Educational Rounds 2020 9 #TEACHwebinar Learning Objectives

1. Explain how a tobacco cessation program (STOP Program) was adapted to meet the needs of employees at a rural worksite. 2. Identify some of the challenges and opportunities associated with the delivery of smoking cessation services at worksites.

Educational Rounds 2020 10 #TEACHwebinar The Spark Spring of 2017 • The upcoming legalization of cannabis sparked interest and concern at Alamos Inc., who had in place a strict no smoking (cigarettes) policy except for in designated areas. • Alamos management approached their Occupational Health Nurse to develop a plan to offer tobacco cessation services to interested employees. • The Occupational Health Nurse reached out to Algoma Public Health Wawa office.

Educational Rounds 2020 11 #TEACHwebinar The Need • Algoma District has high smoking rates* • Algoma Public Health (APH) collaborated with community partners to reduce smoking rates by 5% in 5 years (2020). • Alamos Gold Inc. is a partner. • The Occupational Health Nurse estimated the smoking rate at Alamos at approximately 38%. • At Alamos, smoking was taking place in areas other than the designated smoking areas.

Educational Rounds 2020 12 #TEACHwebinar Wawa APH Smoking Cessation

• Emma Blanchet – Psychiatric Case Manager – Community Mental Health Services • Amanda Gauthier – Public Health Nurse • Role of APH Wawa cessation to fill a gap, not duplicate services in the community. • Wawa, cessation clinic had very few clients.

• Employer drug coverage benefits for nicotine replacement therapy required a physician’s prescription, and was limited. • Limited access to a pharmacy to purchase nicotine replacement therapy products during work schedule. • Employees requested tobacco cessation services. – Scheduled at worksite – Convenient appointments

Educational Rounds 2020 17 #TEACHwebinar Challenges

• Employee schedules vary, e.g. some shift workers do 7 in 7 out at a time. • Employees from all over the area, Dubreuilville, Wawa, White River, Blind River, etc.

Educational Rounds 2020 21 #TEACHwebinar The Pitch

• Business case to Employer (Alamos Gold Inc.) – Determine how many employees would use program – Cost – No smoking policy – easier to support with a tobacco cessation program available. – Healthier employees = ++ benefits, productivity, less absenteeism etc. • APH goal was to help initiate and support Alamos in the development of a workplace smoking cessation program.

• Flexible APH staff schedule • Mental Health Worker involved in project • Creative with the timing of the program and follow up phone calls • Try something new & connection with a workplace • Collaborate with partners (Occupational Health Nurse)

Educational Rounds 2020 25 #TEACHwebinar Short Term Goals

1. Assist current smokers with a quit attempt – Objectives: • enroll 8 employees per site visit (16 employees total) • To achieve a 50% ongoing participation rate throughout the project 2. Implement modified tobacco cessation services and determine: – Cost effectiveness – Efficiency – A snapshot of behaviour patterns of a sample of employees 3. Develop a project summary report

Educational Rounds 2020 26 #TEACHwebinar How Did We Do?

• 21 employees enrolled – 10 participants completed the first follow up phone call – Phone call follow up declined after first call • second phone call completed by 4 employees – 5 employees reported that they quit smoking • OTRU conducted interviews with Occ. Health Nurse and APH smoking cessation staff: – 2 interviews: 2-3 months & 6 months • Project summary report shared with Alamos Gold Inc. and APH. The report highlighted: – project implementation – amount of NRT required to support the program – considerations for moving forward

Educational Rounds 2020 27 #TEACHwebinar Long Term Goals

1. Assist Alamos Gold Inc. to initiate and develop the capacity to implement an onsite smoking cessation program. 2. Identify the support needed for the Occupational Health Nurse to deliver cessation services onsite. – Development of a Medical Directive for NRT (Employer Physician) – Employer provide funding to support the cost of NRT

Educational Rounds 2020 28 #TEACHwebinar How Did We Do? • Alamos continues to be an active 5 in 5 partner • Collaboration – APH available as needed for ongoing support • APH provides support with referrals to available smoking cessation programs (e.g. connecting with other health units and health care providers). • Ripple effect – Occupational Health Nurse reported that the program helped to create important discussions, as well as helped to build awareness about tobacco cessation and the available supports.

Educational Rounds 2020 29 #TEACHwebinar Employee Feedback

• Unaware of the proper use and the importance of titration (e.g. amount of NRT to use) • Troubleshooting patch adherence (e.g. sweating) • Increased awareness about short acting nicotine products (e.g. use with cravings) • Increased awareness of nicotine toxicity if smoking while using patches & short acting NRT • Did not feel pressured to quit • CO monitor measures effects of smoking • Appreciation for the program

Educational Rounds 2020 30 #TEACHwebinar Lessons Learned • Workplace buy in and partnership at the outset. • Occupational Health Nurse involvement was key. • Talking about smoking cessation doesn’t do anything unless you are prepared to help. • Intervention will be more successful in workplaces where employees are ready to quit. • Brief is best. • Meeting people more than halfway to make things equitable is ok.

Educational Rounds 2020 31 #TEACHwebinar Lessons Learned

• Don’t expect 100% success rate • Convenience of program was appealing • Long term goals are a work in progress • Humbling experience – industry is essential to this community
