The basic contract is for 10 programmed activities but the post holders will also participate in the middle tier emergency surgery resident on-call rota which will attract EPA’s.

The post holder would have the opportunity & support to join the University of Teeside MCh programme

1. JOB SUMMARY

The post advertised is for a Speciality Grade Doctor working in the department of general surgery at Darlington Memorial Hospital & General Hospital. Working as a member of the General Surgery Department Team the successful candidate will provide a high quality service that aims to deliver excellent patient care. Team allocations will rotate to allow exposure to subspecialties including colorectal, upper GI & bariatric, benign hepatobiliary, emergency & breast surgery. These posts offer the opportunity for post-holders to develop/consolidate their surgical skills & experience with direct consultant supervision. Exposure to advanced laparoscopic surgery & gastro-intestinal endoscopy is available. Personnel development opportunity similar to that given to specialist trainees is available & scope of practice can be adjusted depending on previous experience. A named consultant within the department will mentor each post holder.

It is envisaged that it will be increasingly difficult to accommodate all future advances in specialist treatments and interventions at each of our main hospital sites in an NHS dominated by strategic change. Taking this into account it is proposed that whilst a successful candidate will spend most of their programmed activities at Darlington Memorial Hospital it is expected that the role would involve programmed activities at other hospitals within the Trust should this be necessary

3. THE TRUST

On the 1 April 2011, County Durham and Darlington NHS Foundation Trust came together with Community Services to form a new integrated health care organisation delivering integrated acute and community health services to patients committed to providing quality care along the whole pathway; at home, in the community and in hospital.

We provide services from three main hospital sites, six community hospitals, and around 80 locations across the area – plus the care delivered at home.

Our range of services includes hospital based emergency and planned care, including surgery, medical treatment for serious and life-threatening conditions, paediatrics and women services, community based services for long term conditions like diabetes or respiratory illness; and health and well-being services to improve health and prevent illness, such as smoking cessation and alcohol reduction. So now, whatever the health care needs of our community, and wherever they need them in County Durham, our commitment is to provide these to the best standard and quality.

We provide acute services, for the most seriously ill or injured patients through our Accident & Emergency departments at Darlington Memorial and University Hospital of North Durham. Planned patient care, rehabilitation and recovery, and an urgent care centre are provided at Bishop Auckland Hospital. Outpatient and diagnostic facilities are provided at all these sites.

We aim to treat patients as close to home as possible so provide outpatient and community facilities, including inpatients and day care at six community hospitals.

The Trust in its aim to continue to drive the quality of service provided to the community is to develop greater interdependence between our hospitals by creating centres of excellence in a range of specialties on each site, and greater integration between hospital and community services.

Recent developments in this area includes the approval to centralise hyper-acute stroke services on a single site, at University Hospital of North Durham allowing us to improve emergency stroke care during the first 72 hours and the development of a specialist bariatric service at Darlington.

Bishop Auckland Hospital is a centre for planned care for a population of approximately 120,000 people across County Durham and Darlington.

It provides a specialist rehabilitation centre including stroke rehabilitation with high quality facilities, highly skilled nursing staff and experienced therapists. It is a centre of excellence in low-risk hip and knee replacement operations and a full range of outpatient and diagnostics is also provided for local patients.

Further services include a cataract centre and eye care unit delivering a full range of ophthalmology outpatient services including cataract surgery and a midwifery led birth centre.

Darlington Memorial Hospital providing acute services to Darlington and South Durham and some specialised services to North Yorkshire. The hospital has 465 beds and provides a wide range of services; Accident & Emergency, Anaesthetics and ITU, Orthopaedics, Paediatrics, ENT, Ophthalmology, General Surgery, Urology, Maxillo- facial and Oral Surgery, General Medicine, Rheumatology, Obstetrics and Gynaecology and Genito-Urinary Medicine. Clinical Support Departments of Imaging (including PACS digital radiology, CT, MRI and Ultrasound), Coronary Angiography, Medical Physics, Pathology, Pharmacy and Allied Health Professionals are all on site.

Since October 2011, Darlington has provided specialist Bariatric surgery service for people with serious weight problems. With the additional skills of specialist bariatric consultants Mr Akeil Samier & Mr Andrew Mitchell County Durham and Darlington NHS Foundation Trust is able to offer a full range of weight loss surgery at the hospital including Gastric bypass, Sleeve gastrostomy, endoscopic techniques and revisional surgery.

The Memorial Hospital building dates from the 1970s. It serves a population of c125, 000. For ENT and Ophthalmology the population served is c330, 000, covering Darlington, Bishop Auckland and North Yorkshire.

University Hospital of North Durham has 482 beds and provides a wide range of acute specialties including Accident & Emergency, Anaesthetics and ITU, General Medicine, General and Vascular Surgery, Urology (Day case only), Orthopaedics, Obstetrics & Gynaecology, Paediatrics, Plastic Surgery, Rheumatology, Dermatology and Genito-Urinary Medicine. Additional out-patient facilities exist for ENT, Nephrology, Neurology, Radiotherapy/Oncology, Ophthalmology, Oral Surgery and Plastic Surgery. Clinical support departments of Imaging (including PACS digital radiology, CT, MRI and ultrasound), Coronary Angiography, Medical Physics, Pathology, Pharmacy and Allied Health Professions are all on site.

Durham provides a hyper-acute stroke unit, supporting 24/7 emergency stroke care, including administration of thrombolysis where appropriate.

The Hospital was built under the Private Finance Initiative and opened in 2001, replacing the former Dryburn Hospital that has been demolished. It serves a population of c250,000. For Plastic Surgery and Dermatology the population served is c750,000, covering Sunderland and South Tyneside as well.

Chester le Street Hospital is 5 miles north of Durham. It has 1 ward of 24 beds providing rehabilitation services. There is also a day hospital with capacity for 15 patients. Within the bed complement there are up to 5 respite care beds for physically disabled patients. The hospital holds outpatient clinics provided by Consultants from the University Hospital; radiological and ultrasound services on site are linked to the PACS at the University Hospital. The hospital was opened in November 2003 as a Private Finance Initiative funded scheme.

Shotley Bridge Hospital is located one mile from Consett in Shotley Bridge village, 14 miles from Durham City. It provides inpatient facilities for Care of the Elderly, a day hospital for the elderly and a Minor Injuries Unit. There is also a Day Surgery Unit on site. with non-invasive cardiological investigation, Radiology and Ultrasound on site. Inpatient facilities for Mental Health are provided on site by another NHS Trust.

The Trust has received recognition for its services as a whole and the Trust has been named Dr Foster Large Trust of Year by Dr Foster Intelligence in the past. The CGQ rates surgical services ar good. This is a real achievement and recognises the on- going commitment of the organisation to deliver the highest standard of care and treatment to our patients and to consistently improve the overall patient experience at each of our hospital sites.

4 General Surgery Darlington / Bishop Auckland

Emergency services & major elective surgery is provided at the Darlington site. Some day case surgery, outpatient clinics & endoscopy are also provided at Bishop Auckland. The theatre suite has recently been renovated and extended including two dedicated 4K Laparoscopic Theatres.

The consultant workforce is supported by ST/CT trainees, MCh fellows, specialty doctors, foundation doctors & various paramedical professionals. A consultant led emergency rota is provided with a daily emergency operating list. A nurse led clinical decisions unit facilitates prompt assessment, investigation & treatment of admissions from primary care.

Four consultants provide a comprehensive range of upper GI & hepatobiliary service across the trust. The majority of surgery is performed using laparoscopic techniques & all bariatric surgery is performed on the Darlington site. A hot biliary service is established with dedicated theatre space to provide timely access to urgent cholecystectomy during index admission. Colorectal services for malignant & benign conditions are provided at Darlington with a total of nine colorectal consultants in post across the trust. Laparoscopic surgery, early rectal cancer surgery including transanal resection & a full range of benign intervention including for IBD are provided. The colorectal team are supported by a range of nurse specialists within the wider MDT. Weekly MDT’s are supported by regional oncology colleagues & link to the bowel screening programme. The GI surgeons have close links with medical GI colleagues who provide experience of interventional endoscopy including ERCP & colonoscopic EMR. Colleagues with a breast interest have an MDT that offers patients access to image guided surgery & oncoplastic reconstructive techniques.

JOB SUMMARY: Working as a member of the General Surgery Department Team to provide a high quality service that aims to deliver excellent patient care.

The post holder will be expected to manage their own work programme within the broad parameters of department policies and procedures, and will be responsible for determining the associated work activities using their initiative.

Duties of the post  To staff outpatient clinics which will include new, review and urgent referrals  Preparation of patients for theatre and post-operative care.  Dealing with both routine and emergency attenders.  Assisting in theatre and post-operative care.  Dependant on experience will undertake certain surgical procedures and minor operations independently.  Be involved in complex clinical, patient management problems and inter- disciplinary planning treatment.  Participation in duty rota. 1 in 9 as part of the middle grade rota.  Endoscopy- training opportunities as part of JAG accreditation process may be available  A face-to-face handover of care must take place at the start and end of the day, with written and verbal communication about all general surgery inpatients.  Completion of electronic discharge summaries is mandatory for all patients on the day of discharge  Help develop new services where needed.  Be expected to take an active part in continuing medical education and undergraduate teaching.  Full engagement with Clinical Audit and Clinic Governance is required.

6. TERMS AND CONDITIONS OF SERVICE

1. The post is subject to the Terms and Conditions – of CDDFT as amended from time to time.

2. The successful applicant will be required to travel independently within CDDFT footprint. The postholder must hold a valid car licence and have use of a car for business purposes.

3. The current salary applicable to the post is £39,060 to per annum, rising through pay thresholds to £72,840 per annum. Starting salary will be dependent on prior NHS experience.

4. For pre-employment health assessment purposes, as part of the Trust’s Occupational Health Service, the successful candidate will be required to complete a health questionnaire. This will be treated in the strictest confidence and will not be seen by any employee of the County Durham and Darlington NHS Foundation Trust, other than Occupational Health Staff. The successful applicant may be required to undergo a medical examination, and any offer of the post is subject to medical clearance from the Trust’s Occupational Health Physician. The appointee may be required to undergo any future medical examinations considered necessary by the Trust.

Darlington and Durham have main line east coast stations with direct access to London in approximately 2 hours 30 minutes.

Surrounding Area

Not only is County Durham and Darlington an interesting place, but it is an ideal base from which to explore and enjoy the regions many tourist attractions.

Darlington provides easy access to the North Yorkshire Moors and the Yorkshire Dales, both offering some truly remarkable scenery and unspoilt countryside. Hamsterley Forest and High Force are also within easy travelling distance as is the Northumbria National Park.
