This form is to be used to request a place at a school in Luton or in another local authority area. If you are using this application form to apply for a place for your child at an infant/primary school, junior school or high school, please thoroughly read through the How to apply for a school place guide before completing this application form. The guide can be downloaded from www.luton.gov.uk/admissions or you can get a copy from the Admissions Team. If you are using this application form to apply for a place for your child at a middle or upper school, please carefully read the relevant Council’s information guide which should be on their website. If you require a hard copy of a guide, please contact the relevant Council’s School Admission Team. If any information given on your application form or any amendments made to the form changes prior to the relevant closing date, you must notify the Admissions Team in writing immediately. Remember: If you have submitted an online application, there is no need to complete this application form. (Please see overleaf) HOW TO APPLY FOR A SCHOOL Application Form PLACE FOR SEPTEMBER 2021 Starting school admissions round Please return the completed application form to your first preference school, if this is in Luton or to Luton Council’s Admissions Team. If you are applying for a place at a Luton Catholic school, you must also complete the supplementary form (on page 13 of this application form) and provide your child’s original Baptismal Certificate to Luton Council’s Admissions Team. Your completed forms and any relevant paperwork must be provided by 15 January 2021. Parents who have applied using Luton Council’s online system, will receive an offer email on 16 April 2021; parents will not receive an offer letter in addition to the email. Parents who have completed an application form will have their offer letter posted to their home address on 16 April 2021. Transfer to junior school If your child attends a school in Luton, please return the completed application form to their current school. If your child attends a school outside of Luton or an independent (private) school, your completed application form must be returned to Luton Council’s Admissions Team. If you are applying for a place at Wenlock Junior School, you must also complete the Wenlock Church of 3 England Junior School supplementary from (on page 10 of this application form) and return this, with your completed application form to Luton Council’s Admissions Team. Your completed forms and any relevant paperwork must be provided by 15 January 2021. Parents who have applied using Luton Council’s online system, will receive an offer email on 16 April 2021; parents will not receive an offer letter in addition to the email. Transfer to middle school Please return the completed application form to Luton Council’s Admissions Team. If you are applying for a place at a middle school on religious or faith grounds, please contact the relevant Council to obtain any additional forms needed. Your completed forms and any relevant paperwork must be provided by 15 January 2021. Parents who have applied using Luton Council’s online system, will receive an offer email on 16 April 2021; parents will not receive an offer letter in addition to the email. Parents who have completed an application form will have their offer letter posted to their home address on 16 April 2021. If your child attends a school outside of Luton or an independent (private) school, your completed application form must be returned to Luton Council’s Admissions Team. If you are applying for a place at Cardinal Newman Catholic School, you must also complete the Cardinal Newman Catholic School supplementary from (on pages 11 and 12 of this application form) and return this, with your completed application form and your child’s original Baptismal Certificate to Luton Council’s Admissions Team. Your completed forms and any relevant paperwork must be provided by 31 October 2020. Parents who have applied using Luton Council’s online system, will receive an offer email on 1 March 2021; parents will not receive an offer letter in addition to the email. Parents who have completed an application form will have their offer letter posted to their home address on 1 March 2021. Transfer to upper school Please return the completed application form to Luton Council’s Admissions Team. If you are applying for a place at an upper school on religious or faith grounds, please contact the relevant Council to obtain any additional forms needed. Your completed forms and any relevant paperwork must be provided by 31 October 2020. 4 Parents who have applied using Luton Council’s online system, will receive an offer email on 1 March 2021; parents will not receive an offer letter in addition to the email. Parents who have completed an application form will have their offer letter posted to their home address on 1 March 2021. If any of your preferred schools are oversubscribed you may be asked to provide the following documentation to a member of staff at the school, if this is in Luton or to a member of the Admissions Team. • A recent (within the last 12 months) Child Benefit CH84TS form which shows your address at the time of application and your child’s name. Alternatively, you can provide your Working Tax Credit/Child Tax Credit award letter for 2020/21, this must include the pages that shows the name of your child. • A recent (within the last three months) gas bill or electricity bill or water bill which shows your address at the time of application. Alternatively, you can provide your 2020/21 council tax bill which shows your address at the time of application. Please note, the Admissions Team do not accept telephone bills, credit card or bank statements as proof of address. If your child attends a school outside of Luton or an independent (private) school, you will also need to provide a copy of your child’s birth certificate.     Contents Luton: Motor Town Luton: Motor Town 1910 - 2000 The resources in this pack focus on the major changes in the town during the 20th century. For the majority of the period Luton was a prosperous, optimistic town that encouraged forward-looking local planning and policy. The Straw Hat Boom Town, seeing problems ahead in its dependence on a single industry, worked hard to attract and develop new industries. In doing so it fuelled a growth that changed the town forever. However Luton became almost as dependant on the motor industry as it had been on the hat industry.     Lilley (Hertfordshire). The ‘Lilley Arms’ www.lilleyarms-lilley.co.uk A walk across a lovely, rolling, Chalk upland farm-scape with wide views. A route map is available at: www.walk4life.info/walk/icknield-way-lilley-circular Lilley is an attractive 'estate' village and many of the thatched cottages bear the silver lion rampant crest of the Sowerby family who once owned the Putteridge estate of which the village was part. Built in 1705 as a coaching inn, much of the historic character of the Lilley Arms has survived, along with its tradition of offering hospitality to travelers by providing bed and breakfast accommodation. Tranquil gardens with fountains and a patio can be enjoyed during the summer and there are cosy log fires in winter. Dogs are allowed in the bar and gardens but not in the food areas. Advertising itself as ‘'worth passing a few pubs for ', the Lilley Arms is a freehouse serving a range of hand-pulled real ales. There is an extensive menu of home-cooked, seasonal pub food, including a choice of vegetarian options and the pub’s Sunday roast lunches are so popular it is advisable to book in advance. Meals are not served on Sunday nights. Opening times: From 12 noon to 11 pm every day, including Sunday. Food is available from 12 noon to 2.00pm and 7.30 to 9 pm Monday to Saturday and 12 noon to 3.00 pm on Sunday. Telephone: 01462 415548. How to get there: Lilley lies 2 miles north-east of Luton. The village is situated just off the A505 between Luton and Hitchin or can be reached by following minor roads from the A6 at Streatley.
