GENERAL PRICE LIST

These prices are effective as of October 1, 2018, but are subject to change without prior notice. The goods and services shown below are those we can provide to our clients. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

We at Evans Funeral Chapel have served families in this community for many years. We have seen a lot of changes in the way people conduct business, and have had to make many changes ourselves. In an effort to keep our prices as low as you can afford, and our services at a consistently high level, we have implemented a modified payment policy.

The Following Are Acceptable Payment Arrangements

§ Cash/Checks § VISA/MasterCard/Discover § Third-party financing with credit approval § An assignable, non contestable life insurance policy (there will be an additional processing fee of $225.00 on the unpaid balance for this method of payment.) § Estates. Due to the complexity and time involved we are unable to accept payments from estates. All payments are to be made prior to family viewing.

Our General Price List is complete with everything available through our firm. We have also included optional Service Selections designed to make your choices as simple and effortless as possible. If you are receiving this information because you recently experienced the loss of a loved one, please accept my sincerest condolences. If you have decided that planning ahead is important to you and your family, I know that you will find the pre-arrangement process to be one of ease and comfort.

Service/Facilities/Automotive Equipment Basic Services of Funeral Director and Staff ...... $3,525.00 Our fee for the services of funeral director and staff includes, but is not limited to, twenty- four hours availability, consultation with family; sheltering of remains; arrangement of funeral; preparation and filing of necessary authorizations and permits; recording vital statistics; service vehicle; preparation and listing of obituary notices; staff assistance prior to, and following the funeral, including coordination with those providing other portions of the funeral, e.g. cemetery, crematory and others. In addition, this fee includes a proportionate share of our basic overhead costs. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are fiberboard.

Funeral Service for Infant or Child up to Age 12 As a way of expressing our sympathy to young families at the death of a child or infant, current charges for any of our services requested will be reduced by 50%.

Professional and Staff Services for Disinterment/Reinterment ...... $1,450.00 Our fee includes basic services of funeral director and staff; presence of licensed funeral director to supervise disinterment and hearse or service vehicle to local cemetery or crematory for reinterment.

Pre-Need Price Guarantee

Some families wish to ensure the funds they pre-pay with us are guaranteed to cover the funeral home’s charges in the future, by paying a premium. The premium on the funeral home charges is 5%. We do not guarantee third party cash advance items.
