Purpose of this handout Congratulations on making it to Year 2 of medical school! You are that much closer to having your Doctor of Medicine degree. If you want to PRACTICE medicine, however, you have to be licensed, and in order to be licensed you must first pass all four United States Medical Licensing Exams. This book is intended as a starting point in your preparation for getting past the first hurdle, Step 1. It contains study tips, suggestions, resources, and advice. Please remember, however, that no single approach to studying is right for everyone. USMLE – What is it for? In order to become a licensed physician in the United States, individuals must pass a series of examinations conducted by the National Board of Medical Examiners (NBME). These examinations are the United States Medical Licensing Examinations, or USMLE. Currently there are four separate exams which must be passed in order to be eligible for medical licensure: Step 1, usually taken after the completion of the second year of medical school; Step 2 Clinical Knowledge (CK), this is usually taken by December 31st of Year 4 Step 2 Clinical Skills (CS), this is usually be taken by December 31st of Year 4 Step 3, typically taken during the first (intern) year of post graduate training. Requirements other than passing all of the above mentioned steps for licensure in each state are set by each state’s medical licensing board. For example, each state board determines the maximum number of times that a person may take each Step exam and still remain eligible for licensure. -

Before the mid-twentieth century, the treatment of tuberculous spondylitis was primarily supportive and typically resulted in dismal neurological, functional and cosmetic outcomes. The contemporary development of effective antituberculous medications, imaging modalities, anesthesia, operative techniques and spinal instrumentation resulted in quantum improvements in the diagnosis, manage- ment and outcome of spinal tuberculosis. With the successful treatment of tuberculosis worldwide, interest in Pott disease has faded from the surgical forefront over the last 20 years. With the recent unchecked global pandemic of human immunodeficiency virus, the number of tuberculosis and sec- ondary spondylitis cases is again increasing at an alarming rate. A surgical revisitation of Pott dis- ease is thus essential to prepare spinal surgeons for this impending resurgence of tuberculosis. Purpose: To revisit the numerous treatment modalities for Pott disease and their outcomes. From this information, a critical reappraisal of surgical nuances with regard to decision making, timing, operative approach, graft types and the use of instrumentation were conducted. Study design: A concise review of the diagnosis, management and surgical treatment of Pott disease. Methods: A broad review of the literature was conducted with a particular focus on the different surgical treatment modalities for Pott disease and their outcomes regarding neurological deficit, ky- phosis and spinal stability. -

Mycobacterium MODULE Microbiology 20 MYCOBACTERIUM Notes 20.1 INTRODUCTION Mycobacterium are slender rods that sometimes show branching filamentous forms resembling fungal mycelium. In liquid cultures they form a mould-like pellicle. Hence the name ‘mycobacteria’, meaning fungus like bacteria. They do not stain readily, but once stained, resist decolourisation with dilute mineral acids. Hence they are called ‘Acid fast bacilli’. They are aerobic, nonmotile, noncapsulated and nonsporing. Koch (1882) isolated the mammalian tubercle bacillus and proved its causative role in tuberculosis. In humans tuberculosis is caused by mycobacterium tuberculosis and also by bovine type called Mycobacterium bovis. The second human pathogenic mycobacterium is the lepra bacillus causing Leprosy. The third group of mycobacterium is a mixed group from varied sources like birds, cold-blooded and warm blooded animals, from skin ulcers, soil, water and other environmental sources. They are called as atypical mycobacteria. They are opportunistic pathogens and can cause many types of diseases. MICROBIOLOGY 203 MODULE Mycobacterium Microbiology 20.2 MYCOBACTERIUM TUBERCULOSIS Morphology M tuberculosis is a straight or slightly curved rod, about 3 X 0.3 µm in size, occurring singly, in pairs or as small clumps. M bovis is usually straighter, shorter and stouter. Tuberculous spondylitis affect the cervical spines in 2 cases (3.45%), the upper thoracic in 10 cases (17.24%), j\4id ' thoracic 20 times (34.48%), lower thoracic 20 cases (34.4S%), lumber spines 6 cases (10.35%) and no lesion in the sacral spines. -

We report a case of lung tuberculosis (TB) and cervical Pott’s disease mimicking a metastatic lung cancer. The patient presented with persistent cervical pain. Radiologic examinations showed the presence of a lytic lesion of C3 vertebral body, associated with spinal cord compression. A CT scan of the thorax showed a lung nodule highly suspicious for malignancy in the apical region of right lung upper lobe. Neurosurgical decompression was performed. Unexpectedly, histological analysis showed the presence of an inflammatory infiltrate suggestive for TB infection. The patient was immediately treated with antituberculous drugs. Atypical forms of spinal TB, such as cervical TB, can be misdiagnosed as primary or metastatic cancers and lead to delay of treatment initiation that could be fatal. Awareness of this uncommon TB presentation is important to prevent morbidity and mortality associated with spinal cord injury and disease dissemination. PENMAN Department of Medicine, University of Kentucky Medical Center, Lexington, Kentucky Blastomycosis is a specific granulomatous disease which tends to be chronic and indolent. It frequently presents in extrapulmonary form by means of haematogenous dissemination from the lungs. It has been shown that tuberculosis, histoplasmosis and coccidioidomycosis are, in the majority of cases, mild and subclinical in effect and often heal without therapy. It is probable that blastomycosis behaves in a like manner. The exact mortality is not known but is probably in the range of 13% in hospitalized cases with disseminated disease (Blastomycosis Cooperative Study of the Veterans Administration, 1964). The most effective form of therapy in active disease is amphotericin B; 2-hydroxy-stilbamidine is also used. Blastomycosis has largely been considered to be a disease of the American continent. However, cases have been reported from Africa and Europe and therefore a wider appreciation of this disease is considered pertinent.
