Drugs from this class alter the functioning of the serotonin system by the potentiation of serotonin through the negative allosteric modulation of its neuronal uptake by the human serotonin transporter. Selective serotonin reuptake inhibitors show few side eff ects compared to those caused by traditional antidepressants and they vary in the binding interactions formed during binding. Generally, their binding involves three specifi c regions of the drug structures, each participating in vital interactions, such as salt bridge formation and additional hydrophobic interactions with conserved residues in the central binding site of the target protein. Side eff ects, however, such as the initial lack of response to treatment, or drowsiness, nausea, and sexual dysfunction occasionally may arise. Additional binding studies, furthermore, highlighted the importance of enantioselectivity in the binding of these compounds, raising concerns about the benefi cial application of racemate mixtures of some of these compounds. -

Emerging Drug List CANADIAN COORDINATING OFFICE FOR HEALTH ESCITALOPRAM TECHNOLOGY ASSESSMENT NO. 35 JANUARY 2003 Generic (Trade Name): Escitalopram (LexaproTM) Manufacturer: Forest Laboratories, Inc. Indication: For the treatment of major depressive disorders in adults.1 Current Regulatory Escitalopram was approved by the US FDA for the above indication in August 2002.2 It Status: was just recently launched in the US, according to the company's web site.3 It was launched in June 2002 in the UK under the trade name Cipralex by Lundbeck for both the treatment of depression and panic disorder.4 In Canada, escitalopram has been sub- mitted for review; no planned marketing date is available (Drug Information, Lundbeck Canada, Montreal: personal communication, 2002 Sep 30). Description: Escitalopram is the S(+) enantiomer of the chiral compound citalopram. It is a selective serotonin reuptake inhibitor that exerts activity in both depressive and anxiety disorders. After oral administration of a single dose, maximal concentrations are reached within three to four hours. It is extensively metabolized via the liver, with eight percent excreted unchanged by the kidneys. SUMMARY OF PRODUCT CHARACTERISTICS 1. NAME OF THE MEDICINAL PRODUCT Deanxit film-coated tablets 2. QUALITATIVE AND QUANTITATIVE COMPOSITION Each tablet contains: Flupentixol 0.5 mg (as 0.584 mg flupentixol dihydrochloride) Melitracen 10 mg (as 11.25 mg melitracen hydrochloride) Excipients with known effect: Lactose monohydrate For the full list of excipients, see section 6.1. 3. PHARMACEUTICAL FORM Film-coated tablets. Round, biconvex, cyclamen, film-coated tablets. 4. CLINICAL PARTICULARS 4.1 Therapeutic indications Anxiety Depression Asthenia. Neurasthenia. Psychogenic depression. Depressive neuroses. Masked depression. Psychosomatic affections accompanied by anxiety and apathy. Menopausal depressions. Dysphoria and depression in alcoholics and drug-addicts. 4.2 Posology and method of administration Posology Adults Usually 2 tablets daily: morning and noon. In severe cases the morning dose may be increased to 2 tablets. The maximum dose is 4 tablets daily. Older people (> 65 years) 1 tablet in the morning. In severe cases 1 tablet in the morning and 1 at noon. Maintenance dose: Usually 1 tablet in the morning. Shen, M.D.1, 2* Background: Trazodone in clinical use is ineffective because of its sed ation to let patient receive an adequate dose for treating a patient with major depressive disorder. We are report a case report to demonstrate this clinical effi cacy issue in the treatment with trazodone. Case Report: A 83-year old male Taiwanese patient had suffered from MDD for six months. He did not response to the treatment of trazodone 150 mg/day for four months, but consequently, he responded to the treatment of milnacipran 100 mg/day or mirtazapine 30 mg/day. Conclusion: Psy- chiatrists in Taiwan should be alert to the fact that trazodone is not as effective as other antidepressants, and that trazodone should not be prescribed as a single anti- depressant in treating an MDD patient.
