth were used. After pulp extirpation, the canals were exposed to the oral cavity for 180 days for induction of periapical lesions, and then instrumented up to a size 55 K-file at the apical cemental barrier. In 20 roots, the cemental canal was penetrated and widened up to a size 25 K-file; in the other 20 roots, the cemental canal was preserved (no apical foramen widening). All canals received a calcium hydroxide intracanal dressing for 21 days and were filled with gutta-percha and 1 of the 2 sealers: group 1: Sealer 26/apical foramen widening; group 2: Sealer 26/no apical foramen widening; group 3: Endomethasone/apical foramen widening; group 4: Endomethasone/no apical foramen widening. The animals were killed after 180 days, and serial histologic sections from the roots were prepared for histomorphologic analysis. -
