Abstract

Low back pain (LBP) is a subjective sensation characterized by a major symptom of pain in the lower spine region. One of the basic causes is the stress inflicted on the inter-vertebral disk due to poor posture, so that the lumbar spine of the lumbar lordosis has too much strain on the posterior element of the inter-vertebral disk. These complaints usually occur in workers due to incorrect posture or work position that does not meet the ergonomic principles of one of the traditional weavingworker. Exercise has been widely developed to cope with LBP such as stretching and muscle strengthening. William's flexion exercise (WFE) is an exercise aimed at stretching the posterior muscles and strengthening the abdominal muscles so that the tendon and spongy muscles are expected to relax. The purpose of this study is to determine the effect of WFE on LBP on traditional weaving workers. This study is a pre-experimental study (One-Group Pretest-Posttest Design). The sample consists of 28 respondents selected by using Purposive Sampling technique. Data collected by using questionnaires for respondent characteristics and Numeric Rating Scale for measuring pain scale before and after intervention. The result of the research before intervention was 75% of respondents had moderate pain, while after intervention 85.7% of respondents had mild pain. Based on the Wilcoxon test results obtained p = 0.000 which means there is the influence of WFE on LBP on traditional weaving worker. Based on the above research, it is suggested to health workers to teach WFE in order to reduce LBP on traditional weaving workers.

Abstract

Human Immunodeficiency Virus (HIV) is a virus that attacks white blood cells (lymphocytes) that cause the decline of human immune. High risk groups such as Female Sex Workers (FSWs) tend not to access health services because of the decrimination they face in society. Health promotion is still an important strategy in increasing self-awareness of FSWs to perform Voluntary Counseling and Testing (VCT) examination, one of them is through the utilization of cellular phone technology. This study aims to determine the effect of cell phone Short Message Service (SMS) as a media health education about HIV and Acquired Immune Deficiency Syndrome (AIDS) to health seeking behavior of FSWs. Pre-experimental design one group pre-test post-test design was conducted at 33 FSWs at Terminal Pesiapan, Tabanan District. The results of pre-test and post-test showed an influence on behavior (knowledge, attitude and action) after SMS based health promotion intervention (p <0,05), knowledge (p = 0,039), attitude (p = 0,003), and action (p = 0,000). After the intervention there was an increase in behavior that is on knowledge 30,3%, 36,4% attitude and 63,6% action. Mobile phone SMS media proves to be effective in conveying health information so as to raise awareness of FSWs in conducting HIV and Sexually Transmitted Infections (STIs). An innovative, short, easy-to-understand message on HIV and AIDS should be provided with simple language and regular use of cellular-based information technology by healthcare workers.

Abstract

Patients with Diabetes Mellitus (DM) often fail in going through the stages of grief, which can lead to depression. Diabetic patients who experience depression tend to be less active in the management of their DM, resulting in complications and death. This study aimed to analyze the correlation between the stages of grief and depression in patients with diabetes. This study used a cross-sectional design with an explanatory study approach. A consecutive sampling technique was used to select the samples of 302 patients with diabetes. Data were collected using Acceptance of Disease and Impairments Questionnaire and Beck Depression Inventory, and were analyzed using univariate and bivariate analyses. The results showed that the majority of respondents were female (75.2%), married (85.8%), and had comorbidities (80.8%). There was a correlation between stages of grief and depression in patients with diabetes. It was predicted that the strongest and influential stage of depression was acceptance. Decreasing level of acceptance will increase the depression by 0.141. This study concluded that stages of grief had a correlation and is predicted to contribute to trigger depression in patients with diabetes. Therefore, appropriate interventions are required for patients with diabetes to go through the stages of grief with adaptive coping, so as to prevent the occurence of depression.

Abstract

The existence of epidemiological transitions and the shift of infectious diseases into non- infectious diseases cause high degenerative and chronic diseases such as Diabetes Mellitus (DM). There are several types of diabetes mellitus, but of all types, type 2 diabetes mellitus is DM, which has the highest prevalence of around 90-95% of all DM patients in the world. The increase in the number of people with Type 2 DM is caused by many factors, such as excessive weight or overweight and obesity. Determination of more nutritional status or overweight can be seen from the Body Mass Index (BMI). High BMI is associated with blood sugar instability and the potential for complications such as micro vascular complications, namely neuropathy. Neuropathy causes problems in the sympathetic and parasympathetic nerves which will have an impact on changes in the salivary acidity. The aim of the study was to determine the correlations of BMI and salivary pH on Type 2 DM patients in Work Area of Health Center III North Denpasar. This study was a correlation study with a cross sectional approach. The sample in this study amounted to 41 participants chosen by consecutive sampling technique. Data collection was done by measuring height, weight, and salivary pH. Correlation analysis used Pearson Product Moment with a confidence interval of 95% (α = 0.05). The results of the Pearson Product Moment statistical test indicated that the correlation coefficient r=-0.398, and the Sig. value 0.010 (p <0.05). This result indicated that there was a weak negative correlation between BMI and salivary pH on Type 2 DM which means the higher BMI value, the lower salivary pH on Type 2 DM.

Abstract

The integration of cellular technology in healthcare (M-Health) has the potential in coping with the constraints of the medical service system and to become a breakthrough to support the patients’ medication adherence. This review was conducted to compare and evaluate the effectiveness of M-Health in terms of the patients' medication adherence. The literature review conducting on PubMed, Cochrane Library, Ebsco Health, and Taylor & Francis was confined to the articles published between 2010 and 2019. Fifteen articles relating to medication adherence were reviewed, eleven of which employed RCT methods including quasi-experiment that assessed the effectiveness of M-Health to measure adherence, while four other types of research employed qualitative studies to explore patients' perceptions. The review showed that the intervention of m-Health is effective to drive the compliance of the treatment following healthcare promotion efforts as well as the prevention, screening, and management of the diseases. Two studies have shown inconsistent results regarding the prevention of obesity and the management of hypertension patients. The development of such a model and the integration of parallel m- Health and healthcare promotion will improve long-term medication adherence.

Abstract

Hypertension remains a major problem in the health sector. Hypertension is a chronic disease that may lead to other serious illnesses and anxiety if managed unproperly. Therefore, hypertension requires a comprehensive treatment, both pharmacological and non-pharmacological. One of the non-pharmacological treatment for lowering blood pressure and anxiety is by relaxation technique, specifically with progressive muscle relaxation technique (PMR). This study aimed to determine the effectiveness of PMR to anxiety changes and blood pressure in patients with primary hypertension. This study conducted nonequivalent control group pretest-posttest design for research design, with simple random sampling technique. This study involved 91 respondents were divided into two groups, namely 71 respondents in the intervention group and 20 respondents in the control group. These results shown a majority of female respondents (57.1%), aged 56-65 years (44%), with the level of basic education (61.5%), and low socioeconomic level (61.5%), as well as the majority of smokers passive (39.6%). After 8-week intervention, there was a significant difference before and after the fourth and eighth weeks of intervention on systolic and diastolic blood pressure and anxiety with p = .000. Results of Mann-Whitney statistical test indicated significant differences between the intervention group and the control group in systolic blood pressure (p = .001) and diastolic (p = .000), anxiety (p = .000). Ordinal logistic regression test results found that intervention PMR provides the most powerful influence on systolic and diastolic blood pressure, and anxiety with p = .000. This study recommended the need for PMR exercises for 8 weeks to control blood pressure and anxiety in patients with primary hypertension, and for the Hospital, it was suggested to develop PMR as a part of independent nursing practices.

Abstract

This study aimed to determine the level of work ethics of proficient teacher. This study also sought to propose a Learning Action Cell (LAC) to improve teacher’s work ethics. A cross-sectional descriptive research design used for this study to describe the participant’s work ethic. The researcher employed a purposive sampling to proficient Teachers that have 4-6 years teaching experience in the West District of the Schools Division of Angeles City. The participants asked to complete Meaning and Measurement of Work Ethic (MWEP). Mean used to determine the average score of the overall Work Ethic of the participants. Most of the participant got a moderately high score on work ethics indicating that most of them place a greater emphasis toward their work. Most of the participants got a lowest score on self reliance, leisure, and morality/ ethics. Since most of the participants got a lowest score on self reliance, leisure, and morality/ ethics, these are the focus of the program. Training on professional ethics helps the teacher to direct the complexities of their role and become knowledgeable of potential unexpected consequences of countless decision they made every day. Teachers rarely trained on personality development such as work ethics that helps them to identify ethical issues early enough to come up the potential solution. Despite of teacher’s moderately high score on overall work ethics, still most of them may put less emphasis on work ethics when they become overwhelmed, anxious and bombarded in their work. This study presumes that there is simply too much convincing evidence that schools cannot neglect the need for the development of work ethics training through the provision of Learning Action Cell (LAC).

Made Ida Mulyati Ergonomics of Work Physiology, Medical Faculty, Udayana University Corresponding author: [email protected]

Abstract

Anthropometry is a term derived from Greek, consisting of two words namely Anthropos which means humans and metrin mean measurement. So anthropometry is the science of measuring human dimensions. Bondres mask is folk art, as a means of staging. There are several types of masks used in Bondres art, one of which is used in portraying "the folk". This mask has a characteristic on the mouth, which can be moved in accordance with the role of the "folk" who do a lot of dialogue. So far, the standard used in making masks by I Wayan Tg is only the length and width of the face. Based on interviews with the mask user, discomfort results were obtained in some parts of the face. For this reason, it is necessary to conduct research to determine the comfort of the mask with a descriptive method, through interviews and measurements of the anthropometric dimensions of the face of the bondres mask users. This study involved 20 samples of bondres mask players, selected purposively. The results showed that bondres mask users experienced discomfort in some parts of the face. As a recommendation for the improvement of mask production, it is better to use the anthropometric principle of the wearer's face to provide comfort.

Abstract

Malnourished and poor nutrition are still a health center problem in Indonesia. A good nutritional intake can often not be fulfilled by a child, including due to various factors. Baby massage is a touch therapy that is carried out on baby so that it can provide assurance of ongoing body contact, maintain a feeling of security in the baby and strengthen the parent's love cord with the baby. The purpose of this study was to analyze the effectiveness of baby massage by mothers on improving the nutritional status of baby with malnutrition aged 6-12 months in the City of Mataram, West Nusa Tenggara. The design used for this study was a quasi-randomized pre-test and post-test control group design. Samples in this study were baby aged 6-12 with malnutrition as many as 24 people as a case group (massage interventions and complementary feeding) and 24 people in the control group (Provision of complementary feeding). This research was carried out in health center working area of Mataram city for 3 months. Effective baby massage stimulation improves nutritional status. Increased nutritional status of difference z-score of the first measurement (before intervention) and the measurement of the 3rd month after being given a baby massage in the case group an average of 0.19 ± 0.39 while in the control group had an average z-score of 0, 08 ± 0.56. Analyst results showed a significant difference (P = 0.035) / (P≤0.05) of the nutritional status of baby based on the BB/U index between case group and control group. Stimulation of baby massage is effective in increasing mean IGF-1 levels for malnourished baby aged 6-12 months, namely the average IGF-1 level in the pre-intervention group for baby massage by 1.37 ± 1.00, an increase of 1.19 to 2.57 and showed a significant increase (p = 0.003) or (p <0.05). Whereas in the control group the average IGF-1 pre-treatment level was 3.37 ± 8.15, it increased to 4.29 ± 1.21 and showed an increase of 0.93 and showed a significant increase (p = 0.001) or (p <0.05). The results of the case and control group analysis showed significant differences in mean IGF-1 levels (p = 0.002) or (p <0.05). Thus it can be said that baby massage is effective in increasing nutritional status and IGF-1 in baby aged 6-12 months. It is recommended that baby massage by the baby's mother can be applied as one of the alternative treatments for infancy to improve the nutritional status of baby who are malnourished.

Abstract

Diabetic foot is such a life threatening condition for people with Diabetes Mellitus which it can be result in hemodynamic instability and loss of consciousness. In order to improve quality of nursing care in patient with diabetic foot, nurses should apply a nursing model approach. Levine conservation theoretical model is one of comprehensive model theory that can be applied in patients with critical diabetic foot in Emergency Department (ED). This case study describes a nursing care using Levine's Conservation Model to care for a patient with diabetic foot. This study was a case study with a single case design. Data were collected using physical assessment, written communications with the patient, interviews patient family members, and observing the patient during intensive care in the ED of Lawang General Hospital on December 21th, 2015. Levine's Conservation Model used as the nursing guideline successfully identified patient issues including ineffective breathing pattern as a major priority of energy conservation problem and damage tissue integrity as a problem of structural conservation. Both of conservation problem were caused by patient's personal integrity conservation maladaptive that caused by ineffective therapeutic regimen management. Levine’s conservation model is useful to investigate nursing problem and applicable to solve the emergency condition of damage tissue integrity in patients with diabetic foot.

Abstract

Multiple myeloma is a rare case of cancer, but need specific care because of its poor prognosis. This study aimed to describe nursing practice on multiple myeloma patient using Roy Adaptation Model (RAM) approach in Gatot Soebroto Hospital. This study using a case study approach. Based on the RAM approach, there were six nursing diagnoses identified on the patient: risk of electrolyte imbalance, risk of injury, nutrition imbalance: less than body requirement, fatigue, chronic pain, anxiety and ineffective role performance. Comprehensive nursing care by using RAM was required in the case because of RAM provided a theoretical model for the nursing intervention in the physiological, self-concept, role and interdependency domain. We suggest the family do some care for the cancer patient and facilitate the patient’s new role after diagnosed as a cancer patient. RAM also can be used by the nurses as a nursing care framework for the cancer patient.

Abstract

School-based health promotion is an effort to combine health education and take into account other relevant factors that contribute to foster students’ healthy behaviours as the main prerequisite to create a health promoting school. An alternative to a conventional health education method is a peer-led education. This study aimed to determine the effect of a school-based peer-led education program on healthy behaviours among high school students in a boarding school located in an urban area in Bali, Indonesia. This study was a one group pre-post-tests design with purposive sampling technique conducted on all high school students’ grades nine to twelve in the targeted boarding school. Respondents were requested to complete a self-administered questionnaire before and after the intervention. The questionnaire was adopted from an existing instrument (Health-Promoting Lifestyle Profile II/HPLP-II) consisted of 52 items. A four-week intervention was delivered by peer educators who had completed a ten-day training program given by the research team in collaboration with practitioners from the local health office. The intervention was based on the Pender’s health promotion model. Paired t-test for total HPLP-II scores yielded non-significant result t = -1,967; p = 0.054 (p> 0.05). Physical activity (t = -2.722, p = 0.008) and health responsibility (Z = -4.848, p = 0.008) domains showed significant results (p< 0.05). These findings show that there is no effect of the implementation of the school-based peer-led education program on healthy behaviours among the high school students. Nevertheless, the program has a potential to increase the students’ physical activity level and their responsibility toward their own health. The effectiveness of peer- led education programs can be fostered by putting more attention on target population’s characteristics, intervention methods (e.g. content, duration, strategy, etc.), resources mobilization including facilitating greater involvement from relevant stakeholders, and ensuring the availability of a more conducive environmental that support the adoption of healthy behaviours.

Abstract

The success of diabetes self-care is the most important part to avoid complications. Technology use to support self-care can be a solution, especially when there is distance constraint and limited endocrine experts in an area. This study aims to produce a valid cico count self-care application according to experts. Descriptive quantitative research is used toward the experts to test the content validity of the developed application. The experts amounted to 7 people consisting of 2 endocrine experts, 2 nutrition experts, 2 nursing experts, and 1 psychologist. Data collection tool is in the form of a questionnaire containing statements that are true or false according to respondents (the experts) from each step of self-care activities that researchers had designed. Data analysis was performed by looking at the number of scores or proportions of statements approved by the experts, and then validity test was carried out by determining the value of the content validity index (CVI / content validity index). CVI was done in 2 steps, namely counting individual item (i-CVI) and adding up the total items (s-CVI). The results indicate that the content validity index value calculated based on the average score index validity of all statements (S-CVI) is 0.96. Based on the validity testing requirements according to 7 experts, the CVI value is considered valid since it is more than 0.8. Cico count is declared by the experts as a valid application so that it can be used by children with Type 1 Diabetes Mellitus (T1DM).

Putu Oka Yuli Nurhesti1, I Nyoman Adiputra2 1,2Doctoral Program, Faculty of Medicine, Udayana University Corresponding author: [email protected]

Abstract

Cancer Related Fatigue is a common symptom that occurs in patients with cancer. This symptom is also experienced by cancer patients who receive chemotherapy, radiation therapy, bone marrow transplantation, or other cancer treatments. A survey conducted on 1,569 cancer patients found: fatigue experienced by 80% of cancer patients who received chemotherapy and or radiotherapy. In patients with cancer that has metastasized, the prevalence of fatigue exceeds 75%. Patients who experience healing from cancer report that fatigue is a symptom of a disorder that is experienced for months or even years after cancer treatment is over. Continued fatigue in cancer patients can interfere with the quality of life of patients so that a good assessment and management of this condition is needed. The purpose of this study is to identify the assessment and treatment of fatigue related cancer conducted by health care providers currently in Indonesia. The study was conducted by requesting the participation of health care providers in electronic surveys conducted through their respective sets. One hundred and twelve health care providers in cancer patients consisting of nurses, doctors, nutritionists and physiotherapists participated in this electronic survey. Overall, there is a perception that fatigue related cancer has only been reviewed by one third of health care providers, treatment is only done by a quarter of health care providers and there are no clinical guidelines regarding the management of this condition. Awareness of interventions for fatigue varies among health care providers. There is no definitive clinical services and management of cancer related fatigue. Respondents identified the need for clinical guidelines both for the assessment and management of fatigue related cancer, the need for training and better expertise and access to provide services to cancer patients who are experiencing fatigue.

Abstract

The Philippine government tried its best to design a law for senior citizens while Filipino families took care of their senior citizen member. They designed a law that recognized the contributions of senior citizens in nation building. However, in the report of Dr. De Leon (2014), even though senior citizens have identification card as their single most important document to access their benefits and privileges, what was identified as a gap is the supply side i.e., limited availability and accessibility of health services and medicines, issues related to access of mandated senior citizen benefits. This aimed to describe the level of awareness on the rights, benefits, privileges, and health services of elderly residing in Barangay Dau. A descriptive design used. Self-made questionnaire to capture awareness and availment of elderly rights, benefits and privileges used. Frequency, percentage, mean, and standard deviation used. The result revealed that all of the participants were aware of grant of twenty percent (20%) discount from all establishments and provision of express lanes for senior citizens in all commercial and government establishments and reported they availed it. However, it is noticeable that not all participants are aware of the free medical and dental services in government establishments and the monthly stipend five hundred pesos (Php500.00) for indigent senior citizens. On the other hand, out of 92.5 percent participants who were aware of the continuance of the same benefits and privileges by GSIS, SSS, PAGIBIG, only 53.3 percent availed it.
