Preparing for Your Visit

We would like to make some suggestions to help you prepare for your appointment: 1. We are located at the address below. You can access directions by visiting PennMedicine.org. If you arrive late for your appointment, you may experience a considerable wait or be asked to reschedule your appointment. 3. Complete any forms in this envelope and bring them with you to your appointment. Please do not mail these forms to the office. 4. If you are scheduled for Balance Function Testing please review the enclosed instructions and complete the questionnaire prior to your appointment or follow the below link to access this information. PennMedicine.org/BalanceFunctionTesting 5. If you are scheduled to see and have not yet seen a Penn Otorhinolaryngology you need to bring an order from your primary care or referring physician the states ‘Consult to Audiology’. This includes Medicare and patients who are seeing one of our following your audiology appointment. 6. You are responsible for getting a referral from your if you need one. If no referral is received, your appointment may be rescheduled. Penn Otorhinolaryngology’s provider ID # is ______. 7. You are responsible for bringing your insurance card/s, a photo ID, and any insurance co-payment due at the time. We accept cash, checks, and credit cards. 8. Please bring medical records, test results, CD’s of imaging and /cytology slides. Our providers may require Penn to re-read your study results. Please contact your insurance company as they may not cover the fee to re-read your studies. 9. You may want to bring a book or tablet to read as you wait for your visit. While we strive to stay on time, occasionally there will be unforeseen delays. If you have questions or concerns about your appointment, we would be happy to help.
