We encourage you to choose a primary care provider (PCP). Seeing a PCP can help you stay healthy and lower your healthcare costs. Please call Member Services to tell us who you prefer. If you need help finding a doctor, call SelectHealth Member Advocates. They can help you find the closest doctors and schedule appointments for you.     Nutrition Counseling Nutrition counseling with a registered dietitian can help you improve your health and feel better — whether you’re already fairly healthy, or you have complex medical conditions. In nutrition counseling (also called Medical Nutrition Therapy, or MNT) you’ll learn how to improve your diet to help you lead a healthy life or to achieve specific health goals. It includes: • One-on-one counseling with a registered dietitian. A registered dietitian is a nutrition expert with an advanced degree in a dietetics program and clinical training in how diet can treat different health conditions. • A personalized eating plan and support.     FINANCIAL ASSISTANCE POLICY POLICY STATEMENT IHC Health services, Inc. (Intermountain) provides care to patients unable to pay. A range of discounts based on financial needs and income levels are offered. Intermountain recognizes its responsibility to fairly distribute and manage the cost of health care, while ensuring that financial assistance is offered to those in need in our communities. Intermountain meets that responsibility by asking those who can pay to do so, and by ensuring that the Financial Assistance policies incentivize people to maintain their healthcare insurance coverage and to seek care at the most appropriate delivery site. The Patient Protection and Affordable Care Act (ACA) requires each not-for-profit hospital to conduct a CHNA every three years to identify significant health needs in the community, report impact of previous community health improvement initiatives, and to develop an implementation plan to address and measure community health improvement activities created to address the significant health need. Alta View Hospital and Intermountain report compliance with the requirements on the IRS Form 990 Schedule H annually. Intermountain created CHNA reports and implementation plans for each of its 221 hospitals to make the documents publicly available. Alta View Hospital completed the CHNA in collaboration with Salt Lake County Health Department and the Utah Department of Health to identify health indicators, gather and analyze data, and prioritize the indicators to determine the significant health needs to address over the next several years.
