JOB DESCRIPTION

SUMMARY:

The advertised post is for a full time General Radiology Consultant with an interest in oncology and GI Imaging at Barts Health NHS Trust. We are looking for a highly motivated and enthusiastic individual to join our dynamic team of radiologists to help deliver the service and to support the active training program. The post will be predominantly based at the Royal and St Bartholomew’s .

The Royal London is a major trauma centre and Barts Health one of the largest Trusts in Europe.

The successful candidate should be able to demonstrate excellent general emergency and onco-radiology skills and have extensive experience in all aspects of cross sectional imaging including ultrasound, trauma imaging and plain film reporting.

A specialist interest in GI/HPB oncology imaging is preferred You will join our dedicated team of 32 general and 3 dedicated GI Radiologists who currently support general Imaging services as well as the rapidly increasing demand for liver and GI imaging particularly in relation to CT and MR Imaging, GI fluoroscopic procedures and supporting the specialist MDT’s.

The HPB Unit at Barts Health is based at the Royal London site. The unit provides a tertiary referral service receiving around 300 referrals per year for HPB disease, and the weekly MDT usually covers 35-40 patients. The surgical team carries out around 100 resections per year for pancreatic cancer, and a similar number for liver and biliary cancer. In addition to surgical resection, patients receive other treatments such as radiofrequency ablation and arterial chemoembolization.

The Gastroenterology unit is based at the Royal London and outpatients at St Bartholomew’s Hospital. The unit works closely with similar units at Whipps Cross Hospital and Newham Hospital following the merger in 2012 into Barts Health. The Digestive Disorders Clinical Group integrates luminal and pancreaticobiliary gastroenterology, hepatology, IBD and nutrition. There are close links with surgery, pathology and radiology and the clinical department of paediatric gastroenterology. The unit sees 8000 out-patients, performs ~7000 endoscopic procedures and admits~500 patients annually. The department takes part in the National Bowel Cancer Screening Programme.

There is a large oncology unit at Barts including uro-oncology, gynae-oncology, haematological malignancies, breast, melanoma and thoracic malignancies as well as GI and head and neck malignancies. The successful candidate will form part of an existing team of 13 consultant radiologists in oncology and breast imaging. They will have the opportunity to develop subspecialty interests in any of the above areas.

The successful candidate should be able to demonstrate excellent experience in general imaging, oncological imaging, and acute general radiology, including trauma. PET CT experience is desirable but not essential.

The successful candidate will be part of a team providing radiological expertise to the new Early Diagnostic Centre at Hospital, predominantly focussed on screening chronic liver disease.

The candidates will participate in a general on-call and weekend reporting session rota on a 1 in 20 basis.

Barts Health has a large number of clinical trials supported by the imaging department. Successful candidates will be expected to participate in RECIST/ mod iRECIST/CHESON trial reporting for oncology clinical trials.

At Barts and the London Hospitals there is a highly successful training programme with 35 registrars. The successful candidate will be expected to be trainers for the training programme.

The successful candidate will need to demonstrate professional excellence and the ability to deliver effective care. They will need to agree levels of activity and throughput with the Imaging Divisional Director and General Manager. These activity levels will be aligned to capacity and demand of the department. They will be required to adhere to Trust’s policy on Maintaining Medical Excellence and to be committed to maintaining personal standards of performance by keeping their knowledge and skills up to date.

General Duties

1. Continuing responsibility for the care of patients in his/her charge, including all administrative duties associated with patient care.

2. Leadership, development and organisation of the services in his/her charge in line with the Trust’s business plans. Liaison and communication with the Clinical Directors in Imaging, the Imaging General Manager and the Clinical and Support Service Clinical Group Directors.

3. Active participation in audit programmes.

4. Corporate and individual responsibility for the professional management of doctors in training.

Duties of the Post

1. To support, develop and enhance the general imaging service at Barts Health NHS Trust.

2. To contribute to the high-quality consultant-led diagnostic radiology service at St. Bartholomew’s and The .

3. The post holder will be required to participate in a general on-call rota (1 in 20) including reporting emergency CT’s at the weekend for 3 hours each day (1 in 20)

4. To support other imaging developments within the Trust

5. To provide evidence-based care for all patients.

6. To have adhered to at all times the appropriate clinical and quality standards

7. To liaise effectively and on a timely basis with colleagues, General Practitioners, community services, and all healthcare agencies.

On-Call Commitments

There is a general on-call commitment for this post. There is a general on-call rota (1 in 20) with the SPR first on-call. This will attract a 3% supplement and time off in lieu for weekend reporting. Other separate on-call rotas include Paediatrics, Neuroradiology, Neurointevention and Interventional radiology.

Accountability The appointees will have full professional independence on medical matters but will be managerially accountable to the Barts Health Chief Executive, through their Clinical Director.

Clinical Governance

The post holders are expected to participate in the clinical effectiveness activities and to encourage and foster improvements in the quality and standards of clinical services. The post-holders will assist with the safeguarding of high standards of care by participating in the creation of an environment in which excellence in clinical care will flourish.

Continuing Professional Development and Continuing Medical Education

The post holders are expected to take responsibility for their own Continuing Professional Development and take part in Continuing Medical Education activities, in line with the requirements of the Royal College of Radiologists guidelines to maintain standards of performance and in consultation with the Clinical Director.

The post-holders will be expected to be committed to the concept of lifelong learning and produce and maintain a Personal Development Plan in agreement with their Clinical Director.

Teaching and Training

The post-holder will assist with the drawing up of training agreements for foundation trainees, core medical trainees and specialist trainees in radiology across the Trust and with the delivery of training specified in the agreement. Salary and Conditions of Service

General Information

The post holder will be expected to work closely with the Head of Imaging, the Clinical Directors and other colleagues as appropriate across the Trust in the management and administration of resources and to be active in promoting changes which will improve the delivery of service to patients.

Imaging Service

Barts Health NHS Trust is a largest teaching hospital in Europe situated across six sites, providing comprehensive medical care to the people of East London.

The hospital sites that make up Barts Health are:

St Bartholomew’s Hospital, The Royal London Hospital, , Whipps Cross Hospital and Newham University Hospital.

Barts Health is committed to a model of medical management, which involves clinicians and has established a cross-site clinical academic unit structure to provide the framework for the

management of services. This ensures that decisions regarding performance, quality and use of resources are taken by those most closely involved in service delivery. A key feature is that they are integrated across the various hospital sites.

The site Clinical Directors, Divisional Director & Network Lead and Head of Imaging share responsibility for the overall performance of the Imaging Service. The Imaging Service is part of the Group Clinical Services (GCS) with Pathology, Clinical Physics, Therapies, out patients, medical records and Pharmacy formed in October 2012. The GCS Acting Managing Director is Jan Flint with Group Divisional Director and network Lead, Dr Matthew Matson. Each hospital has a Clinical Director, Dr Emma Friedman at The Royal London and Dr Stephen Ellis at St Bartholomew’s Hospital. There is a management team that includes a general manager who is supported by finance, human resource and planning and commissioning managers.

The Imaging Board meets monthly and the Clinical Directors attend this along with the Modality managers and the management team.

Appraisal is performed annually by the Clinical Director or by a senior consultant who has attended the Trust’s appraisal course. A record of annual and study leave forms part of the appraisal. Annual review of the job plan and objective setting with the Clinical Director are separate from appraisal.

ACADEMIC TEACHING

The Radiology Departments at St Bartholomew’s and The Royal London Hospital are approved for all levels of training by the Royal College of Radiologists. There are currently 35 junior Radiologists including Fellowship posts in Paediatric Radiology, Breast Imaging, MRI, Neuroradiology and interventional Radiology. Dr Deborah Low is the training programme director.

AUDIT

Audit is well established and takes place once a month (in association with the regular teaching day) and looks at specific clinical activities. The meetings are chaired by one of the consultant radiologist and presentations are given by all staff in strict rotation.

RESEARCH

We work collaboratively with our academic partners, especially those at Barts and The London School of Medicine and Dentistry and at St Bartholomew School of Nursing and Midwifery, to support the effective planning and commissioning of education to meet the changing workforce needs of healthcare organisations across north-east London in the future. Closer alignment of research, teaching and clinical services means that increasing numbers of patients at our hospitals will be some of the first to receive new treatments and cures.

Investigator led research is encouraged with regular research meetings and academic support. Library facilities are available, within The Royal London Hospital X-ray departments, as well as the Royal London Hospital Medical School, St Bartholomew’s Hospital and Queen Mary’s and Westfield College (basic sciences). The post holder will be expected to maintain professional expertise by attendance at meetings and courses. Barts Health has a large number of clinical trials supported by the imaging department. Draft Job Plan *The MDTs will be on a 1:4 rotation and the post holder will be expected to provide alternate DCC activities as above when not conducting or preparing the MDTs. The general on call is 1:20 with 3 hours of approving SPR’s emergency reports each on a Saturday and a Sunday. This is remunerated by time off in lieu and a 3% up lift.

Management

Candidates will be expected to take part in the running of the department. You will be asked to obtain an e-mail address on the Trust Intranet and to set aside time each working day to answer your e-mails. You will be expected to take a full part in the appraisal process. There are departmental and clinical meetings (e.g. local consultant meetings) that you will be expected to make every effort to attend. You will also be encouraged to take part in the Trusts annual planning cycle, contributing to activities such as prioritising the departments’ capital bids.

Other, Flexible Duties

Beyond these tasks all consultants are expected to take a special interest in one of the department's areas of interest and contribute to the delivery of the department objectives. Below are listed number of these activities and candidates will be asked in which particular area they would be interested in participating:

Participation in the Department's training and trainee appraisal process. This important exercise, vital to the success of training, requires a team of consultants committed to carrying it out. Training will be provided if required, by the Trust's own Appraisal and Assessment course.

Participation in the Directorate's managerial activities. The large size of the Service Group requires a significant managerial input. It is vital that clinicians play a major role in this function, and there are numerous areas in which interested consultants can play a part. Participation in the Department’s research activities. Above and beyond that expected from all consultants. There is a continuing need for senior staff to produce ideas for research projects and supervise their development through all stages from ethical approval to publication, and to actively involve other members of the Departments, particularly the trainees in this process. Advice and supervision also need to be provided to trainees for their own research ideas.
