Introduction

Timmins Martelle Heritage Consultants Inc. (TMHC) was contracted by the Ontario Ministry of Community and Social Services (MCSS), through Infrastructure Ontario (IO), to carry out a third party review of records pertaining to the cemetery at the Huronia Regional Centre. This work was commissioned as part of a larger effort to commemorate and better document the burials made within the institutional cemetery, between the time of its opening to the last burial made in 1971.

TMHC was provided with sets of original records pertaining to the cemetery, many of which are on file with the Ontario Archives.

The following sections of this report describe the general methodology used during the third party records review, offer a thorough discussion of the source material and issues identified therein and provide recommendations for consideration.

General Methodology

The general methodology used during this review was as follows:

1) The existing cemetery register compiled by MCSS (available on the website) was entered into a master database to facilitate comparison with the individual data sources for review.

2) The information from each data source for each individual within the cemetery register was entered into the master database and checked for errors and consistency.

3) The information from each data source was compared to other data sources to check for consistency and errors.

4) General summaries were made for each data source identifying their content, issues, problems and errors identified.

5) All errors and problems that were identified were inventoried on the database, a corrections file to update the registry and in the text of this report.

6) Quality control checks were undertaken for each data source using the master database. This allowed for the identification and/or correction of additional problems with the data sources and those in TMHC data entry when compiling the master database.

Numerous electronic data files were created during this process to help with the reading of this report, to provide a source for cross-checking data in the future and to establish the baseline data used to suggest changes to the existing cemetery register and propose recommendations.

Discussion and Review of Source Material

For this study, a set of original source documents containing information regarding interments made at the HRC cemetery were reviewed. Cumulatively, these records show a progressive attempt to formally and efficiently track interments in the cemetery and details of those interments. Additional information is added over time to improve tracking procedures and record keeping. This set of source documents represents a fairly good record for a cemetery originating in the late- 19th and early-20th centuries.

The following sections provided a detailed discussion and review of all original source documents provided for review in this study. For each original source, a summary of content is provided alongside an identification of issues encountered and, where possible and necessary, a list of steps taken to mitigate the issues. In the event that the information provided in one data source was unclear, the same information was consulted in other data sources and death records were reviewed, when these were available. For example, when it seemed as if the name on the MCSS cemetery registry did not match that in the alphabetical index, the name was entered into Ancestry.ca and an attempt to obtain and review the original death record for Simcoe County was made. When the Simcoe death record showed a name different to that in the MCSS cemetery registry, a copy of the Simcoe death record was made and an indication in red placed on the spread sheet to indicate where a change was required. In instances where individuals were unintentionally excluded from the MCSS cemetery registry despite being listed as being buried in the institutional cemetery they were added to the registry master list. When individuals who were unintentionally included on the MCSS cemetery registry despite being listed as being buried elsewhere, their death records were looked up on Ancestry.ca to confirm place of burial if listed. Individuals whose documentation suggests that their burial was elsewhere were recommended to be removed from the MCSS cemetery registry. In sum, an attempt was made to confirm information with an outside source, where this was immediately possible.

I) Source: Volume 7 Part I 1876-1916 – Alphabetical Listing of Deaths During the Period

Summary of Content

This document represents an alphabetized index of patients who died at the facility. Within the master record, there are individual tabs organized by letter. In other words, all individuals with last names beginning in A are found listed under the tab marked “A.” Under the tab for each letter is a listing of individual names (generally but not always last name first, given name second) handwritten in cursive, followed by a column for death registry number. The sequence of the listing for each tab is generally organized by death registration number and is consequently chronological, with the individuals who died early in the history of the institution listed first and the more recent deaths listed last. This was a document that was updated by the institution as deaths occurred, so entries were made by a number of different individuals, as different hand writing is evident throughout the chronological sequence.

The document is archived as beginning in 1876 and ending in 1916.

This document does not provide dates of death or place of interment.

For the purposes of tracking, we numbered the individual pages of this document. There are 29 pages, numbered 1 to 20 and 22 to 30. Page 21 (the letter “O”) is missing. When the names on the Cemetery Registry were checked against this document, the relevant source document and page were listed in the spreadsheet. The following links provide access to the individual pages this document.

Issues with Interpretation and Other Issues to Note

There are several issues with this document that lend to difficulties with interpretation of entries or demonstrate inaccuracies:

1) The earliest entry is for death registration number 1, Charles Rowan, who died on November 3, 1876. The latest entry is for registration number 1445 (mislabeled 1446) – Redford G. Dell who died on May 19, 1917. Therefore, there are several entries that post-date 1917. The correct temporal span should be Nov 3. 1876 to May 19, 1917.

2) The document is written in cursive and the spelling of names is subject to interpretation. There are many places where names are illegible or difficult to establish correct spelling for. The death registration numbers are also difficult to read or decipher in spots, with difficulties experienced differentiating certain numbers from others (e.g., is that a curly 5 or an 8?).

3) Many short forms are used for first names and these can often lead to misreading of the names.

4) The spelling of many names is different from that used in the register of deaths.

5) Certain names are struck through with a single line. The reason for this is not always clear, although sometimes it is a result of placing the name under the wrong table (e.g., Roland Charles Hill was recorded under the A tab and his name was later struck out).

6) There are numerous places where ink blots obscure the written information provided.

7) Some entries are “squeezed” in between normally spaced entries, making some illegible.

8) On some pages, tape was used to mend ripped pages and this obscures the written information.

9) Entries for the “O” tab (presumably page 21) are not recorded. It is not clear if there is a page missing in the document or, in fact, no names with the last name O recorded.

10) Although entries were generally organized by death registration number (i.e., chronological), there were a few places where this ordering was not kept. This generally occurred when there was more than one page of names under a single alphabetical tab. When new entries were made, there were not necessarily made on the correct page.

11) There are several entries inserted on the wrong page. This usually occurred when last and first names were reversed, so the entry was made under the first letter of the first name rather than the first letter of the last name. For example, Arnold William (1262) is indexed under the A tab rather than the W.

12) Many entries with 700 series death registration numbers are not recorded in this index. This suggests that there were periods of time when the index was not used. The entries that are not recorded are sometimes in contiguous chronological blocks.

13) In total, 35 entries are not recorded in this index, including two individuals with a last name starting with the letter “O” (page missing):

Summary of Content

This document is the Register of Deaths from the Huronia Regional Centre between November 3, 1876 (Charles Rowan, Patient Registration No. 9, Death Registration No. 1) to December 14, 1904 (May Young, Patient Registration No. 1759, Death Registration No. 860).

The document contains five columns entitled:

1) No. (i.e., patient registration number); 2) Name (first, last); 3) Date (i.e., date of death); 4) County ( or place of origin – e.g., London Asylum, Middlesex, Lanark, Muskoka); and 5) Cause of death.

In addition, another separate set of entries was made to the left of the No. column, reflecting a cumulative tally of entries that are known to be Death Registration numbers.

The information provided in this register is written in cursive. There are separate subheadings for year and to signify when a new year begins.

There is no information provided on place of burial.

The first page in the document (cumulatively Page 31) contains some unconsolidated written notes that are not relevant to the interpretation of the registry entries. The second page in the document (cumulatively Page 32) is a repeat of Page 1 of the alphabetical index but shows the short forms used later in the registry to list undertaker. The list is as follows: S = Seventon (?) & Son; C = Clark; B = Bingham (& Strachan); P = Private Burial; M = Mundell.

This document contains 18 pages. For tracking purposes, these were numbered pages 31 through 48. The first two pages (numbered 31 and 32) contain the reference notes described above and do not contain register entries. The links below lead to the 18 pages referenced in this document.

2) There is variability in how names were recorded between this document and the alphabetized index. Some names are given in one document with initials for the first name only, while others have written out the full names of each person.

3) There are numerous places where ink blots obscure the written information provided.

4) Some entries are “squeezed” in between normally spaced entries, making some illegible.

5) Abbreviations or short forms for many 19th century first names are often used (e.g., Jno. For John, Elizth for Elizabeth, lending some difficulty in reading first names written in cursive. These have sometimes been misinterpreted in the MCSS cemetery registry.

III) Source: Volume 7 Part 3 1905-1916 - Record of Deaths

Summary of Content

This document is the Register of Deaths from the Huronia Regional Centre between January 7, 1905 (Donald Campbell, Patient Registration No. 1775, Death Registration No. 861) to May 19, 1916 (Redford G. Dell, Patient Registration No. 2547, Death Registration No. 1445).

The document contains five columns entitled:

1) No. (i.e., patient registration number); 2) Name (first, last); 3) Date (i.e., date of death); 4) County ( or place of origin – e.g., London Asylum, Middlesex, Lanark, Mukoka); and 5) Cause of death.

In addition, there are separate secondary columns of information that have been added:

1) another separate set of entries was made to the left of the No. column, reflecting a cumulative tally of entries (i.e., death registration numbers); 2) an initial occurs to the left of the date column (e.g., N, B, W, C); what these letters represent is not known; 3) a separate entry occurs left of the Cause of Death column for “age at death”; the ages are entered for each person in very small numbers that are not always legible. 4) An entry of hyphenated numbers (e.g., 4-4-18, sometimes written in the format 4:4:18) occurs to the right of the Cause of Death column; what these numbers represent is not known.

Beginning in 1911 there are some individuals who also have hand written entries beside their name indicating a dollar amount, presumably cost for burial or delivery of the body. This cannot be confirmed.

By 1912 and 1913, some entries also have a check mark after the entry under the column County and the letter W. It is not known what this makes reference to.

In 1913 another column has also been written in for describing Religion (Quaker, Anglican, Methodist, Roman Catholic, Presbyterian, etc.).

By 1913 another column has also been written in to define marital status (married, single, divorced, windowed).

By late 1914, a new column has been added with no title. It contains the entries “P.C”, “C,” “Inst.” “Inst Cem”, “P.M.” amongst others and would appear to indicate where burial was made, but the exact meaning of the notations is unknown.

The information provided in this register is written in cursive. There are separate subheadings for year and to signify when a new year begins.

6) Once the location of burial or undertaker is recorded it is easy to track who is recorded as buried in the institutional cemetery. Based on the records and their interpretation, a small number of individuals who are listed as buried in the institutional cemetery are not listed in the MCSS cemetery registry that has been created. Also, several individuals who are in the MCSS cemetery registry are shown on the register of deaths as being buried elsewhere and by non-institutional undertakers.

IV) Source: Volume 9 Part 1 1916-1919 – Record of Deaths

Summary of Content

The first page of this document lists deaths that occurred of patients from the Whitby Psychiatric institution who were temporarily in residence at the Orillia facility from February 1917. It includes a hand written table with the following columns:

1) No. of death; 2) Name; 3) Date of death; 4) Registration Number; and 5) Burial (i.e., burial place).

For the purposes of recording, this page is recorded as Page 1 of Volume 9 Part 1 1916-1919.

Opposite to this table are some unconsolidated hand written notes. One indicates P = private burial. Two other sets of writing indicate yearly tallies of burials made by individual undertakers (e.g., Clark = 3; Strachan = 2; Mundell = 11).

The next 8 pages of this document is a Record of Deaths with the following columns and headings:

1) Register Number (i.e., Patient Registration No.); 2) Name; 3) Date of Death; 4) Age; 5) County; 6) Time In (time in facility; expressed in Y-M-D years, months, days); 7) War. Or Cert. (which is believed to refer to admission documents); 8) Annual No. of Autopsy; 9) Cause of Death; 10) Mental State; 11) Type; 12) Religion; 13) Undertaker (and if private mark “P”); 14) M or S (meaning married or single); 15) Annual (death) No. (columns for male, female and total – tallies for year); and 16) Number Since Opening (i.e., death registration number).

The term “Inst.” under the column “Undertaker” indicates that the individual was buried in the Institutional Cemetery.

The information provided in this register is written in cursive. The year of death is written at the top of the column. The legibility of the entries is far improved over earlier entries.

On Page 6, for patient 2244 – Arthur Wilson, there is a hand printed note above indicating “not in computer.” Between the entry for patient 1863 Norman McCuaig and 2764 James Fullerton, there is a written note indicating “Floyd Badour” but no patient numbers are associated with it.

6) A small number of individuals in this listing are shown as being buried elsewhere yet their names are included on the cemetery registry. To confirm place of burial, their municipal death records were searched on Ancestry.ca.

This document consists of 11 pages of entries in the Record of Deaths. The Record contains the following columns and headings:

Cemetery where we had ???? to pay a private undertaker.” The undertaker is listed as Doolittle, while the Hospital Cemetery is also listed under the undertaker column, indicating place of burial.

9) Beginning sometime in 1948, the recorder begins to use a new system for recording undertaker and place of burial. In the top corner in each box under the undertaker column is the letter I or P indicating Institutional or private burial, respectively. Across the main portion of the box, the name of the undertaker is written. This suggests that around 1948 the institution began to hire

The term “Inst.” under the column “Undertaker” indicates that the individual was buried in the Institutional Cemetery.

The information provided in this register is written in cursive. The year of death is written at the top of the column. The entries are fairly legible.

As was the case beginning in 1940, the majority of columns were not filled in by the registry recorder. Only the patient registration number, name, date of death, undertaker/place of death, cumulative tallies of deaths and death registration number were recorded.

Beginning in May of 1958, only those individuals buried in the cemetery are listed on this register. The note indicates “as of this date only Institution Burials.”

Beginning in July 1959, dates of birth were also recorded. Notes are included where relatives paid for burial plots and stones. Indications where made when the burials represented stillborn babies. At the end of the list, there is a note indicating which is the last burial with a grave stone, that being Renee Silver (death registration 4246, date of death March 7, 1971). The last 26 entries list details of burial locations elsewhere and not in the institutional burial ground.

The entries begin with Patient 11108 Paul Andrews, Death Registration No. 3541 (DoD May 25, 1957) and end with Patient 16351 George Roworth, Death Registration No. not listed (DoD Dec 9, 1975). For

Issues with Interpretation and Other Issues to Note

There are several issues with this document that lend to difficulties with interpretation of entries:

1) The document is written in cursive and the spelling of names is subject to interpretation. There are many places where names are illegible or difficult to establish correct spelling for. Generally this information is easier to see and interpret than the early register and the alphabetical index.

2) A few are “squeezed” in between other entries, giving the impression that they were originally missed. This suggests that the record of deaths was potentially not a “current” record. That is, entries were made to record longer sequences of death and not as each individual death occurred. There are other places on the register where the consistency of the hand writing suggests the same thing. This also raises the question as to whether this was an original record or the information was duplicated at a later date from an earlier record or a different source.

3) Some entries are struck through with a single line. The reason or this is not known.

4) A few entries on Page 92 and 93 have been struck out and corrected.

5) Note that Robert McCauley (Page 99) has a death registration of 5868 listed. This is likely his patient registration number as it does not match the sequence of death registration numbers listed for subsequent and previous entries.

XIV) Source: Cemetery Stone Data Collected by TMHC in 2014

Summary of Content

As part of the GPR study, TMHC mapped and recorded the location of existing and visible grave markers within the cemetery. General information about each stone (i.e., size, death registration number and/or name and date of death) was recorded through either photo-documentation or compilation in an excel spreadsheet, which also recorded the relative placement of individual stones by row. In the field, each row was given a number, as shown on Map 10 in the cemetery investigation report. This data was collected to provide some general guidance to TMHC during the evaluation of the GPR data and to assist us in gaining a better understanding of cemetery organization. It was not collected as part of this third party records review.

There are generally four classes of grave markers within the cemetery:

a) Flat concrete stones (roughly 25 cm by 10 m wide) stamped with Death Registration numbers; these date prior to 1958 and were designed/installed by the institution; there are currently seven rows of these stones in the west end and old portion of the cemetery, with stones oriented north-south, marking graves oriented east-west in typical Christian tradition. Additional stones of this type occur in the west end of the new (north) cemetery area, in TMHC mapped rows 8, 9 and 10 (south end of row).

b) Flat granite stones (varying sizes with many 25 cm by 53 cm) inscribed with names and year of birth and death; these date 1958 and after and were designed/installed by the institution; these are generally concentrated in TMHC mapped rows 10 (north end of row), through 17.

c) Personalized individual stones (either flat or standing) added to or set to replace the institutional markers; these were presumably designed and placed by family members; these are scattered throughout the new and old sections of the cemetery.

d) Formerly standing long, narrow hand inscribed numbered stones set, as a group, in a concrete slab in the northeast portion of the new part of the cemetery; presumably, these formerly marked graves in the old, south portion of the cemetery, east of the rows currently still marked with stones.

The excel spreadsheet file recording our observations of grave stones in the field has been provided as a separate file. Photographs of individual stones can also be provided upon request, although many of these do not provide sufficient clarity with respect to stone numbers as they were overgrown.

Issues with Interpretation and Other Issues to Note

The following are the issues noted with respect to the field grave marker data:

1) Many of the stamped and numbered markers were difficult to read in the field (and sometimes partially broken away) and, in some cases, a best guess was made of what number was represented. Some were also partially grown over at the time of recording, obscuring the numbering.

2) The stones set in the concrete slab are hand inscribed and some were difficult to read/interpret.

3) It was deemed possible that some of the flat earlier stones may have also grown over and were not visible in the field.

4) With respect to the stones set in the concrete slab, it is important to note that these have not been placed in any real order. These stones generally represent the earliest burials in the cemetery. The earliest burial represented in the slab is registered death 1363, who was Charles Brown who died on March 16, 1916. The most recent burial represented is that related to registered death 2318, who was Jean Chute who died on November 1, 1932. With the exception of the small number of stones set in the concrete slab, markers are generally absent for burials pre-dating November 1934. Stones are generally present for burials beginning May 23, 1934.

5) For the stones that are present in the field, these are generally in chronological and sequential order, but there are a few places where the ordering is not precisely maintained. The reason for this is not known.

6) There are no entries on any cemetery map for the four most westerly stone rows (TMHC mapped rows 1 to 4).

7) TMHC Map 10 row 5 corresponds to the Ontario Hospital School Orillia Cemetery Plots Map row 27. There are variances between the cemetery map and the stones in the field. First, there are no stones for 2637 (George Sauve) and 2634 (Reginald Lyons). Second, there is a stone for 2583 (Harold Dupre) at the end of this row. Harold has a personalized stone in the south end of TMHC mapped row 6 of stones, where the old cemetery map shows his burial place to be, once the note of the “reversal of entries is considered).

8) TMHC Map 10 row 6 corresponds to the Ontario Hospital School Orillia Cemetery Plots Map row 26. Between stone 2499 and 2490 is another stone, although the cemetery map does not show a burial between the two. There are not entries for institutional burial between 2499 and 2490 in the register of deaths. Also in this stone row, two stones (2482 and 2486) are in reverse order to what is shown on the cemetery map.

9) TMHC Map 10 row 7 corresponds to the Ontario Hospital School Orillia Cemetery Plots Map row 25. TMHC did not observe a stone for 2417 (Frederick Johnston) at the beginning (south end) of the row. The cemetery maps his burial at the beginning of the row. Also, the first few entries after

this are a bit blurred, so we’ve made interpretations about these numbers, although these may not be correct. Between stone 2402 and 2398 we observed a stone marked 2401 (William Dodds) or 2407 (Henrietta Bennett). The cemetery map says 2407 should be in this location however, the person related to the number 2407 (Henrietta Bennett) did not receive an institutional burial. William Dodds (2401) was buried in the institutional cemetery, so it is likely caused by an issue with the transcription of the number on the map. Between stones 2375 and 2372 at the north end of this row, TMHC observed a stone marked 2374. This stone would relate to Sarah Boyington, who is credited with an institutional burial. The cemetery map indicates 2373 in this location (Annie Dale). According to the death register, Annie Dale received a private burial by undertaker Mundell and was not interred in the institution cemetery and a municipal death record confirms this.

10) TMHC Map 10 row 8, which correlates to the HRC Cemetery Map 1953-1971 map row 1 (most westerly), matches the map data. TMHC Map 10 row 13, which correlates to the HRC Cemetery Map 1953-1971 map Row 6 (6 most westerly), matches the map data. TMHC Map 10 row 15, which correlates to the HRC Cemetery Map 1953-1971 map row 8 (8th most westerly) matches the map data. TMHC Map 10 row 16, which correlates to the HRC Cemetery Map 1953-1971 map row 9 (9th most westerly), matches the map data. TMHC Map 10 row 10, which correlates to the HRC Cemetery Map 1953-1971 map row 17 (most easterly), matches the map data.

11) TMHC Map 10 row 9, which correlates the HRC Cemetery Map 1953-1971 map row 2 (second most westerly), shows one discrepancy. In the field, we mapped stones in order 3405-3414- 3410, with 3414 also having a personalized stone for Anne Florence Leslie. On the cemetery map the order is shown as 3405-3410-3414. In the central portion of this row, many of the burial entries are inconsistent with the death register as far as the registration numbers or on the inscription of names on the cemetery map. These errors are discussed in more detail when the source information for the cemetery map is provided. The correct order for stones should be (south to north): 3396- 3398-3399-3402-3405-3414-3410-3422-3426-3429-3432-3435-3440- 3446-3448-3449-3458-3462-3464-3470-3477-3480-3483. Therefore the stone marked 3431 in the field is incorrect and should be 3432.

12) TMHC Map 10 row 10, which correlates to the HRC Cemetery Map 1953-1971 map row 3 (third most westerly), generally matches the cemetery map. However, the cemetery map entry for entry 3621 “Fauno” should be “Tauno.”

13) The TMHC Map 10 rows 10 to 17 contain stones with names rather than numbers or a combination thereof. There are a few places where spelling varies between the map, the registry and the stone. Those are detailed in the previous excel sheets already provided.

14) TMHC Map 10 row 12 generally corresponds to the details of the HRC Cemetery Map 1953-1971 map. Between John Shaw and Frank Weber should be the burial of Ronald Luciow (3821). This burial is not represented in the field with a stone. Also, it is important to note that the burial of

Angelina Rizzo (3879) who was disinterred in recent years appears on the cemetery map but a stone no longer stands in the field.

15) TMHC Map 10 row 14, corresponding to HRC Cemetery Map 1953-1971 map row 7 (7th most westerly row), generally matches the details of the map with a few exceptions. The stone for Florence McCready occurs at the beginning of the row in the field, although her entry does not occur in the beginning of the row on the map. Her burial was omitted from the sequence and is one of two shown out of order on the right hand side of the cemetery map. So, this is interpreted as a mapping recording error. Also, there is stone in this row for Fred Corby, immediately beneath that for James Corby. Fred Corby died in 2000 (not at HRC) and his ashes were placed next to his brother’s grave at the request of family.

16) Since many of the stones marking earlier burials are no longer present in the burial grounds, a significant number of known burials are not represented by stones in the field. The table below provides a listing. However, the plot map can assist in determining the approximate location of many of the burials.

The TMHC inventory of stones was compared to the Record of Death listings, cemetery maps and the MCSS cemetery registry. The data was checked for consistency and changes in the inventory were made when inscription and interpretation errors were recognized.

Issues with Interpretation and Other Issues to Note

The following are the issues noted with respect to this source:

1) The register contains death registration numbers in the left hand column, with names provided for only a smaller number of entries. The information is hand written and there are places where it is difficult to read names and numbers.

2) The note difference in “boys” and “males” in the listings could suggest that adults were buried separately from youngsters. An examination of age at death would be needed to confirm.

3) In some cases, short forms and ‘nicknames” were used to record names, which may suggest some familiarity with patients by the recorder. In some cases only the last name is recorded, while in others the full name is recorded. There are places where the spelling in this record (which seems far more informal) is different from that in the formal record of death.

4) Entries for John Fulton and Davie McLeod were recorded by patient registration numbers rather than death registration numbers.

5) There are instances where death registration numbers associated with female patients occur for rows marked male and vice versa. This suggests that the registration numbers may be incorrect, or the recording system not that accurate.

6) The entry for 699 is particularly odd since it occurs under the listing for female burials and the name is recorded as Aida Hall. However, municipal death records and the record of death indicate that this individual is male and the correct name is Amos Hall. Again, this may question the accuracy of this set of informal records during this early period when staff at the institution were attempting to create a record keeping system.

7) For the most part, it is recommended that individuals who are listed in this registry be added to the MCSS cemetery register as potential burials within the institutional cemetery. Many individuals in this listing are already on the MCSS registry and have confirmation of institutional burial by some other means (e.g., cemetery map). In other cases, these death registration numbers do not have associated graves marked on the cemetery map and therefore confirmation of institutional burial cannot be arrived at. Municipal death records available for review for this period do not specify place of burial.

8) There are several duplicate entries for the same registration number, sometimes with the same entry made for a “male row” and “female row.” This, again, suggests this informal system of record keeping was not that accurate.

9) The correct spelling of some names cannot be established from the records examined and municipal death records that were available for review.

10) The earliest entry in this listing is from August, 1899 while the most recent is from July of 1903. It is not a continuous listing of burials made through this entire period, as established from other sources. It seems possible that these are a small subset of pages from a once larger document that no longer exists.

XVI) Source: Ontario Hospital School Orillia Cemetery Plots Map (Archives of Ontario RG 29-24- 3-94) – Two Versions

Summary of Content

This is a map of interments within the oldest section of the institutional cemetery. It is titled “Ontario Hospital School Orillia” and has a notation that the “Cemetery Plot Scale” equals 1 “ – 10 ‘ (although the scale does not appear to be accurate).

The map shows an empty central corridor extending north-south in through a gate in the south fence line. To the east of the corridor, 12 rows of grave plots are illustrated number row 1 to 12, with the lowest row numbers in the east and the highest in the west, adjacent to the central corridor. On the original map, to the west of the corridor are 13 rows of graves numbered 13 through 25, with the lowest numbered graves being in the east and closest to the corridor, with the highest numbered rows in the west. The second version of this map shows two additional rows, numbered 26 and 27, further west of row 25. The latter two rows are written in with a different script, again suggesting another authorship for the new information. Based on fieldstones observed on the ground, additional rows were added further west, although these do not appear on this or any other cemetery map.

The cemetery map depicts individual plots or graves containing death registration numbers. The spacing of the individual graves seems to vary according to how many graves were in each row. It is not known if this is a reflection of burial spacing in the field and one could argue that the map is only meant to be a schematic of the relative sequencing of individual burial sites.

Some general observations on this map:

1) The original version appears to have been prepared in one sitting or at least by the same individual. It is most likely that it was authored at the same time that the outermost rows were in place (the most recent being row 25) and therefore likely circa 1936. That it was not a cumulative map (a single document that was added to as individual graves were placed in the cemetery), seems obvious from the consistency in the hand writing and the generally organized state of the map. If this is the case, the death registration numbers would have been transcribed from an original document(s) of records held elsewhere which have not been located. That this is the case is also supported by the fact that the earliest and original cemetery rows (those located adjacent to the corridor in the centre of the cemetery) are not numbered 1 and 2, for example, but instead 12 and 13. This suggests that the author of the map simple began numbering rows at the easternmost extent of the cemetery and added to the sequence moving westward.

2) The earliest burials within the cemetery were placed on either side of the central corridor, with a general pattern of placing females on the west side of the corridor and males to the east. This type of gender segregation would have been typical for institutional care of the time. The cemetery records generally support this pattern, although there are small

inconsistencies here and there, which raise questions about whether or not the numbers entered for the individual graves are correct.

3) The amended map shows a large gate along the southern cemetery fenceline near rows 26 and 27 as illustrated.

4) A note on the amended version of the map indicates that for row 26, the order of burials has been reversed. It should be noted that for row 26 and 27, no individual burial plots are shown, only a relative listing of numbers.

5) The pattern of segregating male and female burials seems to have been generally adhered to (notwithstanding some of the problematic entries described above and itemized in detail below) until around 1931. At that time, it would appear that the cemetery could not be expanded further to the east, where it then ended at the hedgerow identified in the field, west of the current gate and former laneway. Row 22 in the west is the last of the female specific rows. Row 23 (largely beginning 1932) is a row of mixed males and females, who appear to have been buried in a more-or-less chronological order. Generally speaking (except for the odd entry that is out of chronological order), the burials are chronological from north to south. Other rows are in order from north to south. This would suggest that the undertakers simply continued burials in the same direction until a row was full and then moved to the next row, without maintaining a consistent directional order. A practice of adding individual burials (out of order) along the ends of rows was also noted in the record. This may have also indicated that they were running out of row space within the original cemetery area.

6) One would be challenged to establish a rationale for the odd burial that is out of chronological order. It may be that entries were initially omitted and added later, that there was some liberty taken by the grave diggers, or that the author of the map simply meant for the map to be a generalized depiction of individuals placed in each row rather than a detailed depiction of specific grave locations. Any attempt to map individual grave sites by this map should be made with caution, as it is not entirely clear that the author planned to precisely map individual grave locations.

7) While the map appears to be a fairly accurate reflection of relative location of burial rows, limits of the cemetery and number of individuals buried prior to 1941, it otherwise contains a number of inconsistencies when matching the number on the map and the location of the grave to the name in the Register of Deaths, as depicted by duplicate and triplicate entries. Also, many of the numbers are illegible and require some degree of interpretation.

Issues with Interpretation and Other Issues to Note

The following are the issues noted with respect to this source:

1) There are several duplicate entries across the map, the majority of which are not clearly explained. In some cases, it seems likely that the numbers are off by one digit and have incorporated the same misnumbering that was observed (but later corrected) in the registry/record of deaths. For instance, if an original registry of death entry was for registration number 706 and later corrected to 707, the map maintains the 706 number. This likely represents the fact that a separate set of records or inventory was made upon burial using the incorrect registration number, with the correction only being made to the official register, once the error was recognized in the bookwork. The following is a list of duplicate or triplicate entries that could be recognized:

2) There is a grave marked 775 in row 13 plot 15. This is a curious entry for a number of reasons. First, the death registration number is associated with James Park. This would represent a male burial in an otherwise female row. Second, in the register of deaths there is a note associated with James Park that indicates he received a “Private” burial. This would seem to suggest he is not buried in the institutional cemetery. The municipal death record that was accessed online for him does not indicate place of death. The burial of Park in this area and in the cemetery seems unlikely and therefore it is recommended to leave him off the register for this reason.

3) There is a grave marked 1313 in Row 17 plot 10. This death registration number is attributed to Cora Thompson who died September 14, 1914. The death registry of the time does not indicate place of burial, nor does the municipal death record. Based on the map and the chronological placement of this grave amongst others in 1914, it seems likely that she was buried in the institutional cemetery. Therefore, she should be added to the cemetery registry.

4) A burial for 1654 occurs in row 4 plot 22. This is attributable to Roydon George Lucas. According to the record of death registry and municipal death record he was not buried at the institution but instead in Goderich. He should be removed from cemetery registry.

5) A burial for 1329 occurs in row 7 plot 16. This is attributable to Charles Graham. According to the register of deaths he did not receive an institutional burial and municipal death records indicate he was buried in Lakefield, ON. He should be removed from cemetery registry.

6) A burial for 1358 occurs in row 7 plot 11. This is attributable to Irene Shaver. According to the Register of Deaths and her municipal death record she was not buried in the institutional cemetery but instead in Cooksville, ON. The inclusion of a female in a male row is also unusual. She should be removed from the cemetery registry.

7) A burial for 1419 occurs in row 18 plot 11. This is for Stella Marples. Her register of deaths entry does not indicate an institutional burial. Her municipal death record only records place of death as “Orillia” which is different from others of the time who received an entry of “Institutional Cemetery” for place of burial. Therefore it is uncertain whether she was buried in the institutional cemetery.

8) Row 19 Plot 14 contains a blank grave, with no death registration number noted. It could be that this represents a grave for 1607 (Victor Archibald) who is recorded on register of death and municipal death record as having been buried in institutional cemetery. There is no other entry on the cemetery for 1607. The burial of him in a female row (row 19) would have been odd, however.

9) Row 23 Plot 14 contains a blank grave, with no death registration number noted. It could be that this represents a grave for 2348 (Mary Lear) who is listed in record of deaths as having

an institutional burial. Her death record also indicates such. There is no other mapping entry for her.

10) Row 19 plot 31 contains an entry for 1546. The number is attributed to Charles McGregor, whose records indicate he was buried in Chatham, ON. Should be removed from cemetery registry.

11) Row 18 Plot 13 contains a grave marked 1412 (Sherlock Johns). This individual was buried in Exeter, ON and should be removed from registry.

12) Row 18 Plot 28 contains a blank grave. It could be associated with 1359 (Louisa Lucy Smith). Records confirm she received an institutional burial but she receives no other entry on the map.

13) Row 6 Plot 29 contains an entry for 1423 Stanford Edgar Bailey. Records indicate he was buried in . Therefore he should be removed from registry.

14) Row 6 Plot 30 contains an entry for 1408 Elville McArthur. Records indicate he was buried in Bruce County. Therefore he should be removed from registry.

15) Row 4 Plot 27 contains an entry for 1653 John Bedard. Records indicate he was buried in Kingston. Therefore, he should be removed from registry.

16) The patients transferred from the Whitby Psychiatric institution who were buried in the institutional cemetery seem to have been put off in their own section, rather than being buried amongst the general population. Their graves are illustrated much wider than the rest, within row 24 and occupy a chronological sequence from 1915 to 1919. This row was filled in at the south end with additional mixed male and female burials from 1932 and 1933, once the male section to the south of the corridor was filled up.

17) 43 individuals have no map entry. These individuals are all some of the earliest burials and listed on the Register of Applications for Admission. Their burial locations likely correlate to the blank spots noted in the first few rows of the cemetery (row 11 and 12 for males and row 13 and 14 for females). They are as follows:

18) A large number of individuals have no map entry because their burials post-date the last old cemetery map and pre-date the creation of the new cemetery and its map. XVII) Source: HRC Cemetery Map 1953-1971

Summary of Content

This map was authored sometime after the creation of the new and most northerly section of the cemetery around 1953. The first burial was that of Georgette Fournier (death registration number 3315) who died in 1953, with the last being Renee Silver (death registration number 4246) who died in 1971.

The map depicts an ordered set of rows of individual grave locations, with the rows extending from south to north, with the earliest burials in each row being in the south. The map shows 10 complete rows of graves, with the westerly rows being the earliest. The number of individuals per row varies and there appears to be no set logic as to when a new row was started. The rows begin just north of the set of mature trees that mark the boundary between the new and old cemetery areas.

The map depicts individual burial locations, upon each of which is recorded the name of the deceased, birth and death years, plot number in the row and death registration number. The writing and depiction of the grave plots is consistent for the first or more westerly four rows of graves. This indicates that one individual recorded the information for these rows and, based on the consistency of the hand writing, this was likely done in a single sitting sometime in 1962. I would read this as another indication that there was an original set of documents recording this information and that the information was transcribed at a later date. At the end of row 4 and continuing for the rest of the cemetery, the hand writing varies considerably from time to time. This would suggest that the individual responsible for recording the information changed from time to time. That said, consistency in the writing of individual recorders gives the impression that the map was updated every once and awhile (i.e., information for a set of graves was entered at a sitting), rather than immediately as a new burial was made.

It should be noted that no plot numbers for the more recent rows were entered and that recording methods were less meticulous than during the earlier period of this map. The latter entries appear to have been made on stickers which were then affixed to the appropriate plot locations on the map.

Issues with Interpretation and Other Issues to Note

There are several issues with this document:

1) Of note is the burial of George Bunn (d. 1967) at the extreme east end of the cemetery. He has a large monument in the field marking his burial, which is not within a defined row and set away from other burials. Why George was buried away from others in the cemetery is not known.

2) A label at the end of Row 8 lists “Orey Cecil Chisholm 1913-1968; death registration 4143). It is oriented in the opposite direction as the other burials in this row. In the field, his stone occurs between that of Fisher and Hill in this row, indicating that an error has been made on the map. This error is identified on the map with an asterisk in the correct position of the Chisholm burial, which is accompanied by a small hand written note indicating that that is where Chisholm is buried.

3) There is an entry at the end of row 6 for Florence McCready. It is oriented opposite to the rest of the burials in this row. The entry is struck through. However, the grave stone for McCready in the field is placed at the beginning of Row 7, where is yet another label with her information. This indicates that the entry at the end of row 6 is in error.

4) There is an entry at the east end of Row 6 for Robert McCauley (died 1965). The entry is oriented opposite to the other graves in this row. There is an arrow pointing from this label to plot 29. His name is listed with an institutional burial in the Record of Deaths. No stone could be found in the field for McCauley. Based on this map, it would appear that McCauley was buried next to Peter Hodgson and as the last in row 6, although there is no field stone associated with him.

5) Plot 33 in Row 5 is left blank. This is odd given that it is a numbered plot. The record of deaths show no other individual between Nora May Hone (last in row 5) and John Czze (Cize), the first in Row 6. Therefore it is very likely that there was, in fact, no burial made in this plot.

6) Row 4 Plot 31, 32, 33 entries for death registration numbers are missing.

7) In the field there is a stone for Fred Corby with James Corby. Fred Corby died in 2000 (not at HRC) and his ashes were placed next to his brother’s grave at the request of family.

8) Angelina Rizzo (d. 1962) in row 5 plot 31 has been disinterred.

9) The burials in row 9 are out of chronological order. In other words, the earliest burial in the row is not in the south, with the most recent in the north. The reason for this is unknown.

10) Some names are misspelled on this map (e.g., row 3, plot 23 – should be Tauno Merlainen).

11) There are a number of errors in Row 2. These are attributable to mistakes made in the record of deaths in the attribution of death registration numbers. The numbers were out by one number and then later corrected in the record. However, the errors have been transferred to the map,

so that the wrong individuals are named in the plots. Based on the register of death and municipal burial record notations of place of burial, the follow corrections should be made:

Conclusions and Recommendations

A review of the source material regarding interments made in the Huronia Regional Centre Cemetery revealed the following:

1) There is a significant data set pertaining to burials made and various documents were used in an attempt to track these interments. For a late 19th and early 20th century cemetery, these records are generally good and far more comprehensive than those for other cemeteries of this period. It is generally the case that cemetery records are poor prior to the implementation of more thorough systems of tracking in the later 20th century. While it is true that errors in recording were made and the sources are not perfect, this is not uncommon for the period.

2) The earliest documents review depict an institution that was trying to establish a formal system of record keeping with respect to burials made in the cemetery. Staff were creating their own system for tracking at a time when no formal system (or previously defined way of tracking) was in place.

3) The tracking system used by the institution seems to have improved greatly through time, particularly once the record of death was updated to include a column for place of burial. There is variability in tracking through time given differences in the recording forms used in the record of deaths, the information collected, the individual(s) doing the recording and so on.

4) The cemetery maps do not appear to be “living” documents in the sense that they were not updated immediately upon the additional of a new burial. Instead, they appear to have been updated periodically, with numerous burials entered at one time. The earliest cemetery map appears to have been yet another and different attempt by institution staff to create a better record of location of interments than just the written inventories that were on file. Again, this indicates a conscious effort to keep a good record of interments.

5) Due to a misinterpretation of some of the data sources (particularly the spelling of names from the record of death) and incorrect transcription of some information in places, the MCSS cemetery registry contains some errors. Corrections for these errors have been suggested in the supporting documents associated with this report.

6) For many periods (particularly the earliest use of the cemetery), the record of death, which includes all individuals who died at the institution, does not indicate place of burial (i.e., institutional or not). For individuals not listed on MCSS’ cemetery registry, outside source information was not used to confirm place of burial as this was out of scope for this project. When municipal records were available for review and could be used to confirm place of burial for the individuals listed in MCSS’ cemetery registry, these were checked and found to be a very useful source of information to assist in this review. They could be further utilized.

7) There are good records for interments being made by 1899. The previously recorded earliest burials listed in the MCSS cemetery register were recorded as occurring in 1876-1877. However, the evidence suggests many of these burials can be attributed to burials of patients who had been temporarily transferred from the Whitby Psychiatric institution in February 1917, with internments occurring between 1917 and 1919. These internments were recorded in a different manner from the institution’s established practices as the institution was apparently trying to keep an administrative separation between the Huronia and Whitby patients. TMHC feels that there is no good evidence in these records of burials prior to 1899 and the date of the opening of the cemetery remains in question.

The following recommendations are made:

1) The Cemetery Register should be updated to incorporate the required corrections.

2) Further investigation should be undertaken to attempt to resolve some of the few outstanding issues that arrived out of the review of the record of death. Many of these issues are small items (e.g., confirmation of name spelling) that could be quickly resolved from a review of other patient records. Other items require more investigation, as outlined below.

3) If desirable and to achieve a more accurate Cemetery Register, municipal death records (where available and useful) could be examined for the full period of cemetery operation to further confirm the accuracy of the cemetery register. Although the municipal records change over time in terms of the details they include, they were found to be helpful for providing clarification for a good period during which the cemetery operated. The records were also helpful in verifying name spelling. Further misspellings in the existing Cemetery Register might be identified through a cross-check to Municipal death records.

4) Additional background research could be undertaken to more firmly establish the exact date the cemetery was opened. A review of background records would seem to indicate that there is good evidence of the use of the cemetery by 1899 but very little firm documentation of interments prior to that time. A search of land registry records, inspector’s reports and internal institutional records could potentially provide more information about the founding date. Concrete information about the date of cemetery founding would help alleviate uncertainties that there were interments prior to 1899.
