Majlis Perbandaran Port Dickson Negeri Sembilan Darul Khusus MAJLIS PERBANDARAN PORT DICKSON NEGERI SEMBILAN DARUL KHUSUS KENYATAAN KERJA UNDI 1. Makluman adalah kepada semua KONTRAKTOR KELAS F (G1) yang berdaftar di MPPD sahaja. 2. Kontraktor-kontraktor yang mempunyai kerja semasa di Majlis Perbandaran Port Dickson tidak akan dipertimbangkan. 3. Kontraktor-kontraktor hendaklah membawa sijil asal serta salinan sijil a) Sijil Perolehan Kerja Kerajaan b) Sijil SSM c) Surat Pengiktirafan Bumiputera dengan PKK d) Sijil Pendaftaran MPPD e) Perakuan Pendaftaran dengan CIDB 4. Kesemua Sijil di atas perlu mempunyai tempoh sah sekurang-kurangnya 1 bulan dari tarikh lawatan tapak. 5. Pada hari mengundi dijalankan, pendaftaran akan bermula pada jam 3.00 petang sehingga 3.45 petang. Kontraktor yang tiba selepas waktu yang ditetapkan tidak dibenarkan untuk mendaftar. 6. Sijil Asal Suruhanjaya Syarikat Malaysia (SSM), Sijil Asal Perakuan Pendaftaran Kontraktor (PKK), Sijil Perolehan Kerja Kerajaan (CIDB), Sijil Taraf Bumiputera (CIDB) dan Sijil Pendaftaran MPPD WAJIB dibawa bersama semasa pendaftaran. Hanya pegawai syarikat yang ditauliahkan seperti di dalam sijil Perolehan Kerja Kerajaan sahaja yang dibenarkan mengambil bahagian pada hari undian berkenaan. 7. Hanya SATU (1) wakil untuk SATU (1) Syarikat sahaja yang dibenarkan untuk mendaftar kerja undi. 8. PIHAK MPPD TIDAK AKAN BERTANGGUNGJAWAB KE ATAS KONTRAKTOR- KONTRAKTOR YANG LEWAT DAN TIDAK BERDAFTAR SYARIKAT TERLEBIH DAHULU. 9. Segala perbelanjaan menghadiri pengundian adalah di bawah tanggungjawab kontraktor sendiri.
