I understand that the handbooks contain information that my child and I may need during the school year and that all students will be held accountable for their behavior and will be subject to the disciplinary consequences outlined in the Parent/Student Handbook and the Student Code of Conduct. If I have any questions regarding the Parent/Student Handbook and the Student Code of Conduct, I should direct those questions to the principal at my child’s campus. Printed name of student: _________________________________________________________ Signature of student: ____________________________________________________________ Printed name of parent: __________________________________________________________ Signature of parent: ____________________________________________________________ Date: ________________________________________________________________________ Please sign and date this page, remove it from the handbook, and return it to the student’s school. ii Notice Regarding Directory Information and Parent’s Response(s) Regarding Release of Student Information The Family Educational Rights and Privacy Act (FERPA), a Federal law, requires that Tyler ISD, with certain exceptions, obtain your written consent prior to the disclosure of personally identifiable information from your child’s education records. However, Tyler ISD may disclose appropriately designated “directory information” without written consent, unless you have advised the District to the contrary in accordance with District procedures. The primary purpose of directory information is to allow the district to include this type of information from your child’s education records in certain school publications. Examples include: • A playbill, showing your student’s role in a drama production; • The annual yearbook; • Honor roll or other recognition lists; • Graduation programs; and • Sports activity sheets, such as for wrestling, showing weight and height of team members. Direct information [see Student Privacy Choice Notices, pg. iii], which is information that is generally not considered harmful or an invasion of privacy if released, can also be disclosed to outside organization without a parent’s prior written consent. Outside organizations include, but are not limited to, companies that manufacture class rings or publish yearbooks. In addition, two federal laws require local educational agencies (LEAs) receiving assistance under the Elementary and Secondary Education Act of 1965 (ESEA) to provide military recruiters, upon request, with three directory information categories – names, addresses and telephone listings – unless parents have advised the LEA that they do not want their student’s information disclosed without prior written consent. If you do not want Tyler ISD to disclose directory information from your child’s education records without your prior written consent, you must notify the District in writing by September 9, 2021 [See Privacy Option Form, page iv]. iii Student Privacy Choice Notices Directory Information Tyler ISD has designated the following student information as directory information: . Student’s name . Grade level . Address . Most recent school attended . Telephone listing . Participation in school activities and . Photograph sports . Video image . Weight and height of a member of an . Date of birth athletic team . Major field of study . Enrollment status . Email address . Student identification numbers or . Honors and awards received identifiers that cannot be used alone . Dates of attendance to gain access to electronic education records. The Privacy Option Form allows this choice to be made and must be returned to your student’s school to make your student’s information private. Release of Student Information to Military Recruiters and Institutions of Higher Learning Federal law requires that the district release to military recruiters and institutions of higher education, upon request, the name, address, and phone number of secondary students (9th through 12th grades) enrolled in a district, unless the parent or eligible student directs the district not to release information released without prior written consent, the Privacy Option Form allows this choice to be made and must be returned to your student’s school to make your student’s information private. Publications and Media Students who attend school at Tyler ISD are occasionally asked to be part of school and/or District publicity, publications and/or public relations activities. Information subject to disclosure includes student’s name, picture, art, written work, voice, verbal statements and/or portraits (video or still) to be used for public relations, public information, school or district promotion, publicity, and instruction. Pictures and articles may or may not personally identify the student. The pictures and/or videos may be used by the District in subsequent years and no monetary consideration shall be paid. The Privacy Option Form allows this choice to be made and must be returned to your student’s school to make your student’s information private. If you choose to restrict information on your student for media and publications, every effort will be made to restrict the information; however, state law permits a school to make a video or voice recording without parental permission for the following circumstances: . When it is to be used for school safety; . When it relates to classroom instruction or a co-curricular or extracurricular activity; or . When it relates to media coverage of the school. iv Privacy Option Form This form must be returned by August 30, 2021, or within ten school days of your child’s first day if you would like any of your student’s information to be kept private. Unless instructed by parent/guardian, Tyler ISD will release the designated (directory) information on your student. You can indicate your choices by returning this form. Privacy options for the previous school year are on file and changes to your student’s information will only be made if a new Privacy Option Form is placed on file. IF A PRIVACY OPTION FORM IS NEVER PLACED ON FILE FOR YOUR STUDENT, THE DESIGNATED DIRECTORY INFORMATION (See Student Privacy Choice Notices) WILL BE AVAILABLE UPON REQUEST (will not be held as private). YOU ONLY NEED TO RETURN THE PRICACY OPTION FORM, IF: 1. YOU WANT TO MAKE A CHANGE TO THE PREVIOUSLY FILED PRIVACY OPTION FORM; 2. YOU WANT YOUR STUDENT’S INFORMATION TO BE PRIVATE. A copy of the district’s Asbestos Management Plan is available in the central administration office and at each campus via district intranet. If you have any questions or would like to examine the district’s plan in more detail, please contact the district’s asbestos coordinator. Pest Management Plan The district is required to follow integrated pest management (IPM) procedures to control pests on school grounds. Although the district strives to use the safest and most effective methods to manage pests, including a variety of non-chemical control measures, pesticide use is sometimes necessary to maintain adequate pest control and ensure a safe, pest-free school environment. All pesticides used are registered for their intended use by the United States Environmental Protection Agency and are applied only by certified pesticide applicators. Except in an emergency, signs will be posted 48 hours before indoor application. All outdoor applications will be posted at the time of treatment, and signs will remain until it is safe to enter the area. Parents who have further questions or who want to be notified prior to pesticide application inside their child’s school assignment may contact the district’s IPM coordinator.
