All rights reserved 1008-682X/12 $32.00 www.nature.com/aja REVIEW Male factor infertility and ART Herman Tournaye For years, the management and treatment of male factor infertility has been ‘experience’ and not ‘evidence’ based. Although not evidence-based, current clinical practice involves extensive use of assisted reproductive techniques (ART). Where specific treatments are not indicated or have failed, ART have become popular adjunctive treatments for alleviating male factor infertility. According to the limited evidence available, intrauterine insemination (IUI) may be considered as a first-line treatment in a couple in which the female partner has a normal fertility status and at least 13106 progressively motile spermatozoa are recovered after sperm preparation. If no pregnancy is achieved after 3–6 cycles of IUI, optimized in vitro fertilization (IVF) can be proposed. When less than 0.53106 progressively motile spermatozoa are obtained after seminal fluid processing or sperm are recovered surgically from the testis or epididymis, intracytoplasmic sperm injection (ICSI) should be performed. Although the outcome of no other ART has ever been scrutinized as much before, no large-scale ‘macroproblems’ have as yet been observed after ICSI. Yet, ICSI candidates should be rigorously screened before embarking on IVF or ICSI, and thoroughly informed of the limitations of our knowledge on the hereditary aspects of male infertility and the safety aspects of ART. Ideally, there is a large concentration of ‘motile’ sperm, which means there are a lot of little guys alive and swimming around. We then turn the microscope up to a higher power and examine a few of the sperm individually to see that they have developed correctly and do not have any abnormalities that would impair their ability to fertilize an egg. One thing that I try to emphasize when fertility testing is that it is simply a snapshot of the bull’s performance at this point in time. SCOPE X Professional X Facility DESCRIPTION Infertility is defined as one year of frequent unprotected intercourse during which pregnancy has not occurred. Often the diagnosis and the procedures related to the diagnosis of infertility are considered medical services because the basis may be medically based, and not infertility based. Once the medical condition has been resolved and active infertility treatment begins, the services are then considered covered under the infertility benefit. There are basic medical services related to infertility that are not defined as infertility because the overlying cause is a metabolic disorder, or may be related to other disease process. Procedure codes are defined as infertility services by the diagnosis reported. POLICY Infertility and Reproductive Services do not require prior authorization if a member has the reproductive infertility benefit. (See terms of coverage below) COVERAGE CRITERIA Medical services/procedures for individuals related to the treatment of infertility include (not an all-inclusive list): 1. Specific drug treatments related to infertility (most drugs are provided through the member’s pharmacy benefit and not medical benefit) A.
